Consensus document on the management of hyponatraemia of the Acqua Group of the Spanish Society of Endocrinology and Nutrition.

INTRODUCTION: Hyponatremia is the most prevalent electrolyte disorder in the outpatient and inpatient settings. Despite this frequency, hyponatremia, including severe hyponatremia, is frequently underestimated and inadequately treated, thus highlighting the need to produce consensus documents and clinical practice guidelines geared towards improving the diagnostic and therapeutic approach to it in a structured fashion. MATERIAL AND METHODS: Members of the Acqua Group of the Spanish Society of Endocrinology and Nutrition (SEEN) met using a networking methodology over a period of 20 months (bet

Renin, antidiuretic hormone (ADH), and ADH receptor levels in cerebral salt wasting associated with tuberculous meningitis.

BACKGROUND: To evaluate serum antidiuretic hormone (ADH), its receptors, and renin levels in cerebral salt wasting (CSW) in tuberculous meningitis (TBM). METHODS: Patients diagnosed with definite (n = 30) or probable TBM (n = 47) who developed hyponatremia (CSW, SIADH, or miscellaneous causes) were included. Sequential measurement of serum ADH, ADH-R, and renin activity by enzyme-linked immunosorbent assay was done and correlated with serum sodium level, urinary output, and fluid balance. RESULTS: Out of 79 TBM patients, CSW was observed in 36, SIADH in four, and miscellaneous hyponatremia i

Associations of hyponatremia and SIADH with increased mortality, young age and infection parameters in patients with tuberculosis.

BACKGROUND AND OBJECTIVE: Hyponatremia and the syndrome of inappropriate antidiuretic hormone secretion (SIADH) are associated with and can be caused by tuberculosis (TB) through meningitis by locally invading the hypothalamus, adrenal, or pituitary glands or possibly through ectopic ADH production. This study assessed the association of TB mortality with hyponatremia and SIADH in a large cohort of a university hospital in Austria. METHODS: This retrospective study enrolled patients with hyponatremia and patients diagnosed with TB from 01/2001-11/2019 to assess the prevalence of TB in hyponat

[Hyponatremia in cancer patients].

Hyponatremia is one of the most common electrolyte disturbances in cancer patients. Patients with extremely severe symptomatic hyponatremia need treatment with the administration of hypertonic saline. Syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a significant cause of cancer-related hyponatremia. A prospective study at a dedicated cancer hospital in Belgium demonstrated that SIADH was the most common cause of hyponatremia (30.4%). Ectopic ADH production by malignant cells (especially in small-cell lung cancer), several anticancer drugs (cyclophosphamide, ifosfamide, vinc

Syndrome of inappropriate secretion of antidiuretic hormone following laparoscopic inguinal hernia repair.

Syndrome of inappropriate secretion of antidiuretic hormone (SIADH) after cardiac surgery and traditional open abdominal surgery has been reported. This disorder also has been associated with minor operative procedures with the patient under local anesthesia. However, SIADH after laparoscopic surgery is not well documented in the literature. We report a case of SIADH after laparoscopic inguinal hernia repair in an elderly woman.

Antidiuretic hormone excess in infant botulism.

Two infants developed evidence of antidiuretic hormone excess as a complication of infant botulism. Neither child received mechanical ventilatory support before the development of hyponatremia, serum hyposmolality, and urinary hyperosmolality. Both infants responded to fluid-intake restriction. The appearance of hyponatremia in an infant with botulism should suggest antidiuretic hormone excess. The recognition of this entity will lead to its appropriate management with fluid-intake restriction.

Syndrome of inappropriate secretion of antidiuretic hormone in Hodgkin's disease.

A patient with clinical Stage IV Hodgkin's disease of the nodular sclerosis type demonstrated all features of the syndrome of inappropriate secretion of antidiuretic hormone. Electroencephalogram, radiography of the skull, and computerized tomography of the brain and cardiac, renal, thyroid, and adrenal function tests were all normal. It is suggested that the patient presents a rare case of Hodgkin's disease with inappropriate secretion of antidiuretic hormone.

The syndrome of inappropriate secretion of antidiuretic hormone in the maxillofacial trauma patient.

The maxillofacial trauma patient whose neurologic status undergoes a rapid and serious deterioration may have a severe hyponatremia secondary to the inappropriate secretion of antidiuretic hormone (SIADH). Other causes of hyponatremia must be ruled out, especially posttraumatic cerebral salt wasting, which necessitates a different mode of therapy. A case of SIADH is reported, and the work-up and differential diagnosis of posttraumatic hyponatremia are discussed.

Hypouricemia in neonates with syndrome of inappropriate secretion of antidiuretic hormone.

A prospective study of serum levels of uric acid in 23 hyponatremic neonates was performed. Infants on diuretic medications or with renal failure were excluded. The infants were separated into two groups: group I consisted of 11 neonates with clinical evidence of syndrome of inappropriate secretion of antidiuretic hormone (SIADH), (mean +/- SD serum sodium 127 +/- 1.36 mEq/liter). Group II included 12 infants with hyponatremia (mean serum sodium 128 +/- 1.10 mEq/liter) associated with decreased effective vascular volume manifest by a fractional sodium excretion less than 1%. The groups were si

Syndrome of inappropriate secretion of antidiuretic hormone caused by vincristine therapy: a case report of the neuropathology.

The syndrome of inappropriate secretion of antidiuretic hormone is a rare but well-recognized neurotoxic side effect of vincristine therapy. The first neuropathological report of a case is presented. A 6-month-old boy with skin leukemia developed inappropriate secretion of antidiuretic hormone caused by vincristine. Postmortem examination revealed axonal spheroids in the ansa lenticularis and the area surrounded by the substantia innominata, amygdala and supraoptic nucleus. The lesion was confined to that area and the neurosecretory neurons were intact with well preserved neurophysin. The path

Inappropriate secretion of antidiuretic hormone treated with frusemide.

Seven out of nine patients with chronic inappropriate secretion of antidiuretic hormone were successfully treated with 40 mg frusemide daily. One patient needed 80 mg, and the remaining patient achieved only a small increase in diuresis after 40 mg frusemide; this was probably related to his low creatinine clearance. In order to maintain a salt intake high enough to compensate for the loss of urine electrolytes 3 to 6 g sodium chloride was added as tablets to the sodium-free diet in six patients. Hypokalaemia occurred in five patients but was easily corrected with either supplements of potassi

Hyponatraemia due to a thiazide diuretic. A case report.

The time course of profound hyponatraemia has been followed in a patient who developed this condition after 6 week's treatment with a thiazide diuretic, bendroflumethiazide. The case is related to the syndrome of inappropriate secretion of antidiuretic hormone.

Unusual presentation of lung carcinoma with pituitary metastasis: a challenging diagnosis and sodium management dilemmas.

SUMMARY: Pituitary metastasis (PM) is a rare complication of an advanced malignancy. Albeit rare, PM can be more detected and achieve a longer survival rate through frequent neuroimaging and newer oncology therapies. Lung cancer is the most frequent primary site, followed by breast and kidney cancers. Patients with lung cancer generally present with respiratory symptoms and are commonly diagnosed at an advanced stage already. Nevertheless, physicians should be mindful of other systemic manifestations as well as signs and symptoms related to metastatic spread and paraneoplastic syndromes. Herei
