[Fenton's syndrome].

The authors report two recent cases of Fenton's syndrome, a very rare carpal fracture-dislocation. After some anatomico-physiopathological considerations and a review of the literature, a wider nosographic frame is proposed in which the entity of the dislocation of the head of the capitate bone is not essential. According to both the literature and the personal findings, the authors remark that this syndrome is always found in the presence of two morphological variants of the distal radioulnar joint. Finally, the authors stress the importance of a correct diagnosis of this lesion to avoid unne…

The distribution of bone islands and juxta-articular bone cysts in the growing hand.

Bone islands and juxta-articular bone cysts are relatively common incidental findings when X-rays are taken for other purposes. We have identified that the incidence of bone islands in the hands of asymptomatic children between the ages of 5 and 13 years is 3.8% and the incidence of cysts in the same population is 2.8%. Bone islands were most common in the scaphoid, whilst juxta-articular bone cysts were most common in the capitate. The age at which they appear has not been reported previously. This study first identifies their presence in the hands of children of age 10 years and 2 months (bo

Scaphocapitate fusion for the treatment of Lichtman stage III Kienböck's disease. Results of a single center study with literature review.

The purpose of this study was to report the outcomes of scaphocapitate fusion without lunate excision for the treatment of stage III Kienböck's disease and to compare these results with historical results of limited wrist arthrodesis and proximal row carpectomy. Clinical and radiographic evaluations were performed preoperatively and at a mean of 5.8-years' follow-up (range 1.5 to 10.5years) on 17 patients with advanced Kienböck's disease (Lichtman stages: IIIA n=4 and IIIB n=13) treated by scaphocapitate fusion without lunate excision between January 2000 and July 2015. The average DASH score

Mid-term outcomes of routine proximal row carpectomy compared with proximal row carpectomy with dorsal capsular interposition arthroplasty for the treatment of late-stage arthropathy of the wrist.

AIMS: The aims of this study were to compare the mid-term outcomes of patients with late-stage arthritis of the wrist treated with proximal row carpectomy (PRC) and dorsal capsular interposition (DCI) arthroplasty with a matched cohort treated with routine PRC alone. PATIENTS AND METHODS: A total of 25 arthritic wrists (24 patients) with pre-existing degenerative changes of the proximal capitate and/or the lunate fossa of the radius were treated with PRC + DCI over a ten-year period. This group of patients were matched 1:2 with a group of 50 wrists (48 patients) without degenerative changes i

Carpal fractures in children.

Carpal injuries in children are relatively rare. Diagnosis is likely to be missed, or the severity of the injury might not fully be appreciated at the time of presentation. Fortunately, in the majority of cases the fracture heals uneventfully with a low incidence of complications. As in adults, the scaphoid is most susceptible to injury and the capitate, which often presents in association with other carpal bone injuries, the next most susceptible. Other carpal bone fractures are very unusual and the literature largely is confined to case reports or very small series. This review article lists

[Unilateral congenital absence of the carpal scaphoid associated with dysplasia of the capitate. Presentation of a case].

We report on a case of aplasia or unilateral congenital absence of the carpal scaphoid associated with dysplasia of the capitate. Congenital absence of the carpal scaphoid is a rare but well-documented condition. As far as we know, the present case is the seventh one reported in the medical literature. Imaging studies (X-ray and MRI) confirmed the absence of the carpal scaphoid associated with a dysplasia of the capitate and malformation of the radial styloid process. Congenital absence of the scaphoid when other congenital abnormalities (such as hypoplasia or aplasia of either forearm bones o

Observer variability in measurements of carpal bone angles on lateral wrist radiographs.

Determinations of carpal bone angles are used in the clinical evaluation of carpal malalignment. Eleven frequently referred radiological measures in lateral projection of the wrists in 23 wrists were assessed using different definitions of axes. Interobserver- and intraobserver variations were calculated. The standard deviation of the interobserver variation ranged from 2.60 degrees to 18.15 degrees, and the intraobserver variation from 1.89 degrees to 4.66 degrees depending on the angles measured. The use of three angles for the least observer variability in assessment of carpal alignment is

[Fixation techniques for slipped capital femoral epiphysis : Principles, surgical techniques, and complications].

The purpose of this review is to present the pros and cons as well as the surgical techniques of conventional implants used for fixation of slipped capital femoral epiphysis (SCFE). Worth mentioning are K‑wires, Hansson pins, transfixing screws, and gliding screws. We searched PubMed for "ECF" and "SCFE" in combination with "in situ fixation," "pin," "wire," "screw," and "nail." We considered Johansson nail, Knowles pin, and Nyström nail to be obsolete and of historical interest only. We noticed a trend from absolute stability towards some form of dynamic fixation over time, likely related to

Successful delayed surgical treatment of a scaphocapitate fracture.

The scaphocapitate fracture syndrome is a rare entity of a perilunate fracture-dislocation. The diagnosis is frequently missed at initial presentation to a physician. Usually, a CT scan is required to reveal the extent of the lesion. Operative treatment consists of open reduction and internal fixation with screws (e.g. headless compression screw) or Kirschner wires. We report on a case of a 19-year-old man who presented to our hospital 2 months after he fell on his left wrist. He complained about persistent pain and a decreased range of motion after conservative treatment in a short arm cast.

Management of greater arc carpal fractures.

Greater arc injuries are fracture dislocations that involve the perilunar carpal bones. The commonest of these injuries is the dorsal transscaphoid perilunate fracture dislocation. The recommended treatment for acute injuries is open reduction and Kirschner wire fixation of the scaphoid fracture through a dorsal midline approach. Stability to the midcarpal joint is thus provided and no further pins are needed. Established scaphoid nonunions are treated by bone grafting and Herbert screw fixation through a volar approach. Scaphocapitate fracture syndrome is treated by open reduction and pin fix

A Large Osteoid Osteoma of Trapezium: A Regenerative Approach and a Review of Literature.

Osteoid osteoma is a benign bone tumor that usually grows in the long bones of the body and arises from osteoblasts and some components of osteoclasts. It represents the third most frequent type of benign bone tumors, accounting for 11% to 14% of the tumors. The entity usually involves the proximal femur and tibia. It has also been reported in the hand, especially the scaphoid, capitate, and proximal phalanx. The most common symptom is pain, usually during the night, relieved by the use of salicylates and nonsteroidal anti-inflammatory drugs. To date, only 5 cases involving the trapezium have

Stress fractures of ankle and wrist in childhood: nature and frequency.

BACKGROUND: Stress fractures of many etiologies are found not infrequently in various tarsal bones but are less commonly recognized in carpal bones. OBJECTIVE: To assess the distribution of tarsal and carpal stress fractures. MATERIALS AND METHODS: During the last three decades, the senior author collected locations of tarsal and carpal bone stress fracture callus seen on plain radiographs. RESULTS: 527 children with tarsal and carpal stress fractures were identified (88 children had multiple bones involved). The totals were: calcaneus 244, cuboid 188, talus 121, navicular 24, cuneiforms 23

Osteofibrous dysplasia: a narrative review.

Osteofibrous dysplasia (OFD) is a rare, benign, self-limited bone disorder with a relatively low incidence, accounting for approximately 0.2% of all primary bone tumors. It was frequently found intra-cortical of the mid-shaft of the tibia. OFD can also occur in other skeletal regions, including the fibula, ulna, radius, femur, humerus, ischium, rib, tarsus, metatarsals, vertebral, and capitate. OFD can present with asymptomatic, mass, pain, swelling, deformity, and even pathological fracture. OFD might be misdiagnosed as adamantinoma (AD) and because they are three subtypes origin from the sam

Complications at extraction of the ASIF epiphysiodesis screws.

A retrospective study was performed of 30 consecutive patients operated on with ASIF epiphysiodesis for slipped capital femoral epiphysis. During extraction, complications were encountered in 13/23 patients. A total of 38 screws were removed. Thirteen screws broke and another three screws were embedded in bone, making extensive chiseling of cortical bone necessary before extraction was possible. Due to the frequent complications at extraction, the ASIF epiphysiodesis screw is not suitable for treatment of the slipped capital femoral epiphysis.

The nuclide bone-scan in the diagnosis and management of Perthes' disease.

The nuclide bone-scan will reliably diagnose Perthes' disease with a sensitivity of 0.98 and a specificity of 0.95. The comparable figures for radiographic sensitivity and specificity are respectively 0.92 and 0.78. In addition, it is possible on the scan to recognise the onset of revascularisation of the femoral capital epiphysis some months before there are radiographic signs of new bone formation. Scintigraphy also suggests that in some cases of transient synovitis there may bae a period of reversible ischaemia of the capital epiphysis, which may have relevance to the pathogenesis of Perthe

Retrospective review of 234 scaphoid fractures and nonunions treated with arthroscopy for union and complications.

The purpose of this paper is to retrospectively review 234 consecutive cases of scaphoid fractures and nonunions treated using arthroscopy with the dorsal percutaneous implantation of a headless compression screw for healing and complications. Solid union of fracture is determined by CT scan. We identified 126 acute injuries, including 65 proximal pole fractures; 67 grossly displaced fractures; 12 trans-scaphoid perilunate dislocations including four trans-scaphoid trans-capitate fractures; and ten combined scaphoid and distal radius fractures. 108 scaphoid nonunions were identified. 98 were c

Sequential radionuclide bone imaging in avascular pediatric hip conditions.

Radionuclide bone imaging was performed on six patients with various hip conditions. Initial bone images revealed diminished uptake of isotope 99mTc-MDP in the capital femoral epiphysis. Following therapeutic intervention, repeat bone scans revealed normal uptake of 99mTc-MDP in the capital femoral epiphysis. Subsequent radiographs revealed that avascular necrosis had not occurred. There are two types of avascularity: the potentially reversible, and the irreversible. Attempts should be made toward early recognition of the potentially reversible avascular insult. With early recognition, surgica

Intraosseous pressure in Kienböck's disease.

Intraosseous pressure was measured in the lunate of 10 patients undergoing surgery for Kienböck's disease. Using the styloid process of the radius and the capitate as reference points of bone pressure, I found a stasis rather than arterial insufficiency in the lunate. This pattern was observed regardless of the stage of the disease according to Ståhl and regardless of the finding by technetium TC 99m-methylene diphosphonate scintigraphy. The significance of this finding is still unknown but questions the current concept that Kienböck's disease is caused by arterial insufficiency, because these
