Cholera outbreak in a fishing village in Uganda: a case study.

In June 2015, the District Health Officer of Kasese District, southwestern Uganda reported an outbreak of cholera in a fishing village. Two fellows of the Uganda Public Health Fellowship Program - Field Epidemiology Track conducted an investigation to verify the existence of an outbreak, determine the mode of transmission, and recommend control measures. This case study describes that investigation, which teaches the steps in an outbreak investigation and the details in each step, what needs to be done in each step to achieve the objectives of the investigation, and what might be the common pi

The impact of small dams on parasitic diseases in Cameroon.

Economic development in Africa implies the provision of vast, sparsely populated regions with improved road networks and agricultural techniques. Climatic constraints mean that control of water resources is essential in arid areas. But hydrogrophic modifications also have the potential for adverse impact on the health of local populations. The extension of water sources and their all year round supply of water mean that people are attracted towards them, and to the same places where conditions favour the spread of intermediate hosts and vectors of parasitic diseases such as schistosomiasis, on

Changes in the mineral composition of food as a result of cooking in "hard" and "soft" waters.

During the last decade many epidemiologists have found an inverse relationship between water hardness and cardiovascular mortality. It seems unlikely that this relationship can only be attributed to a deficiency of calcium and mangnesium in drinking water, because only 10-20% of the total daily intake of calcium and magnesium is derived from drinking water. The purpose of this study was to investigate changes in the mineral composition of food to investigate changes in the mineral composition of food when cooking with waters of different hardness. The most significant differences were found fo

Two Sporadic Cases of Erysipelothrix rhusiopathiae Infection, Minnesota 2024.

INTRODUCTION: Erysipelothrix rhusiopathiae is a gram-positive, rod-shaped bacterium that can cause infections in livestock, marine mammals and humans. Human infections are traditionally affiliated with occupational exposures to E. rhusiopathiae through farming, butchering, or cooking. In July 2024, the Minnesota Department of Health was notified of two E. rhusiopathiae infections in the Minneapolis-St. Paul metropolitan area. Concerned about a possible outbreak, an investigation was initiated to determine the possible source(s) of infection. METHODS: Epidemiologists conducted reiterative, sem

Yellow fever: A brief history of a tropical Virosis.

Yellow fever is a zoonotic arbovirosis, the agent of which is transmitted by mosquitoes. In humans, this virus can cause hemorrhagic hepato-nephritis, while mild or inapparent infections are common. The catastrophic epidemics that occurred, mainly in the 18th and the 19th centuries, in Latin America and the United States as well as in the port cities of West Africa and Europe, had considerable demographic, socio-economic and political repercussions. The viral nature of the infectious agent and its transmission by the Aedes aegypti mosquito, previously suspected by Beauperthuy, were demonstrate

Time to control of anthrax outbreaks in Africa, 2014-2023: A systematic review and meta-analysis.

Anthrax is a notifiable zoonotic disease targeted for control in Africa, however, outbreaks due to anthrax are still frequent and large. Surveillance systems should monitor and detect anthrax outbreaks early for prompt response. This systematic review and meta-analysis aimed to determine anthrax outbreaks epidemiological investigations gaps and time to control in Africa, Jan 2014-Dec 2023. We searched MEDLINE, PubMed, Scopus, Embase, Google Scholar, and Web of Science databases using PICO framework for studies on anthrax investigations. Using Covidence, we screened and extracted studies, analy

The changing epidemiology of SARS-CoV-2.

We have come a long way since the start of the COVID-19 pandemic-from hoarding toilet paper and wiping down groceries to sending our children back to school and vaccinating billions. Over this period, the global community of epidemiologists and evolutionary biologists has also come a long way in understanding the complex and changing dynamics of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the virus that causes COVID-19. In this Review, we retrace our steps through the questions that this community faced as the pandemic unfolded. We focus on the key roles that mathematical mod

Tick-borne pathogens and their vectors in Kazakhstan - A review.

Tick-borne diseases (TBDs), in particular Crimean-Congo hemorrhagic fever (CCHF) and tick-borne encephalitis (TBE), have been found to be expanding beyond their traditional geographic locations in Kazakhstan, which led to increased awareness and concern against TBDs in recent years. The tick fauna of Kazakhstan includes more than 30 species of human-biting ixodid ticks, recognized as carriers of a number of dangerous infectious pathogens that cause CCHF, TBE, tularemia, tick-borne rickettsioses and Lyme borreliosis. This paper presents the current knowledge of the main vectors of tick-borne pa

Is angiotensin I-converting enzyme a "master" disease gene?

Clustering of diseases has been appreciated by health insurers and epidemiologists for some time. Co-morbidity suggests shared pathways of disease. It is by now well agreed that common diseases have a strong genetic component. Here we present evidence that the angiotensin I-converting enzyme (ACE) deletion/deletion (D/D) genotype is associated with a large number of common adult diseases, including cardiovascular disease, cancer, and psychiatric disease. Since the ACE D/D genotype has been shown to be associated with increased levels of tissue ACE expression at the protein level, these data su

Physical activity and the prevention of breast cancer.

Epidemiologists have identified several risk factors for breast cancer, yet clinical advice to women to change these risk factors has been uncommon. Physical activity promises to be one modifiable risk factor through which women can reduce their risk for breast cancer. Clinicians can now advise women that reducing risk for breast cancer may be one additional reason to adopt an active lifestyle. There are still questions about the type and amount of exercise needed, the ages at which exercise should be done, and the interactions with other risk factors such as reproductive and menstrual history

Adverse virus-drug interactions.

Over the last 3 decades, epidemiologists and clinicians have identified a few clinical entities that appear to result when a viral infection and a chemical exposure overlap and interact. Ampicillin rash during infectious mononucleosis, Reye's syndrome following salicylate ingestion and certain viral infections, and the association of AIDS-related Kaposi's sarcoma with abuse of nitrite inhalants and infection due to human immunodeficiency virus are examples of such phenomena. Preclinical research provides additional evidence that viruses and chemicals may interact and produce illnesses in anima

[Coxsackie B enterovirus infection in Astrakhan Province].

Overall 132 patients with Coxsackie B enteroviral infection were admitted to an Astrakhan infectious hospital in 1989. Of these, 100 patients had aseptic meningitis and 32 presented with respiratory infection syndrome. The severity of enteroviral meningitis may vary from mild forms running its course without marked meningeal syndrome to grave ones associated with high pleocytosis, manifest syndrome of CSF hypertension and potential exacerbation of the infectious process. The patients with the syndrome of respiratory infection demonstrated its combination with intestinal disorders (39%). The EC

Cases of Neisseria gonorrhoeae with plasmid-mediated resistance to penicillin increase. The National Study Group.

Penicillinase-producing isolates of Neisseria gonorrhoeae (PPNG) were submitted to the National Laboratory for Sexually Transmitted Diseases for biological and genetic identification as part of an on-going national surveillance program for monitoring antibiotic-resistant gonococci. The primary identification of the isolates was completed in provincial laboratories. The isolates were documented and forwarded to LCDC through the provincial laboratories and provincial epidemiologists. Duplicate isolates from the same patient were excluded from the final analysis; retrospective clinical and epidem

Immediate hypersensitivity: a clinical review.

Diseases in which immediate hypersensitivity (IH) reactions occur, (asthma, allergic rhinitis and eczema) are very common in Australia although their overall prevalence is unknown. Until recently the clinical investigation of IH has been undertaken almost exclusively by allergists using scratch tests. The discovery of Ige by Ishizaka et al. in 1966 resulted in a rapid increase in the understanding of the processes involved in IH reactions and, although much remains unclear, respiratory physicians, immunologists and epidemiologists are becoming increasingly involved in investigating the IH react

Seeing the Forest for the Trees: Evaluating Population Data in Allergy-Immunology.

A population-level study is essential for understanding treatment effects, epidemiologic phenomena, and health care best practices. Evaluating large populations and associated data requires an analytic framework, which is commonly used by statisticians, epidemiologists, and data scientists. This document will serve to provide an overview of these commonly employed methods in allergy and immunology research. We will draw upon recent examples from the allergy-immunology literature to contextualize discrete principles of relevance to population-level analysis that include statistical features of

Pelvic inflammatory disease: identifying research gaps--proceedings of a workshop sponsored by Department of Health and Human Services/National Institutes of Health/National Institute of Allergy and Infectious Diseases, November 3-4, 2011.

In November 2011, the National Institutes of Health convened a workshop of basic researchers, epidemiologists, and clinical experts in pelvic inflammatory disease to identify research gaps hindering advances in diagnosis, treatment, and prevention. This article summarizes the presentations, discussions, and conclusions of this group and highlights significant controversies that reveal aspects of pelvic inflammatory disease research that would most greatly benefit from the application of newer molecular, immunologic, and radiologic techniques. Multiple limitations to performing new clinical tri

European efforts to harmonize typing of meningococci.

Neisseria meningitidis causes life-threatening disease in infants, toddlers, and adolescents. Besides representative case notification, public health management of the disease requires bacterial typing information. European reference laboratories and state epidemiologists in collaboration with European institutions have driven forward the harmonization of typing by rigorously adopting DNA sequence typing and using common reference databases. External quality assessment has been provided by supranational networks, i.e. EU-IBIS and IBD-Labnet. The recent development of novel protein-based vaccin
