BACKGROUND: The tennis serve is a complex, high-velocity motion dependent on the efficient transfer of energy through the kinetic chain, from the lower extremities to the racquet. Fatigue-induced alterations in this chain are primary precursors to overuse injuries, such as rotator cuff tendinopathy and elbow medial collateral ligament stress. However, current wearable monitoring solutions predominantly rely on unimodal kinematic data, failing to capture the neuromuscular compensatory mechanisms that precede mechanical performance degradation.
METHODS: This study introduces a novel multimodal

The emergence of glucagon-like peptide-1 receptor agonists (GLP-1RAs) and dual incretin-based therapies has fundamentally transformed obesity pharmacotherapy, enabling magnitudes of non-surgical weight loss that were previously unattainable. Yet, the clinical success of these treatments cannot be measured in kilograms alone. Total body weight is a composite, tissue-nonspecific endpoint that fails to distinguish between adipose reduction and losses in skeletal muscle mass, strength, and physical function-compartments of direct relevance to metabolic health, functional independence, and long-ter

BACKGROUND: Inadequate neuromuscular blockade (NMB) reversal can cause residual NMB (RNMB) and hypoventilation, but the incidence after sugammadex or neostigmine for kidney transplantation is unknown. We compared hypoventilation for up to three days after neostigmine or sugammadex for kidney transplantation, adjusting for RNMB, opioids, and other confounders.
METHODS: This single-center double-blind trial randomized adults undergoing renal transplantation with rocuronium-based NMB and reversal with neostigmine at train-of-four count (TOFC) ≥ 2 or sugammadex at post-tetanic count PTC ≥ 1 follo

Dyspnoea is a severe symptom, causing immediate distress and significantly impairing health-related quality of life, particularly in chronic conditions. Pulmonary rehabilitation is a cornerstone in the management of chronic obstructive pulmonary disease, as supported by evidence highlighting its efficacy in improving dyspnoea, aerobic capacity, and health-related quality of life. In this multidisciplinary approach, much of those physical effects are enabled by a physiotherapist-led intensive training strategy: exercise training. In recent years, numerous studies have supported the benefits of

Accurate soleus (SOL) activation assessment is essential for Achilles tendon rupture (ATR) recovery, yet direct measurement remains a clinical challenge. This study proposes a physics-informed transformer neuromusculoskeletal model (NMM) to estimate muscle activations during various locomotor tasks. We evaluated the framework using data from 40 participants, including 20 healthy controls and 20 postoperative patients with ATR. By integrating forward dynamic constraints, the NMM estimates SOL activation using joint kinematics and a minimal subset of superficial electromyography (tibialis anteri

BACKGROUND: Dysregulated peptidyl deiminase 2 (PAD2) and aberrant protein citrullination (PC), a posttranslational modification (PTM), are involved in various inflammatory and neurodegenerative diseases. We previously showed in transgenic mice and postmortem human tissues that PC and PAD2 are altered in amyotrophic lateral sclerosis (ALS), a neurodegenerative disease characterized by motor neurons loss, paralysis, and death. Herein, we investigated the role of PAD2 in ALS by PAD2 knockout in a SOD1-ALS mouse model.
METHODS: To investigate the role of PAD2-induced citrullination in ALS pathoge

INTRODUCTION: Cough impairment in neuromuscular diseases (NMD) increases pulmonary infection risk. Peak cough flow (PCF) is crucial for assessment, with spirometry (PCFs) as gold standard. Portable peak flow meters (PCFp) are practical, yet their comparability with spirometers in NMD remains debated. This study compared PCFs and PCFp in NMD patients with restrictive lung disease.
METHODS: A prospective, cross-sectional observational study included patients with NMD from a home mechanical ventilation unit. Inclusion criteria comprised a definitive diagnosis of NMD and the ability to perform fo

Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy characterized by rapidly progressive limb weakness and areflexia. While the classical form is well-recognized, variants such as Miller Fisher syndrome (MFS) and Bickerstaff brainstem encephalitis (BBE) also exist, each with distinct clinical features. Rarely, these syndromes may present in combination, forming GBS-MFS-BBE overlap syndromes. These atypical presentations pose significant diagnostic challenges that may delay the initiation of appropriate treatment. We report the case of a 60-year-old male who initia

Botulinum toxin (BoNT), a neurotoxic protein produced by Clostridium botulinum, is widely used for cosmetic and therapeutic purposes, including managing muscle hyperactivity, movement disorders, and chronic migraines by blocking acetylcholine release at neuromuscular junctions. While generally safe, it can cause localized adverse effects (erythema, ptosis, pain) and rare systemic complications (dysphagia, dysarthria, respiratory distress), particularly with improper dosing. From August 2024, This case report presents three female patients, aged 25, 50, and 49, in the Affiliated Hospital of Yun

Botulinum neurotoxins (BoNTs) are the most potent toxins known, causing life-threatening flaccid paralysis by blocking acetylcholine release at neuromuscular junctions. Clinical botulism results in respiratory failure, requiring prolonged artificial ventilation for survival. The only specific therapy is antitoxin, which neutralizes circulating toxin but cannot affect toxin within neurons, resulting in a narrow therapeutic window. Due to its high potency and lack of treatment options, large-scale BoNT exposures present profound risks to human life. Thus, there is an urgent need for rapidly acti
