Peptic ulcer--a new look.

This review covers major advances in peptic ulcer disease over the last 25 years. Flexible endoscopy enables accurate diagnosis of peptic ulcer to be made and its introduction made possible the large number of controlled clinical trials on the use of various agents in peptic ulcer treatment. The histamine H-2 receptor antagonists, which reduce gastric acid output, were the first major group of potent ulcer healing drugs introduced. In our patient population with chronic aspecific abdominal complaints Helicobacter pylori gastritis is no rarity. This entity has been diagnosed in 10 children out of 48 (mean age: 8 years 11 months) who underwent an endoscopy of the upper gastrointestinal tract. Dose-finding clinical assay of triletide in gastric and duodenal ulceration.

Three groups of 15 patients with endoscopically confirmed active gastric or duodenal ulcer were treated over a period of 8 weeks with daily doses of 1, 1.5 or 2 g triletide, an anti-ulcer agent which acts by enhancing the mucosal defence capacity. Acute erosive gastritis in Port Moresby.

A retrospective analysis of 50 cases of haematemesis and melaena during the period from June 1978 to July 1983 at Port Moresby General Hospital (PMGH) was undertaken to establish the cause, clinical profile and prognosis of this problem. Acute erosive gastritis is the commonest cause of acute gastro-intestinal bleeding found in this study. The significance of its occurrence, age and sex distribution, associated aetiological factors and management are discussed.

[In-patient treatment of peptic ulcer with cimetidine. I. Effect on duodenal ulcer healing (author's transl)].

The effect of 300 mg cimetidine q.i.d. on ulcer healing was studied in a controlled double-blind clinical trial of 71 in-patients with duodenal ulcer. Healing occurred in 48.5% of patients in the cimetidine group after two weeks, and in 20.6% in the placebo group (P less than 0.05). The healing rate was 88% in the cimetidine group at four weeks, 79.4% in the placebo group. Only during the first day was ulcer pain significantly reduced in the cimetidine-treated patients.
