Reaction times of cochlear implant users to a change in electrode or modulation frequency in the presence of a masker.

Spatial and temporal selectivity vary substantially between cochlear implant users, resulting in large differences in speech perception. Direct electrode stimulation offers precise control to capture this variability. However, most studies using direct stimulation have not incorporated interfering stimuli, which may not reflect the complex nature of speech and ambient noise. To address this gap, we introduce the Electrode Interaction Reaction Time (ELI-RT) paradigm, which uses amplitude-modulated, interleaved pulses on two closely spaced electrodes at clinical pulse rates. ELI-RT includes two

Quantitative Investigation of Implant Effect of Intracranial EEG Electrodes on Cortico-Cortical Evoked Potentials.

Intracranial EEG (iEEG) electrodes allow for direct sampling of neural signals from cortical substructures that provide valuable insight for the treatment of medically-refractory epilepsy and other neurological conditions. Changes in neural behavior (i.e., seizure frequency or motor function) have been documented as the result of the implantation of the intracranial electrodes, termed the implant effect, but the mechanism responsible for these changes is not well understood. Implant revision (i.e., the implantation of new iEEG electrodes to sample additional brain regions) during pre-surgical

Diving technique of cochlear implant electrode insertion for hearing preservation: how I do it.

BACKGROUND: Preservation of intracochlear structures and residual hearing has become a major concern in modern cochlear implant. Consequently, many efforts have been made to minimize intraoperative trauma, especially while cochlear fenestration and electrode insertion. METHODS: Building on the core concept of "soft surgery", a modified approach, described as diving technique for cochlear implant electrode array insertion is proposed. Steps and technical points are presented with figures, video and review of relevant anatomy. CONCLUSIONS: This novel diving technique is operationally feasible

Automatic localization of cochlear implant electrodes using cone beam computed tomography images.

BACKGROUND: Cochlear implants (CI) are implantable medical devices that enable the perception of sounds and the understanding of speech by electrically stimulating the auditory nerve in case of inner ear damage. The stimulation takes place via an array of electrodes surgically inserted in the cochlea. After CI implantation, cone beam computed tomography (CBCT) is used to evaluate the position of the electrodes. Moreover, CBCT is used in research studies to investigate the relationship between the position of the electrodes and the hearing outcome of CI user. In clinical routine, the estimation

Robotic Milling of Electrode Lead Channels During Cochlear Implantation in an ex-vivo Model.

Objective: Robotic cochlear implantation is an emerging surgical technique for patients with sensorineural hearing loss. Access to the middle and inner ear is provided through a small-diameter hole created by a robotic drilling process without a mastoidectomy. Using the same image-guided robotic system, we propose an electrode lead management technique using robotic milling that replaces the standard process of stowing excess electrode lead in the mastoidectomy cavity. Before accessing the middle ear, an electrode channel is milled robotically based on intraoperative planning. The goal is to f

[A novel intraoperative imaging tool to follow the cochlear implant electrode array insertion dynamics].

Estimating health of the implanted cochlea using psychophysical strength-duration functions and electrode configuration.

It is generally believed that the efficacy of cochlear implants is partly dependent on the condition of the stimulated neural population. Cochlear pathology is likely to affect the manner in which neurons respond to electrical stimulation, potentially resulting in differences in perception of electrical stimuli across cochlear implant recipients and across the electrode array in individual cochlear implant users. Several psychophysical and electrophysiological measures have been shown to predict cochlear health in animals and were used to assess conditions near individual stimulation sites in

Factors responsible for early postoperative mental alterations after bilateral implantation of subthalamic electrodes.

INTRODUCTION: Early postoperative mental changes are the most frequent problem after bilateral subthalamic electrode implantation. The study aims to find an association between them and factors related to patient, disease and surgery, including the size of the third ventricle as brain atrophy marker. MATERIAL AND METHODS: The study included 80 patients with bilateral subthalamic electrodes implanted for motor complications of Parkinson's disease (PD). Patients' age, disease and motor complications duration, medication, neuropsychological tests, surgical reports, third ventricle length (interc

Fitting prelingually deafened adult cochlear implant users based on electrode discrimination performance.

OBJECTIVE: This study investigated the hypotheses that (1) prelingually deafened CI users do not have perfect electrode discrimination ability and (2) the deactivation of non-discriminable electrodes can improve auditory performance. DESIGN: Electrode discrimination difference limens were determined for all electrodes of the array. The subjects' basic map was subsequently compared to an experimental map, which contained only discriminable electrodes, with respect to speech understanding in quiet and in noise, listening effort, spectral ripple discrimination and subjective appreciation. Non-penetrating round window electrode stimulation for tinnitus therapy followed by cochlear implantation.

One main theory behind the origin of tinnitus is based on the idea that alterations of the spontaneous electrical activity within the auditory system lead to abnormal firing patterns in the affected nervous structures [1]. A possible therapeutic option is the use of electrical stimulation of the auditory nerve for the recovery or at least limitation of the abnormal firing pattern to a level that can be easily tolerated by the patient. The Tinnelec Implant consists of a single non-penetrating stimulation electrode connected to a Neurelec cochlear implant system. As a first feasibility study, be

Staged implantation of multiple electrodes in the internal globus pallidus in the treatment of primary generalized dystonia.

OBJECT: Deep brain stimulation (DBS) is used for treating various types of dystonia. Multiple electrodes could be proposed to improve the therapeutic outcome enabling the targeting of specific neuronal populations not reached by the electrical field generated by the initially implanted electrode. The authors address the question of the feasibility and safety of staged multiple lead implantations in the sensorimotor internal globus pallidus (GPi) in primary generalized dystonia (PGD). Criteria for patient selection, surgical technique, target selection, electrical settings management, and clini

Consonant recognition as a function of the number of stimulation channels in the Hybrid short-electrode cochlear implant.

Consonant recognition was measured as a function of the number of stimulation channels for Hybrid short-electrode cochlear implant (CI) users, long-electrode CI users, and normal-hearing (NH) listeners in quiet and background noise. Short-electrode CI subjects were tested with 1-6 channels allocated to a frequency range of 1063-7938 Hz. Long-electrode CI subjects were tested with 1-6, 8, or 22 channels allocated to 188-7938 Hz, or 1-6 or 15 channels from the basal 15 electrodes allocated to 1063-7938 Hz. NH listeners were tested with simulations of each CI group/condition. Despite differences

Determination of daunomycin at a novel COOH/indium tin oxide ion implantation-modified electrode.

A novel COOH+ ion implantation-modified indium tin oxide (COOH/ITO) electrode was prepared for the first time and applied for determination of daunomycin (or daunorubicin [DNR]). The electrode showed higher catalytic activity than bare ITO electrode with good reproducibility and stability. The determination condition of linear voltammetry was optimized. A calibration curve was obtained over the range 2.0 x 10(-7) to 5.0 x 10(-6)mol/L, with a correlation coefficient of 0.9909 and a limit of detection of 1.0 x 10(-7)mol/L. The selectivity of the electrode was illustrated by determination of DNR

Histologic evaluation of the tissue seal and biologic response around cochlear implant electrodes in the human.

HYPOTHESIS: Histopathologic study of the tissue seal and biologic response around cochlear implant electrodes in patients who had received a cochlear implant during life could provide clues concerning the pathogenesis of meningitis after cochlear implantation. BACKGROUND: Bacterial meningitis has been reported as an infrequent complication of cochlear implantation using a variety of electrode designs. The cause of meningitis in cochlear implant recipients has not been firmly established. In an analogous surgical situation, namely stapedectomy, delayed meningitis could occur as a complication

Use of a single coil transvenous electrode with an abdominally placed implantable cardioverter defibrillator in children.

While transvenous defibrillator electrode placement avoiding a thoracotomy is preferable, electrode size, a large intercoil spacing, and the need for subclavicular device placement preclude this approach in most children. We investigated a single RV coil to an abdominally placed active can ICD device. Five children ages 8-16 years (weight 21-50 kg, mean 35 kg) underwent ICD placement. Placement of a single coil Medtronic model 6932 or 6943 electrode was performed via the left subclavian vein approach and the electrode positioned in the RV apex with the coil lying along the RV diaphragmatic sur

Erosion of implantable cardioverter defibrillator patch electrode into airways: an unusual cause of recurrent hemoptysis.

A 74-year-old man presented with a 9-month history of recurrent hemoptysis. He had implantable cardioverter defibrillator (ICD) patch electrodes placed 4 years before. A chest x-ray film showed crinkling of his posteriorly placed ICD patch which also appeared to have separated from his ventricle on a CT scan of the chest. Bronchoscopy localized the ICD patch electrode to the lower lobe of the left lung. He underwent a lobectomy and was treated with antibiotics at home. In patients with known ICD implantation, patch erosion into the airways should be considered in the differential diagnosis of

Endoscopic electrode implantation for enhanced stimulation of the hamstring branches of the sciatic nerve in paralyzed subjects.

Seven hamstring branches of the sciatic nerves in eight posterior thighs of four paraplegics were implanted with electrodes endoscopically. Strong initial responses were obtained in each case on continuous electrical stimulation. Three of the implants were removed immediately due to rapid decay in the response to stimulation; two had muscle power reduced to grade IV and two to grade III over 2-week period postoperatively. We believe the reduction in strength was partly due to hematoma surrounding the electrode and partly due to migration of the electrode away from the target nerves. Further wo

Transcutaneous cardiac pacing in patients with automatic implantable cardioverter defibrillators and epicardial patch electrodes.

In patients with automatic implantable cardioverter defibrillators, insulation of the epicardial patch electrodes (patches) prevents externally applied current from passing through the electrode to the cardiac muscle so that external transthoracic and even internal defibrillation can be unsuccessful. Because emergency cardiac pacing may be required in such a case, a study was performed to evaluate whether, and at what threshold and electrode orientation, transcutaneous pacing is possible in patients with implanted patches. Thresholds for transcutaneous pacing were determined during general ane
