BACKGROUND: Combined 'hybrid' thoracoscopic and percutaneous atrial fibrillation (AF) ablation is a strategy used to treat AF in patients with therapy-resistant symptomatic AF. We aimed to study efficacy and safety of single-stage hybrid AF ablation in patients with symptomatic persistent AF, or paroxysmal AF with failed endocardial ablation, and assess determinants of success and quality of life.
METHODS: We included consecutive patients undergoing single-stage hybrid AF ablation. First, we performed epicardial ablation, via thoracoscopic access, to isolate the pulmonary veins and superior c…

OBJECTIVES: The objective of this study was to determine course and treatment of atrial fibrillation (AF) in nonagenarians. Incidence of AF increases with age. Due to the demographic change in the industrialized world, an increase of AF in the group of elderly and very elderly is expected in the next decades. However, only few data exist on the clinical relevance of AF in patients aged 89 years or older.
DESIGN: Observational, mono-centric registry.
SETTING: University hospital.
PARTICIPANTS: Of the 11,888 patients included in the Berlin Atrial Fibrillation (BAF) Registry, 279 patients aged…

BACKGROUND: Cerebral infarction associated with atrial fibrillation usually causes permanent neurologic disability, but several studies also reported patients with silent cerebral infarcts on CT. The clinical relevance of this findings is unknown, partly because of variable criteria for the diagnosis of "silent" brain infarction.
METHODS: This report describes the frequency of silent brain infarcts in 985 patients with nonrheumatic atrial fibrillation, who shortly before suffered a symptomatic transient ischemic attach or nondisabling ischemic stroke, and evaluates the predictive value of sil…

Atrial fibrillation (AF) affects about 2% of the general population and 8%-11% of those older than 65 years. The demand for effective therapeutic strategies for AF is anticipated to increase substantially as the proportion of the elderly population increases. Atrioventricular nodal ablation accompanied by permanent pacemaker implantation is an established option in elderly patients with intractable arrhythmia and poor ventricular rate control. However, it renders most patients pacemaker dependent and does not eliminate symptoms associated with loss of atrial transport or reduce the risk of str…

Clinical examination of a four-and-a-half-month old foal revealed evidence of progressive heart failure associated with an atrial septal defect and atrial fibrillation. Atrial septal defects are rarely reported in foals and the foal's growth, development and demeanour were normal until a sudden onset of lethargic behaviour. The value of B-mode echocardiography in identifying the congenital lesion is indicated.

BACKGROUND: Association between the presence of left atrial low-voltage areas and atrial fibrillation (AF) recurrence after pulmonary vein isolation (PVI) has been shown mainly in persistent AF patients. We sought to compare the AF recurrence rate in paroxysmal AF patients with and without left atrial low-voltage areas.
METHODS: This prospective observational study included 147 consecutive patients undergoing initial ablation for paroxysmal AF. Voltage mapping was performed after PVI during sinus rhythm, and low-voltage areas were defined as regions where bipolar peak-to-peak voltage was <0.5…

BACKGROUND: An endoscopic transdiaphragmatic epicardial radiofrequency ablation procedure combined with percutaneous endocardial radiofrequency ablation--a hybrid procedure--is a potentially curative treatment option for patients with persistent atrial fibrillation (AF). Long-term effects of arrhythmia elimination on atrial and ventricular remodeling are not completely understood. Therefore, the aim of our study was to quantify echocardiographic structural and functional changes of the left atrium (LA) and left ventricle (LV) following a hybrid procedure.
METHODS AND RESULTS: Thirty-seven con…

BACKGROUND: Prior studies have suggested that left atrial (LA) volume and frequency of atrial fibrillation (AF) are associated with suboptimal outcomes in patients undergoing catheter ablation of AF. However, the interaction of these factors and their relative impact on outcome are not clear.
METHODS: Seventy-nine consecutive patients underwent catheter ablation of persistent AF. LA volume was determined by echocardiography. Electrograms from the LA appendage (LAA), coronary sinus (CS), and lead V(1) were obtained before ablation, and the dominant frequency (DF) was determined by fast Fourier…

The circadian variation of paroxysmal atrial fibrillation (AF) was studied in 67 patients who received a dual-chamber pacemaker 3 months before a planned atrioventricular node ablation. A distinct circadian variation of AF was observed with 2 time peaks in initiation (1 in the early morning and 1 in the early evening hours), which was modulated by atrial pacing, the duration of AF, and the use of beta-adrenergic blocking agents.

BACKGROUND: Recent studies have indicated an association between periodontitis and atrial fibrillation (AF), although the underlying mechanisms remain unclear. Porphyromonas gingivalis (P ginigivalis) is a causative agent of periodontal disease and is highly pathogenic. This study focused on P gingivalis and aimed to investigate the relationship among periodontitis, atrial translocation of P gingivalis, and atrial fibrosis and AF.
METHODS: An experiment was conducted using P gingivalis-infected C57BL/6J mice, in which P gingivalis was inoculated into the pulp of the molars. Immunohistochemist…

A 34-year-old woman presented with palpitations and paroxysmal atrial fibrillation (AF). Workup revealed anterior mitral valve prolapse with severe mitral regurgitation. She was referred for surgical repair and underwent a mitral valve replacement, tricuspid valve repair, and bi-atrial cryoMAZE procedure with left atrial appendage ligation. Her postoperative course was complicated by inferior wall myocardial infarction. She subsequently presented with palpitations and underwent electrophysiology study and ablation. This case illustrates pitfalls associated with the surgical MAZE procedure and …

A right atrial separation procedure was performed for the ablation of atrial fibrillation during the concomitant repair of an atrial septal defect. This procedure consisted of a Y-shaped incision in the right atrium, followed by cryoablation of the tricuspid annulus and the atrial septum without any procedures performed on the left atrium. This is a simple and effective method for the elimination of chronic atrial fibrillation associated with atrial septal defects in adults.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia, yet current therapies-including drugs and catheter ablation-remain suboptimal. Gene therapy offers a promising way to modulate AF's molecular drivers. This review summarizes recent preclinical studies using viral and non-viral vectors, atrial-specific delivery strategies, and key targets such as ion channels, fibrosis, and oxidative stress. Despite promising results, no AF gene therapy has FDA approval, due to challenges in atrial targeting, immune control, and durable expression. Closing this translational gap is critic…

Atrial fibrillation is a chronic and complex condition. The new 2024 European Society of Cardiology guidelines emphasize a holistic approach to managing atrial fibrillation, introducing the AF-CARE framework. The guidelines prioritize addressing (C) Comorbidities and risk factors for all patients, irrespective of their thromboembolic risk or symptoms. This is followed by strategies to (A) Avoid stroke and thromboembolism, and to (R) Reduce symptoms through rate and rhythm control, potentially improving prognosis. Finally, (E) Evaluation and reassessment should be individualized, with a dynamic…

Habitual physical activity improves cardiovascular health but there is a higher risk of atrial fibrillation (AF) in endurance athletes. The physiological processes underlying this observation are not fully understood, but adaptations to exercise, such as bradycardia and atrial dilatation, may contribute to arrhythmia susceptibility. Further data on long-term implications and individualised management strategies in athletes with AF are required.

Atrial fibrillation (AF) in the postoperative phase is a manifestation of numerous factors, including surgical stress, anesthetic effects, and underlying cardiovascular conditions. The resultant cardiac hyperactivity can induce new onset or exacerbate existing AF. A common phenomenon, postoperative atrial fibrillation (POAF) affects nearly 40% of patients and is associated with longer hospitalization stays, and increased mortality, heart failure, stroke, and healthcare costs. Areas of controversy in POAF include whether to anticoagulate patients who have short-lived POAF, especially given thei…

PURPOSE OF REVIEW: Cognitive dysfunction is a complex condition that is becoming increasingly more prevalent. There has been growing acknowledgement that individuals with atrial fibrillation are at an increased risk of cognitive dysfunction beyond the association of age with both disorders. The purpose of this review is to explore the potential underlying mechanisms connecting atrial fibrillation and cognitive dysfunction and to examine the existing evidence for potential treatment options.
RECENT FINDINGS: Many mechanisms have been proposed for the association between cognitive dysfunction a…

Atrial fibrillation disrupts contraction of the atria, leading to stroke and heart failure. We deciphered how immune and stromal cells contribute to atrial fibrillation. Single-cell transcriptomes from human atria documented inflammatory monocyte and SPP1+ macrophage expansion in atrial fibrillation. Combining hypertension, obesity, and mitral valve regurgitation (HOMER) in mice elicited enlarged, fibrosed, and fibrillation-prone atria. Single-cell transcriptomes from HOMER mouse atria recapitulated cell composition and transcriptome changes observed in patients. Inhibiting monocyte migration …
