Degloving lower leg injury - the importance of additional treatment: negative pressure and hyperbaric oxygen therapy.

Degloving injury poses a severe therapeutic challenge concerning both trauma and plastic surgery. The injury involves separation of skin and subcutaneous tissue from fascia and muscles. Treatment is often long-lasting and brings unsatisfying results due to the extent of damage, risk of infection and massive blood loss. In this article, we present the management and therapeutic outcomes of a patient admitted due to the degloving injury of the lower extremity caused by workplace accident. We described the complexity of treatment including surgical intervention as well as additional treatment, wh

Effect of Locomotor Training on Exhaustion of Leg Muscle Activity in Chronic Complete Spinal Cord Injury.

The aim of this study was to evaluate the effect of a continuous locomotor training on leg muscle electromyographic (EMG) exhaustion during assisted stepping movements in a patient with motor complete spinal cord injury (SCI). EMG exhaustion and loss of potentials starts to develop in untrained patients at ∼6 months after injury. In the trained patient examined in this study, exhaustion was also observed but occurred with a delay of several months. In contrast to an untrained patient, no more EMG exhaustion was observed in the very chronic stage. At this time (12 years after injury) a basic lo

Common peroneal nerve injury during a straight leg raising test, the result of an intraneural ganglion.

We report a case of iatrogenic common peroneal nerve (CPN) injury during a straight-leg raising test. Significant swelling in the CPN was observed on ultrasound, most likely due to the loss of the normal fascicular pattern. A postoperative pathological diagnosis indicated a sheath ganglion and high degree of mucinous degeneration of the CPN. A follow-up 3 months after the operation showed marked improvement in the patient's symptoms, corroborated by an electromyogram.

Clinical implications of gene polymorphisms in venous leg ulcer: a model in tissue injury and reparative process.

Wound healing is a multi-step process involving complex pathways at cell and molecular level. Lack of understanding of the molecular mechanisms and pathogenesis of impaired healing in chronic leg ulcers limits clinical assessment and management. In addition, individual genetic background certainly affects the response to treatment and specifically modulates the unfavourable lesion environment. Although the number of actors involved in the aetiology of chronic wounds is extremely high, the ability to find out groups of candidate genes on the basis of clinical and physio-pathological findings is

Successful replantation of an amputated leg with severe crush and avulsion injury in an elderly patient: case report.

An amputated leg with severe crush and avulsion injury was replanted in a 57-year-old man. Since the skin and soft tissue around the ankle became necrotic 11 days postoperatively, a flow-through forearm free flap was transferred into the defect, preserving the blood circulation to the replanted leg. Seven weeks after the replantation, sural nerve cable grafts were interposed between both amputated edges of the tibial nerve. As the function of the ankle joint did not recover, arthrodesis of the ankle joint was performed 7 months after the replantaion. As a result, the patient could walk 1 year

Effects of electrical stimulation-induced leg training on skeletal muscle adaptability in spinal cord injury.

BACKGROUND: Alzheimer's disease is associated with abnormalities in the levels of some brain metabolites, including decreases in N-acetyl-aspartate (NAA) and increases in myo-inositol and choline levels. Cholinesterase inhibitors have proven modest effects on cognition in patients with mild or moderate Alzheimer's disease; however, there is little information on the effects of these drugs on metabolic parameters in the brain. Magnetic resonance spectroscopy (MRS) provides a method of determining changes in such parameters. OBJECTIVE: To assess the effect of rivastigmine on metabolite levels i

The Effects of Lower Leg Compression Garments on Lower Extremity Sports Injuries, Subjective Fatigue and Biomechanical Variables: A Systematic Review with Meta-analysis.

The objective of this study was to systematically review the literature on the effect of CGs versus non-CGs (such as regular socks) or versus placebo garments on 1) the incidence of lower extremity sports injuries and 2) subjective ratings of fatigue and biomechanical variables in athletes at participating in any sport that required any level of running performance, given that fatigue-related biomechanical alterations may increase the risk of sports injuries. This study was a systematic review with meta-analyses. PubMed, Embase, CINAHL, Cochrane, PEDro, and Scopus were searched for eligible st

Can a Boxer Engine Reduce Leg Injuries Among Motorcyclists? Analysis of Injury Distributions in Crashes Involving Different Motorcycles Fitted with Antilock Brakes (ABS).

OBJECTIVES: Several studies have shown that motorcycle antilock braking systems (ABS) reduce crashes and injuries. However, it has been suggested that the improved stability provided by ABS would make upright crashes more frequent, thus changing the injury distributions among motorcyclists and increasing the risk of leg injuries. The overall motorcycle design can vary across different categories and manufacturers. For instance, some motorcycles are equipped with boxer-twin engines; that is, with protruding cylinder heads. A previous study based on a limited material has suggested that these co…

Benchmarking cow comfort on North American freestall dairies: lameness, leg injuries, lying time, facility design, and management for high-producing Holstein dairy cows.

In this paper, we describe a novel approach to corporate involvement in on-farm assessment, driven by the desire to provide a service for dairy producers and to create a vehicle for engagement on issues of dairy cow welfare. This program provides producers with feedback on animal-based (including gait score, leg injuries, and lying time) and facility-based (including freestall design, bedding practices, feed bunk design and management, and stocking density) measures that can be used to better address their management goals. The aim of this paper is to describe variation in the prevalence of la…

Stature and mitigation systems affect the risk of leg injury in vehicles attacked under the body by explosive devices.

A finite-element (FE) model, previously validated for underbody blast (UBB) loading, was used here to study the effect of stature and of mitigation systems on injury risk to the leg. A range of potential UBB loadings was simulated. The risk of injury to the leg was calculated when no protection was present, when a combat boot (Meindl Desert Fox) was worn, and when a floor mat (IMPAXXTM), which can be laid on the floor of a vehicle, was added. The risk of injury calculated indicates that the floor mat provided a statistically significant reduction in the risk of a major calcaneal injury for pea

An Injury Risk Function for the Leg, Foot, and Ankle Exposed to Axial Impact Loading Using Force and Impulse.

Most injury risk functions (IRFs) for dynamic axial loading of the leg have been targeted toward automotive applications such as predicting injury caused by intrusion into the occupant compartment from frontal collisions. Recent focus on leg injuries in the military has led to questions about the applicability of these IRFs shorter duration, higher amplitude loading associated with underbody blast (UBB). To investigate these questions, data were collected from seven separate test series that subjected post-mortem human legs to axial impact. A force and impulse-based Weibull survival model was …

Evaluation of the effectiveness of toe board energy-absorbing material for foot, ankle, and lower leg injury reduction.

OBJECTIVE: Since 2000, numerous improvements have been made to the National Association for Stock Car Auto Racing, Incorporated (NASCAR®) driver restraint system, resulting in improved crash protection for motorsports drivers. Advancements have included seats, head and neck restraints (HNRs), seat belt restraint systems, driver helmets, and others. These enhancements have increased protection for drivers from severe crash loading. Extending protection to the driver's extremities remains challenging. Though the drivers' legs are well contained for lateral and vertical crashes, they remain large
