Effect of uterine peristalsis at the time of embryo transfer on the outcomes of IVF cycles: a prospective cohort study.

BACKGROUND: It has recently been discovered that one of the key elements influencing endometrial receptivity is uterine peristalsis. This was a prospective cohort study aiming to assess uterine peristalsis at the time of embryo transfer in women undergoing IVF. METHODS: Uterine peristaltic waves were assessed using transvaginal ultrasound by recording a 3-minute video to determine the frequency and the direction of peristalsis. Primary outcome was clinical pregnancy while secondary outcomes were biochemical pregnancy, miscarriage, ectopic pregnancy and live birth. Outcomes were compared in wo…

Enhancing the diagnostic yield of esophageal manometry using distension-contraction plots of peristalsis and artificial intelligence.

Our prior study reveals that the distension-contraction profiles using high-resolution manometry impedance recordings can distinguish patien … ustom-built software was used to extract the relevant distension-contraction features of esophageal peristalsis. Ensemble methods, i.e., gradient boost, support vector machines (SVMs), and logit boost, were used as the primary machine learning algorithms. Although the individual contraction features were marginally different between the two groups, the disten …

Numerical simulation of peristalsis to study co-localization and intestinal distribution of a macromolecular drug and permeation enhancer.

In this work, simulations of intestinal peristalsis are performed to investigate the intraluminal transport of macromolecules (MMs) and permeation enhancers (PEs). Properties of insulin and sodium caprate (C10) are used to represent the general class of MM and PE molecules. Nuclear magnetic resonance spectroscopy was used to obtain the diffusivity of C10, and coarse-grain molecular dynamics simulations were carried out to estimate the concentration-dependent diffusivity of C10. A segment of the small intestine with the length of 29.75 cm was modeled. Peristaltic speed, pocket size, release loc…

Distension contraction plots of pharyngeal/esophageal peristalsis: next frontier in the assessment of esophageal motor function.

Esophageal peristalsis consists of initial inhibition (relaxation) followed by excitation (contraction), both of which move sequentially in the aboral direction. Initial inhibition results in receptive relaxation and bolus-induced luminal distension, which allows propulsion by the contraction with minimal resistance to flow. Similar to the contraction wave, luminal distension has unique waveform characteristics in normal subjects; both are modulated by bolus volume, bolus viscosity, and posture, suggesting a possible cause-and-effect relationship between the two. Distension contraction plots i…

Clinical Symptom Presentation of Hypercontractile Peristalsis in the Era of High-Resolution Manometry: A Single-Center Experience.

BACKGROUND: Hypercontractile motility of the esophagus is occasionally noted on high-resolution manometry (HRM), but its clinical correlations are unclear. We compared symptom severity and clinical presentation of patients with hypercontractile motility of the esophagus. METHODS: This was a retrospective cohort study. We queried a prospectively maintained database for patients who underwent esophageal function testing from October 1, 2016, to October 30, 2018. We included patients with jackhammer esophagus (JE; ≥2 swallows with distal contractile integral [DCI] ≥8,000 mm Hg∙cm∙s), nutcracker

Variable characteristics of viscosity and thermal conductivity in peristalsis of magneto-Carreau nanoliquid with heat transfer irreversibilities.

BACKGROUND: Peristaltic is a basic way of fluid transportation in physiology, engineering and nuclear industry. Importance of peristalsis is due to its contraction and compulsion property of symmetric and asymmetric type channel walls. Another beauty of this mechanism is that the channel walls propagates and push the material along the tube/conduit channel walls. This mechanism shows its presence in physiology while food particles are transferred through esophagus and stomach, urine through intestines, spermatoza transportation in reproductive tract. In industry it is found in roller and finge…

Mechanisms underlying distension-evoked peristalsis in guinea pig distal colon: is there a role for enterochromaffin cells?

The mechanisms underlying distension-evoked peristalsis in the colon are incompletely understood. It is well known that, following colonic distension, 5-hydroxytryptamine (5-HT) is released from enterochromaffin (EC) cells in the intestinal mucosa. It is also known that exogenous 5-HT can stimulate peristalsis. These observations have led some investigators to propose that endogenous 5-HT release from EC cells might be involved in the initiation of colonic peristalsis, following distension. However, because no direct evidence exists to support this hypothesis, the aim of this study was to dete…

Measurement of the axial force during primary peristalsis in the oesophagus using a novel electrical impedance technology.

The oesophagus serves to transport food and fluid from the pharynx to the stomach. Oesophageal function is usually evaluated by means of manometry which is a proxy of the force in the radial direction. However, force measurements in the axial direction will provide a better measure of oesophageal transport function. The aim of this study was to develop a probe based on electrical impedance measurements to quantify the axial force generated by oesophageal contractions, i.e. probe elongation was associated with the axial force. Calibration with weights up to 200 g was done. The dispersion, creep

Association between preoperative pan-esophageal pressurization and peristalsis recovery after per-oral endoscopic myotomy in achalasia.

BACKGROUND: Per-oral endoscopic myotomy (POEM) has become the mainstream treatment for achalasia. Previous studies have found that some patients with achalasia might experience partial recovery of esophageal peristalsis after POEM. However, the clinical significance of this recovery and the associated factors remain to be further elucidated. METHODS: This was a multicenter retrospective cohort study conducted by four academic hospitals in China. Patients diagnosed with achalasia who underwent POEM between 2020 and 2025 were included. Demographics, Eckardt score, gastroesophageal reflux diseas…

Assessing mechanical function of peristalsis with functional lumen imaging probe panometry: Contraction power and displaced volume.

BACKGROUND AND AIMS: The distal contractile integral (DCI) quantifies the contractile vigor of primary peristalsis on high-resolution manometry (HRM), whereas no such metric exists for secondary peristalsis on functional lumen imaging probe (FLIP) panometry. This study aimed to evaluate novel FLIP metrics of contraction power and displaced volume in asymptomatic controls and a patient cohort. METHODS: Thirty-five asymptomatic controls and adult patients (with normal esophagogastric junction outflow/opening and without spasm) who completed HRM and FLIP panometry were included. The patient grou…

Mode Switch of Ca2 + Oscillation-Mediated Uterine Peristalsis and Associated Embryo Implantation Impairments in Mouse Adenomyosis.

Adenomyosis is a debilitating gynecological disease of the uterus with no medicinal cure. The tissue injury and repair hypothesis for adenomyosis suggests that uterine hyperperistalsis or dysperistalsis plays a pivotal role in establishing adenomyotic lesions. However, specific impairments in uterine peristalsis and the underlying cellular signals for these changes in adenomyosis remain elusive. Here, we report a precision-cut uterine slice preparation that preserves in vivo uterine architecture and generates peristalsis similar to that seen in the whole uterus. We found that uterine peristals…

Analysis of Ureteral Diameter and Peristalsis in Response to Irrigant Fluid Temperature Changes in an In Vivo Porcine Model.

Objective: It has been previously reported that warming irrigation fluid higher than body temperature may decrease ureteral spasm and thereby facilitate ureteroscopic access to the proximal ureter. Our objective was to examine the effects on ureteral peristalsis and ureteral diameter if the irrigant was warmed to just under the biological threshold for injury. Materials and Methods: Two female adult Yorkshire pigs were studied in this pilot study. In the first pig, a dilute mixture of contrast and irrigation fluid at 37°C and then at 43°C was instilled for 30 minutes into each renal pelvis thr…

Effect of indomethacin on unilateral obstructed renal pelvis and on ureteral peristalsis. Experimental study in awake sheep.

An experimental model to study the physiology of the obstructed kidney in awake sheep is described. Unilaterally, a kidney was obstructed, and the pressure in the renal pelvis was measured before and after intravenous injection of indomethacin. This experiment study on awake sheep enabled us to compare the clinical behavior of the animal with the recorded objective data, to demonstrate the rapid decrease in pressure in the obstructed renal pelvis. In addition, we observed a decrease in ureteral peristalsis frequency following the injection of indomethacin.
