[Diagnosis and treatment of middle ear myoclonic tinnitus].

Middle ear myoclonus tinnitus is an abnormal sound perceived by unusual contraction of the stapes muscle or tensor tympani muscle. The current diagnostic methods include medical history inquiry, physical examination and audiological tests. The diagnosis was confirmed by tympanic exploration of myoclonus. Etiological treatment is considered to be the first treatment of choice, supplemented by behavior therapy and pharmacotherapy. Surgery will be performed when conservative treatment fails. This article reviews the diagnosis and treatment of middle ear myoclonus tinnitus.

New tender, bleeding papule on the left ear.

Chondrodermatitis nodularis helicis (CNH) is a painful, inflammatory condition that impacts the skin of the ear. It is commonly associated with pressure on the pinna causing a nodule that may have erythema, bleeding and exudate. We present a case of a woman in her 60s with a history of basal cell carcinoma who presented with a new tender spot on the antihelix of her left ear. The tenderness of the new spot forced her to switch from holding her phone to her left ear to using her right ear. A shave biopsy confirmed CNH and ruled out non-melanoma skin cancer. Although most prior cases report asso…

Pharyngeal Arches, Chapter 2: Ear and Neck Abnormalities.

The pharyngeal arches are the foundation of face and neck development. Impaired development of these embryologic structures can result in craniofacial abnormalities. Surgeons who manage head and neck pathology will invariably encounter conditions associated with aberrant pharyngeal arch anatomy, and a thorough understanding of the normal development of these structures is paramount to accurate diagnosis and treatment. This manuscript is the second of a four-part series written for plastic surgeons, focusing on the abnormal development of pharyngeal arches leading to pathologic ear and neck ano…

Perception of cauliflower ear amongst combat sport athletes.

OBJECTIVE: Cauliflower ear, or "hematoma auris," is a permanent condition that is typically viewed as a deformity. Despite this, it has anecdotally been observed that combat sport athletes view cauliflower ear as a respected aesthetic trait. This study characterizes and quantifies the differences in initial impressions of subjects with cauliflower ear between combat sport athletes and the general population. METHODS: In a cross-sectional survey, participants were shown frontal and profile views of four subjects with cauliflower ear and five control subjects. Respondents rated the subjects on …

Middle ear adenoma: Cytohistologic features and differential diagnosis.

Middle ear adenomas are rare, low grade glandular neoplasms with epithelial and neuroendocrine components and with varying patterns of differentiation. Due to the rarity of this tumor, there is a dearth of publications detailing the cytological features. We herein review our institution's pathological database for cytological material between 1992 and 2022 for MEA specimens and discuss possible differential diagnoses based on clinical, pathological, and cytologic data and material.

Real-time monitoring of middle ear prosthesis coupling.

INTRODUCTION: In reconstructive middle ear surgery, acoustic quality has received a high level of attention in recent years. Careful intraoperative selection and positioning of passive middle ear prostheses during tympanoplasty with ossiculoplasty is essential to ensure satisfactory sound transmission and postoperative hearing outcome. The reconstruction quality of the ossicular chain (OC) can be evaluated intraoperatively using a surgical assistance system which is based on a real-time monitoring system (RTM system) that acquires the transmission (middle ear transfer function (METF)) with ele…

Microbubble-assisted ultrasound for inner ear drug delivery.

Treating pathologies of the inner ear is a major challenge. To date, a wide range of procedures exists for administering therapeutic agents to the inner ear, with varying degrees of success. The key is to deliver therapeutics in a way that is minimally invasive, effective, long-lasting, and without adverse effects on vestibular and cochlear function. Microbubble-assisted ultrasound ("sonoporation") is a promising new modality that can be adapted to the inner ear. Combining ultrasound technology with microbubbles in the middle ear can increase the permeability of the round window, enabling ther…

Inner ear organoids: Recent progress and potential applications.

The inner ear is one of the most complicated structures that harbor organs for the perception of sound and balance, which is deep in the temporal bone and challenging to operate. Organoids serve as promising platforms for understanding developmental processes and pathological dysfunctions and discovering therapeutic drugs and gene therapy strategies for disorders in the inner ear. To better understand the origin and application value of organoids, we reviewed the developmental history and advancement of inner ear organoid research. We summarized the cell sources of organoids and the matrices s…

Malignant Melanoma of the Middle Ear: Case Report.

We present a case of malignant melanoma in the middle ear in a 63-year-old woman who presented with severe pain on the right side of her face and head, tinnitus, photophobia, and hemorrhagic discharge from the right ear, as well as right facial nerve paralysis. After more than two years of conservative treatment for otitis media and sclerotic mastoiditis, a pathologically enlarged lymph node on the right side of the neck appears. It is removed, and the histopathological examination reveals that it is metastatic malignant melanoma. Imaging examinations reveal a tumoral lesion in the middle ear …

Middle Ear Meningioma With Recurrence Secretory Otitis Media.

Middle ear meningiomas (MEMs) are rare tumors that can present with nonspecific symptoms, posing challenges in diagnosis and management. This case report focuses on a middle-aged female patient who was misdiagnosed with secretory otitis media for 5 years. However, further evaluation through computed tomography imaging and subsequent pathologic biopsy revealed the presence of a MEM. The patient underwent surgical and gamma knife resection of the tumor and follow-up examination after 1 year showed no signs of recurrence. This case report highlights the importance of considering meningiomas in th…

Prevalence of cauliflower ear in high level judoka.

Judo is an Olympic sport, and the way of its performing can lead to repetitive blunt injuries on head and ears. The chronic consequences of such traumata on the auricle are the formation of so-called cauliflower ear. This condition is painful, can lead to interruptions in the training process and long-term consequences for the athlete's health. There is limited knowledge of epidemiological data about cauliflower ear deformities in judo. Evaluation of the prevalence of cauliflower ear among judokas based on their profile pictures on the international judo federation was performed. A large cohor…

Battery Lifespan of an Implantable Middle Ear Device.

BACKGROUND: The Carina system (Cochlear Ltd., Sydney, Australia) is a totally implantable device providing acoustic amplification in adult patients with moderate-to-severe sensorineural or mixed hearing loss. One of the main concerns about such a totally implantable device has been represented by the subcutaneous battery lifespan. The aim of this article is to report the analysis of battery performances in a series of Carina-implanted patients after a long follow up. METHODS: In this retrospective study, the technical data of a series of patients implanted with the Carina middle ear implant i…

Heterotopic Pancreas in Middle Ear: A Case Report.

Heterotopic pancreas is the congenital presence of pancreatic tissue outside its normal location in the absence of vascular and anatomical connection with the main pancreas. To our knowledge, no case of heterotopic pancreas cyst in the middle ear has been reported to date. In this study, we report the first case of a 6-year-old boy with ectopic pancreas in the area of middle ear. The patient underwent canal wall down mastoidectomy with tympanoplasty. It was finally diagnosed as ectopic pancreas (left middle ear). During the 6-year follow-up, no evidence of recurrence or residual disease in the…

Neuralgia and Atypical Facial, Ear, and Head Pain.

Though there have been considerable strides in the diagnosis and care of orofacial pain disorders, facial neuralgias, and myofascial pain dysfunction syndrome remain incredibly cumbersome for patients and difficult to manage for providers. Cranial neuralgias, myofascial pain syndromes, temporomandibular dysfunction (TMD), dental pain, tumors, neurovascular pain, and psychiatric diseases can all present with similar symptoms. As a result, a patient's quest for the treatment of their orofacial pain often begins on the wrong foot, with a misdiagnosis or unnecessary procedure, which makes it all t

Endoscopic Ear Surgery for Congenital Cholesteatoma in Children.

BACKGROUND: Endoscopic ear surgery is a promising technique for removing congenital cholesteatoma in children. It can provide greater visual access to hidden areas of the middle ear and facilitate middle-ear manipulation. This study compares a single-center experience in manag- ing congenital cholesteatoma with an endoscopic approach with that in managing congenital cholesteatoma with a conventional microscopic approach. METHODS: Records of consecutive patients aged under 8 with congenital cholesteatoma confined to the middle ear at our tertiary referral hospital from January 2013 to December…

[Middle-ear cholesteatoma and otoscopy in primary care].

Middle-ear cholesteatoma is a serious illness of the ear, which untreated can often lead to serious complications. The treatment is always surgical. The operation focuses on removing the cholesteatoma and repairing damaged structures like the ossicular chain. It is of utmost importance, that primary care physicians maintain a high suspicion for the presence of cholesteatoma. The most accessible tool is otoscopy. This review gives an overview of symptoms, signs in otoscopy and treatment modalities of cholesteatoma patients in Denmark.

Middle Ear Adenomatous Neuroendocrine Tumor: A Rare Entity.

Middle ear adenomatous neuroendocrine tumors (MEANT) are rare, mainly benign neoplasms and account for less than 2% of the middle and inner ear neoplasms. There are no specific radiological or clinical findings; therefore, the diagnosis is often difficult. Biopsy prior to surgery is often proposed. The definite treatment is the complete surgical resection. We present an uncommon clinical report of a MEANT, which was treated successfully with surgical excision.
