[Surgery of loco-regional recurrences in colorectal cancer].

By the term of local recurrence (LR), after apparently curative resection of a colorectal cancer, we mean the recurrence of the disease at the site of the original tumor, in correspondence to the regional lymph nodes or to intestinal anastomosis. The frequency of LR, which differs widely from one case to another because of the unhomogenity of the samples under examination, is on average around 16%, although this figure is to be considered an underestimation since it is not supported by routine autopsies. The authors analyse, together with the data in the literature, the results of their person

[A Study of Bridge to Surgery for Obstructive Colorectal Cancer].

The JSCCR Guidelines 2024 for the Treatment of Colorectal Cancer weakly recommend stent treatment as a bridge to surgery(BTS)for obstructive colorectal cancer in the context of curative surgical resection. We retrospectively examined the backgrounds and treatment outcomes of 10 patients with obstructive colorectal cancer who underwent stent placement for BTS at our hospital between April 2020 and October 2024. The study included 4 males and 6 females, with a median age of 82 years. The lesions were located in the ascending/transverse/descending/sigmoid colon in 2/3/3/2 cases, respectively. CRO

The ileus and the oddities after colorectal surgery, continued.

In this second part of a series, complications related to colorectal surgery are highlighted and discussed, including anastomotic bleeding and hemorrhage, wound infection, anastomotic dehiscence and fistula formation, small bowel obstruction, and genitourinary complications. Prevention of complications may be improved by adequate selection of appropriate procedures for the patient, good surgical technique, and good postoperative care.

Surgery for colorectal cancer in the small town of Komotini.

BACKGROUND: Here we report our experience in treating colon cancer in the 5 years from 200 to 2011. Our surgical clinic treated 49 patients with colorectal cancer, of whom 28 (57.14%) were men of mean age 62 years and 21 (42.86%) were women of mean age 66 years. METHODS: In 15 cases, the cancer was related to the rectum (30.61%) and the remaining 34 cases (69.39%) were related to the colon. We found synchronous cancer in two patients. One was found in the blank and the upper right while the second was found in the transverse and sigmoid colon. Six of our patients suffered from coexisting bili

Heparins and mechanical methods for thromboprophylaxis in colorectal surgery.

BACKGROUND: Colorectal surgery implies higher risk of postoperative thromboembolic complications as deep venous thrombosis (DVT) and pulmonary embolism (PE) than general surgery. The best prophylaxis in general surgery is heparin and graded compression stockings. No systematic review on combination prophylaxis or on thrombosis prophylaxis in colorectal surgery has been published. OBJECTIVES: To compare the incidence of postoperative thromboembolism after colorectal surgery using prophylactic methods focussing on heparins and mechanical methods alone and in combinations. SEARCH STRATEGY: Elec…

Results of the double stapling procedure in colorectal surgery.

In this report we review our results with the double stapling technique (DST) in 162 patients with colorectal diseases in an attempt to identify some of the potential pitfalls of this new technique. Among these 162 patients, there were 125 patients with colorectal cancer, 25 with chronic ulcerative colitis (UC), 9 with familial adenomatous polyposis (FAP), 2 with adult Hirschsprung's disease, and 1 with sigmoid colon fistula. A total of 46 anastomoses (28 for rectal cancer, 13 for UC, 3 for FAP, and 2 for adult Hirschsprung's disease) were performed at or near the dentate line. Of these, 10 ha

[The value and limit of extended surgery in colorectal cancer].

Since the majority of colorectal cancers are well differentiated adenocarcinomas and grow rather slowly and well limited, surgery is the most available and favored treatment. The cancer-related 5-year survival rates of patients having undergone an extended operation versus those undergoing the conventional operation in our hospital (1969-1983) were 86% and 70% for Dukes B stage, and 60% and 38% for Dukes C stage. The survival superiority of the extended operation was confirmed as statistically significant (p less than 0.05). However, this survival advantage not true for patients with lateral l

Does Colorectal Stenting as a Bridge to Surgery for Obstructive Colorectal Cancer Increase Perineural Invasion?

OBJECTIVES: To clarify whether self-expandable metallic stent (SEMS) placement for obstructive colorectal cancer (CRC) increases perineural invasion (PNI), thereby worsening the prognosis. METHODS: In total, 1022 patients with pathological T3 or T4 colon or rectosigmoid cancer who underwent resection were retrospectively reviewed. The study patients were divided into a no obstruction group (n=693), obstruction without stent group (n=251), and obstruction with stent group (n=78), and factors demonstrating an independent association with PNI, the difference in PNI incidence and severity between

The impact of previous abdominal surgery on colorectal cancer patients undergoing laparoscopic surgery.

The current study aimed to investigate whether previous abdominal surgery (PAS) could affect the outcomes of colorectal cancer (CRC) surgery. We conducted the search strategy in three databases (PubMed, Embase, and the Cochrane Library) from inception to May 26, 2022. The short-term and long-term outcomes were compared between the PAS group and the non-PAS group. Odds ratios (ORs) and 95% confidence intervals (CIs) were pooled up. Stata (V.16.0) software was used for data analysis. We included 34,827 patients from 14 studies in the current study. After pooling up all the data, we found that th

Early and late outcome after surgery for colorectal cancer: elective versus emergency surgery.

AIMS AND BACKGROUND: Emergency surgery for colorectal cancer is associated with a higher postoperative morbidity and mortality rate and a poor long-term outcome compared with elective surgery. The aim of the present study was to compare early and late outcome after elective and emergency surgery for malignant colorectal cancer, looking for the principal determinants of a worse outcome after emergency colorectal surgery. METHODS: A retrospective study of 236 patients presenting with colorectal cancer over an 8-year period was undertaken. Of these, 118 presented as emergencies, whereas 118 pati…

Influence of proficiency in conventional laparoscopic surgery in colorectal cancer on the introduction of robotic surgery.

PURPOSE: Although there have been many reports on learning curves for robotic surgery, it is unclear how surgeons' conventional laparoscopic surgical skills influence their ability in performing robotic surgery for colorectal cancer (CRC). The aim of this study was to determine the surgical outcomes of robotic surgery for CRC during the induction phase by skilled laparoscopic surgeons. METHODS: Surgical outcomes of consecutive CRC cases between January 2021 and March 2023 following the skilled phase of laparoscopic surgery and introductory phase of robotic surgery performed by three skilled l

Artificial intelligence performance in generating colorectal surgery board questions.

BACKGROUND: Large language models (LLM) can pass medical licensing and specialty board exams, but their ability to generate high-quality board-style exam questions is uncertain. METHODS: Three LLMs each generated 20 colorectal surgery board questions in accordance with American Board of Colon and Rectal Surgery guidelines. Questions from the Colon and Rectal Surgery Educational Program (CARSEP) served as comparators. Board-certified colorectal surgeons, blinded to source, graded each question on clarity, relevance, suitability, distractor quality, and adequacy of rationale, and categorized qu…

Understanding Surgery Refusal in Patients with Colorectal Liver Metastases.

PURPOSE: To investigate the risk factors, reasons, and survival outcomes associated with refusal of surgery for colorectal liver metastases (CRLM). Surgery is the only curative treatment for CRLM, however, some patients refuse. The risk factors and reasons for refusal remain uninvestigated in patients with CRLM. METHODS: Patients with resectable and operable CRLM referred for treatment at Copenhagen University Hospital (2013-2023), were identified through the Danish Liver Cancer Group registry. Those who refused surgery were matched 1:4 by WHO performance status with patients who accepted sur

Mental health outcomes following emergent versus elective colorectal surgery.

INTRODUCTION: Emergency surgery carries risk for psychological adverse events, yet procedure-specific mental-health outcomes remain poorly characterized. This study evaluated the association between surgical acuity (emergent vs elective), ostomy formation and new-onset postoperative mental-health conditions following colorectal surgery. METHODS: A retrospective cohort study of patients undergoing emergent or elective colorectal surgery from 2015 to 2025 in the TriNetX research-network was performed. Patients with pre-existing mental-health diagnoses were excluded. Analyses were stratified a p…
