Differentiation of osteophyte types in osteoarthritis - proposal of a histological classification.

OBJECTIVE: Osteoarthritis is not only characterized by cartilage degradation but also involves subchondral bone remodeling and osteophyte formation. Osteophytes are fibrocartilage-capped bony outgrowths originating from the periosteum. The pathophysiology of osteophyte formation is not completely understood. Yet, different research approaches are under way. Therefore, a histological osteophyte classification to achieve comparable results in osteophyte research was established for application to basic science research questions. METHODS: The osteophytes were collected from knee joints of osteo…

Cervical osteophyte resulting in compression of the jugular foramen: Case report.

Jugular foramen syndrome is a condition characterized by unilateral paresis of cranial nerves IX, X, and XI in the setting of extrinsic compression. Here, the authors describe the case of a giant cervical osteophyte resulting in compression of the jugular foramen. A 74-year-old man who presented with progressive dysphagia and dysarthria was found to have right-sided tongue deviation, left palatal droop, and hypophonia. His dysphagia had progressed to the point that he had lost 25 kg over a 4-month period, necessitating a gastrostomy to maintain adequate nutrition. He underwent extensive workup…

[Mathematical simulation of biomechanical background of osteophyte formation in cervical vertebra].

PURPOSE OF THE STUDY: The aim of this study was to simulate different types of cervical vertebra loading and to find out whether mechanical stress would concentrate in regions known in clinical practice as predilection sites for osteophyte formation. The objective was to develop a theoretical model that would elucidate clinical observations concerning the predilection site of bone remodelling in view of the physiological changes inside the cervical vertebral body. MATERIAL AND METHODS: A real 3D-geometry of the fourth cervical vertebra had been made by the commercially available system ATOS I…

Characterization of a mesenchymal-like stem cell population from osteophyte tissue.

Osteophytes are a distinct feature of osteoarthritis (OA). Their formation may be related to pluripotential cells in the periosteum responding to stimulus during OA. This study aimed to isolate stem cells from osteophyte tissues and to characterize their phenotype, proliferation, and differentiation potential, as well as their immunomodulatory properties. Osteophyte-derived cells were isolated from osteophyte tissue samples collected during knee replacement surgery. These cells were characterized by the expression of cell-surface antigens, differentiation potential into mesenchymal lineages, g…

[Surgical treatment of complete dysphagia caused by a giant cervical osteophyte].

Cervical osteophytes (due to new bone formation) occur in 30% of the population in middle or old age, but it is quite exceptional for this pathological condition to cause severe dysphagia. The authors report such a case, in which the diagnosis was confirmed by endoscopy, X-ray and computed tomography. In the absence of any curative treatment, surgery was required to relieve obstruction and an operation was performed via an antero-lateral extra-pharyngeal approach. Comparison with the literature confirms the validity of the authors decision, though some risks were involved.

Mediastinitis due to Cervical Osteophytes in a Patient after Hemithyroidectomy.

Descending necrotising mediastinitis is a terrible disease, usually a complication of an inflammatory process of the pharynx, pathology of t … or wall of the oesophagus occurred as a result of penetrating trauma to cervical and thoracic spine osteophytes during hemithyroidectomy. The pathology section revealed that acute angle osteophytes located at the cervical level injured the oesophagus when the pressure was applied in removing part of the thyroid gland with malignant neoplasia.

Osteophytes may be a rare cause of recurrent pleural effusions.

We report a rare case of a 59 year-old female, who was admitted to the hospital because of a recurrent right-sided pleural effusion. The initial work-up was non-diagnostic. The diagnosis was finally confirmed following medical thoracoscopy, where an osteophyte of a rib was found to protrude sharply into the thoracic cavity. Chronic inflammatory changes of the pleura suggested mechanical irritation due to long lasting friction between this bony structure and the underlying lung. This is the first report where an osteophyte seems to be implicated in pleural pathology. A brief review of the avail…

Detection of osteophytes and subchondral cysts in the knee with use of tomosynthesis.

PURPOSE: To evaluate the diagnostic performance of tomosynthesis in depicting osteophytes and subchondral cysts, with use of magnetic resonance (MR) imaging as the reference, and to test whether the lesions detected at radiography and tomosynthesis are associated with pain. MATERIALS AND METHODS: The study was approved by local institutional review board, and all subjects gave written informed consent. Forty subjects (80 knees) older than 40 years were recruited irrespective of knee pain or radiographic osteoarthritis. Knees were imaged with radiography, tomosynthesis, and MR imaging. Presenc…

A case of obstructive sleep apnoea with anterior cervical osteophytes.

Osteophytes of the cervical spine are usually seen in elderly adults. When prominent, they have been blamed for dysphagia, cough, dysphonia and dyspnoea. This paper reports on an obstructive sleep apnoea (OSA) patient with cervical spinal osteophytes, one cause of airway obstruction. A 75-year-old male complained of pronounced snoring. The diagnosis was mild OSA, apnoea hypopnoea index was 9.4. Patient reported no restrictions in neck movements, experiences of neck pain or neck trauma. Previously, patient underwent a tonsillectomy due to discomfort in the pharyngeal region. A lateral cephalome…

[Cervical osteophytes and ENT disease: three surgery cases].

Vertebral cervical hyperostosis is frequent in the general population but is rarely symptomatic. Dysphagia is the most frequently encountered symptom. We report three cases of cervical hyperostosis leading to ENT symptoms. Two patients had bilateral laryngeal paralysis, rarely described in the literature. These three cases were treated surgically.

Distribution of distal femoral osteophytes in a human skeletal population.

OBJECTIVES: To examine objectively spatial patterns of osteophytes around the distal end of the femur and to identify distinct subgroups. METHODS: A sample of 107 human femora from a large skeletal population were selected for study. These femora included subjects with evidence of late stage osteoarthritis (that is, with eburnation present) and those with no such evidence. The location of osteophytes was recorded using a video camera and digitised computer images were extracted. Multidimensional scaling was used to identify clusters of femora based upon osteophyte location. RESULTS: A distin…

Transpedicular approaches to cervical uncovertebral osteophytes causing radiculopathy.

OBJECT: The anterolateral and posterior transpedicular foraminotomies were developed to enhance the treatment of cervical spondylotic radiculopathy secondary to uncovertebral osteophytes. These novel surgical approaches may be advantageous because they allow direct decompression of the nerve root while preserving normal movement and stability of the motion segment. The use of the pedicle to obtain access to compressive uncovertebral osteophytes, without causing damage to the motion segment, has not been previously reported. METHODS: The authors present the early outcome data obtained in 27 pa…

Updated management strategy for patients with cervical osteophytic dysphagia.

Anterior cervical osteophytes impinging upon the pharynx or esophagus constitute a rare cause of dysphagia. In severe cases, surgical removal of these osteophytes can provide symptomatic relief. We describe a patient of this type who failed to improve postoperatively, only to be found subsequently to have a carcinoma of the base of the tongue. To assist other clinicians in evaluating similar patients, and also to emphasize the great utility of modern radiologic techniques in these cases, we propose a diagnostic algorithm that incorporates magnetic resonance or computerized tomographic imaging.

Pathological Priors Inspired Network for Vertebral Osteophytes Recognition.

Automatic vertebral osteophyte recognition in Digital Radiography is of great importance for the early prediction of degenerative disease but is still a challenge because of the tiny size and high inter-class similarity between normal and osteophyte vertebrae. Meanwhile, common sampling strategies applied in Convolution Neural Network could cause detailed context loss. All of these could lead to an incorrect positioning predicament. In this paper, based on important pathological priors, we define a set of potential lesions of each vertebra and propose a novel Pathological Priors Inspired Netwo…

Empirical Modelling Workflow for Resolution Invariant Assessment of Osteophytes.

OBJECTIVE: Traditional quantitative analysis of bone microstructure in micro-computed tomography (microCT) is dependent on animal scale and requires parametric tuning in new implementations. This study aims to develop an automated and resolution-invariant 3D image processing workflow for quantitative assessment of osteophytes. METHODS: In this workflow, cortical bone was segmented from microCT scans, and a 3D sphere-fitting transform was performed to obtain a thickness map, for which each voxel is assigned a thickness value corresponding to the size of the largest sphere containing the voxel …
