Intracranial pressure in patients with the empty sella syndrome without benign intracranial hypertension.

The intracranial pressure was monitored continuously for at least 48 hours in five patients with empty sella syndrome, who did not have clinical benign intracranial hypertension (BIH). It has been suggested that the empty sella syndrome is a result of chronically elevated intracranial pressure in the presence of a congenitally deficient diaphragma sellae. However, whilst the intracranial pressure in two of the five patients was abnormally high, in three patients in whom it was monitored, the CSF pressure was normal. Although these cases may represent "burnt out" forms of intracranial pressure …

Treatment of intracranial hypertension. Analysis of 105 consecutive, continuous recordings of intracranial pressure.

One hundred and five consecutive recordings of intracranial pressure (ICP) in 95 patients over a three-year period, using a Scott cannula inserted through a burr hole or a twist drill hole into the anterior horn of the lateral ventricle, represent the patient material for this report. The clinical diagnoses were head injury 32, intracranial tumour 31, aneurysm and arteriovenous malformation 18, brain swelling secondary to systemic disease 8, and brain swelling of unknown etiology 6. ICP exceeded 20 mm/Hg in 86 of the recordings (maximum 110 mm/Hg). Hypertonic mannitol was administered 73 times…

The intraocular pressure could not be used to determine the intracranial pressure in patients with hydrocephalus.

PURPOSE: An accurate intracranial pressure (ICP) measurement is vital for patients with hydrocephalus. ICP is generally measured by lumbar puncture opening pressure, which is invasive and has potential adverse side effects. Intraocular pressure (IOP) was investigated to measure ICP indirectly; however, its accuracy was controversial. We conducted this retrospective study to investigate whether IOP can be used as a surrogate for ICP in hydrocephalus patients. METHODS: We performed a retrospective study enrolled 50 cases, including 32 patients with normal pressure hydrocephalus (NPH) and 18 pat…

Intracranial pressure and conductance to outflow of cerebrospinal fluid in normal-pressure hydrocephalus.

Forty patients with clinical evidence of normal-pressure hydrocephalus were studied by monitoring intraventricular pressure during a 24-hour period, and by a lumboventricular perfusion test for measurement of the conductance to outflow of cerebrospinal fluid (CSF). The purpose of the study was to investigate whether there is a relationship between intraventricular pressure and conductance to outflow of CSF, and whether it is possible to use the results from pressure monitoring in the selection of patients who may be expected to benefit from shunting therapy. The conductance to outflow was used

Intracranial pressure in head-injured patients with various intracranial lesions is identical throughout the supratentorial intracranial compartment.

Subarachnoid pressure was measured by bifrontal catheterization to obtain concurrent comparative intracranial pressures (ICPs). These ICPs were compared in 15 head-injured patients who had differences in the pathological processes affecting the right and left hemispheres. ICPs in these patients ranged from -3 to 130 mm Hg, but no difference was found in the concurrent comparative ICPs. These findings suggest that the supratentorial space can generally be considered as one compartment regardless of the differences in types of intracranial lesions.

Systemic vascular responses to increased intracranial pressure. 1. Effects of progressive epidural ballon expansion on intracranial pressure: and systemic circulation.

This paper details the results of experimental studies, on 16 dogs with artificially-induced intracranial space-occupying lesions, of the systemic vascular responses and the intracranial pressure changes (both in the supratentorial and infratentorial compartments) induced by increasing intracranial pressure. The changes produced were divided into two phases such that phase 1 detailed the alterations observed from the start of the balloon inflation up to the initiation of the systemic pressor response. Phase 2 recorded those alterations which occurred during, and immediately after, the period o…

Chapter 5 cerebral perfusion pressure and intracranial pressure in traumatic brain injury.

Nearly 300,000 children and adults are hospitalized annually with traumatic brain injury (TBI) and monitored for many vital signs, including intracranial pressure (ICP) and cerebral perfusion pressure (CPP). Nurses use these monitored values to infer the risk of secondary brain injury. The purpose of this chapter is to review nursing research on the monitoring of ICP and CPP in TBI. In this context, nursing research is defined as the research conducted by nurse investigators or research about the variables ICP and CPP that pertains to the nursing care of the TBI patient, adult or child. A modi…

Erroneous intracranial pressure measurements from simultaneous pressure monitoring and ventricular drainage catheters.

The objective of this report is to highlight the potential for false pressure measurements from systems that combine intracranial pressure (ICP) measurement and ventricular drainage. If the ports of the drain become blocked to the extent that they present a high resistance to cerebrospinal fluid flow, then a significant pressure gradient between the inside and outside of the catheter may be established. Thus, any intracatheter transducer will faithfully record a pressure much lower than true ICP. This holds true for catheter-tip transducers when the transducer lies inside the catheter. In the …

Relationship between intracranial pressure and critical closing pressure in patients with neurotrauma.

BACKGROUND: The driving pressure gradient for cerebral perfusion is the difference between mean arterial pressure (MAP) and critical closing pressure (CCP = zero flow pressure). Therefore, determination of the difference between MAP and CCP should provide an appropriate monitoring of the effective cerebral perfusion pressure (CPP(eff)). Based on this concept, the authors compared conventional measurements of cerebral perfusion pressure by MAP and intracranial pressure (CPP(ICP)) with CPP(eff). METHODS: Simultaneous synchronized recordings of pressure waveforms of the radial artery and blood f…

Intracranial pressure changes and outcomes in the early phase of experimental subarachnoid hemorrhage: severity of subarachnoid hemorrhage and maximum intracranial pressure.

PURPOSE: To investigate the correlation between intracranial pressure (ICP) and the outcomes in rat subarachnoid hemorrhage (SAH) model and determine whether maximum ICP can predict severity of SAH. METHODS: Sprague-Dawley rats underwent the monofilament puncture procedure to induce SAH, and were divided into 4 groups according to the maximum ICP for survival prediction: sham group, mild ICP group (ICP < 50 mmHg), moderate ICP group (ICP 50-149 mmHg), and severe ICP group (ICP ≥ 150 mmHg). RESULTS: Maximum ICP showed strong correlations with the neurological score, SAH grade at 24 h after in…

Cerebrovascular tone rather than intracranial pressure determines the effective downstream pressure of the cerebral circulation in the absence of intracranial hypertension.

Cerebral perfusion pressure is commonly calculated from the difference between mean arterial pressure and intracranial pressure because intracranial pressure is known to represent the effective downstream pressure of the cerebral circulation. Studies of other organs, however, have shown that effective downstream pressure is determined by a critical closing pressure located at the arteriolar level. This study was designed to investigate the effects of PCO2-induced variations in cerebrovascular tone on the effective downstream pressure of the cerebral circulation. Sixteen patients recovering fro…

Rebound surges of intracranial pressure as a consequence of forced ultrafiltration used to control intracranial pressure in patients with severe hepatorenal failure.

Cerebral edema remains the most common immediate cause of death in patients with fulminant hepatic failure. We have used ultrafiltration as a method of controlling intracranial pressure (ICP) when other measures have failed. Two cases in which an initial decrease in ICP was followed by a marked rebound increase resulting in death are reported.

[Idiopathic intracranial hypertension and spontaneous cerebrospinal fluid fistula. Usefulness of intracranial pressure monitoring].

Spontaneous cerebrospinal fluid (CSF) fistulas are rather common in daily practice. The aim of the surgical treatment is closure of the leak, but recurrences are quite frequent. The association between spontaneous CSF fistulas and idiopathic intracranial hypertension (IIH) is not uncommon, and this is probably the cause of the low rate of success of the surgical treatment. Symptoms of IIH associated with spontaneous CSF fistula are atypical, and diagnosis is often missed. Continuous intracranial pressure monitoring is very useful in the diagnosis of chronic IIH and in patients with spontaneous…

The minor symptoms of increased intracranial pressure: 101 patients with benign intracranial hypertension.

Of 101 patients with benign intracranial hypertension not related to vasculitis, neck stiffness occurred in 31, tinnitus in 27, distal extremity paresthesias in 22, joint pains in 13, low back pain in 5, and gait "ataxia" in 4. Symptoms resolved promptly upon lowering the intracranial pressure by lumbar puncture, and were probably directly caused by intracranial hypertension. Awareness of these "minor" symptoms of increased intracranial pressure can facilitate diagnosis and management.
