Three patients with complex perineal fistula after extensive pelvic surgery and radiotherapy underwent surgical treatment combining a biological mesh for pelvic floor reconstruction and a unilateral superior gluteal artery perforator (SGAP) flap for filling of the perineal defect. All patients had both fecal and urinary diversion. Two fistulas originated from the small bowel, necessitating parenteral feeding, and one from the bladder. UNLABELLED: The objective of this review was to address the main indications for palliative surgery in the treatment of ovarian cancer.
DESIGN: Articles from MEDLINE/PUBMED, EMBASE, and LILACS databases up to May 05, 2012 were included with no bars on foreign languages. The key words used were taken from the Medical Subject Headings and were as follows: ovarian cancer AND palliative surgery, ovarian cancer AND complications, and ovarian cancer AND intestinal obstruction. Tuberculosis of the gastrointestinal tract (GIT) occurs as a primary lesion or secondary to a focus of tuberculosis elsewhere in the body, most commonly in the lungs. Tuberculosis can affect any part of the GIT from the oesophagus to the anal canal. From 1987 to 1988, 122 patients with rectal carcinoma underwent Mile's procedure with their artificial anus reconstructed at the perineal region. In 59 patients (48.36%), complications occurred including wound infection on the perineum, mucosa prolapse of the artificial anus, anus retraction or severe edema, and perineal erosive dermatitis. The causes of the complications and the therapeutic approach were discussed.

A complete 8-year experience using the circular stapling instrument is described; 109 rectal and 59 oesophageal anastomoses were performed. Sixteen patients underwent anterior resection and coloanal anastomosis between October 1984 and September 1987. Indications included adenocarcinoma of the low rectum in 9, villous tumor in 2, carcinoid tumor in 1, radiation proctitis in 1, adult onset Hirschprung's disease in 1, rectourethral fistula in 1, and megarectum in 1. From April 1975 through April 1983, 152 patients were treated for localized adenocarcinoma of the prostate with interstitial implantation of 125iodine seeds. In ulcerative colitis, polyposis coli, deep seated cancer and other diseases of the rectum the bowel continuity can be restored without loss of continence, despite the removal of the entire ampulla recti. After proctocolectomy and ileoanostomy the loss of the natural receptaculum is compensated through the positioning of an ileal pouch. After total resection of the rectum and coloanal anostomosis the excised ampulla is replaced with acceptable functional results by the mobilised left colon.

The authors analyse the results of a survey of 450 patients who had undergone abdominoperineal resection. 142 replies were exploitable. After recalling the analytical criteria, they specify the incidence of sexual dysfunctions by sex and causal disease. AIM: Most patients presenting with rectourethral fistula acquire it as a complication of radiotherapy for prostate cancer, as a result of injury to the rectum during prostatectomy, through trauma or from Crohn's disease.
