Perioperative lung expansion and pulmonary outcomes after open abdominal surgery versus usual care in the USA (PRIME-AIR): a multicentre, randomised, controlled, phase 3 trial.

BACKGROUND: Postoperative pulmonary complications (PPCs) are a leading cause of morbidity, death, and increased use of health-care resources. We aimed to determine whether a perioperative lung expansion bundle including individualised intraoperative management reduces PPC severity in patients undergoing major open abdominal surgery compared with usual care. METHODS: In this multicentre, randomised controlled phase 3 trial (PRIME-AIR), we enrolled adult patients (age ≥18 years) scheduled for an elective open abdominal surgery that would last at least 2 h, who were at intermediate or high risk

A Retrospective Comparison of Transcutaneous and End-Tidal Carbon Dioxide Monitoring in Pediatric Polysomnography.

Background Accurate monitoring of carbon dioxide (CO₂) is important in pediatric polysomnography (PSG), particularly for identifying nocturnal hypoventilation, a condition with significant clinical consequences in children, especially those with neuromuscular or pulmonary disorders. Two primary modalities are available for non-invasive CO₂ estimation: end-tidal CO₂ (EtCO₂) and transcutaneous CO₂ (TcCO₂). While both are widely used, their diagnostic equivalence remains unclear. This study aimed to address the clinical question of whether TcCO₂ and EtCO₂ measurements in pediatric PSG can replace

Non-invasive high voltage electrical stimulation as a monitoring tool of nerve root function in lumbosacral surgery.

OBJECTIVE: To verify the safety and clinical use of non-invasive high-voltage electrical stimulation (HVES) in patients with compressive radiculopathy. To test the feasibility of HVES to survey nerve root function during lumbosacral surgery. METHODS: In 20 patients undergoing lumbosacral surgery for degenerative spinal diseases, compound muscle action potentials (CMAPs) evoked by maximal HVES were bilaterally recorded throughout surgery from L3 to S2 radicular territories. A preliminary study was performed in awake patients to rule out detrimental effects caused by HVES. RESULTS: Preoperativ

The effect of oral pyridostigmine bromide nerve agent prophylaxis on return of twitch height in persons receiving succinylcholine.

Prophylactic pyridostigmine is used for troops under threat of organophosphate exposure and can affect the neuromuscular blocking agent succinylcholine. The purpose of this study was to determine what effect pyridostigmine would have on neuromuscular blockade from succinylcholine. Thirty-nine subjects were enrolled in this double-blind, placebo-controlled investigation and assigned to either the pyridostigmine or the placebo group. All subjects were evaluated for pyridostigmine or placebo side effects and given an anesthesia induction that included the neuromuscular blocking agent succinylchol

A prospective, open-label trial of clevidipine for controlled hypotension during posterior spinal fusion.

OBJECTIVES: Controlled hypotension is one means to limit or avoid the need for allogeneic blood products. Clevidipine is a short-acting, intravenous calcium channel antagonist with a half-life of 1 to 3 minutes due to rapid metabolism by non-specific blood and tissue esterases. To date, there are no prospective evaluations with clevidipine in the pediatric population. We prospectively evaluated the dosing requirements, efficacy, and safety of clevidipine for ontrolled hypotension during spinal surgery for neuromuscular scoliosis in the pediatric population. METHODS: Patients undergoing poster

Real-world outcomes of spinal muscular atrophy treatment with onasemnogene abeparvovec in Croatia: a comprehensive case series and literature review.

INTRODUCTION: The development of novel treatment options and the implementation of newborn screening programs have significantly transformed the landscape of care for patients with spinal muscular atrophy (SMA). In a relatively short span, SMA has evolved from a debilitating and fatal disorder into a treatable condition, primarily due to advancements in gene-targeted therapies. Onasemnogene abeparvovec-xioi, an adeno-associated viral vector-based gene therapy delivering a functional copy of the SMN1 gene, has shown significant efficacy in improving motor function and survival rates. Stride-to-Stride Fluctuations and Temporal Patterns of Muscle Activity Exhibit a Stronger Relationship in Running-Induced Fatigue.

During gait, the temporal patterns of stride-to-stride fluctuations and muscle activation exhibit similar responses. This study examined if increasing task demands such as fatigued high intensity running affects this relationship. Eleven experienced runners completed two 400-m runs (R1, R2) with a 3-min break to induce fatigue. Stride Time Intervals (STI) were measured using inertial sensors (IMUs), while Inter-Muscle Peak Intervals (IMPI) were derived from the electromyographic (EMG) activity of the Vastus Lateralis and Gastrocnemius Medialis. We calculated the Hurst exponent and coefficient

Effects of high-intensity interval cross training and classical training on physical and sprint performance in male junior sprinters.

High-Intensity Interval Cross Training (HIICT) combines sprinting, plyometric, and functional strength exercises, but its effects compared with classical sprint training in male junior sprinters remain unclear. This study examined the effects of HIICT and Classical Training (CT) on physical and sprint performance in male junior sprinters. Sixty-four male junior sprinters were randomly assigned to an HIICT group or a CT group for an 8-week intervention. Both groups completed similar overall internal training loads. Assessments before and after the intervention included sprint performance, jump

Dose duration effect of concurrent and synchronized motor cortex theta burst stimulation and upper-limb neuromuscular electrical stimulation.

Intermittent theta burst stimulation (iTBS) can induce rapid cortical facilitation, while neuromuscular electrical stimulation (NMES) can activate sensorimotor networks to excite the central nervous system. Application of iTBS and NMES delivered concurrently at a synchronized 50 Hz frequency was recently shown to elicit greater but transient corticospinal facilitation compared to each intervention. Our current study aimed to investigate the dose-duration neuromodulation effects of concurrent and synchronized associative stimulation using iTBS-NMES. A single dose consisted of 600 pulses of iTBS

A randomized cross-over study protocol to evaluate long-term gait training with a pediatric robotic exoskeleton outside the clinical setting in children with movement disorders.

Individuals with neuromuscular disorders display a combination of motor control deficits and lower limb weakness contributing to knee extension deficiency characterized by exaggerated stance phase knee flexion. There is a lack of evidence for long-term improvement of knee extension deficiency with currently available clinical treatment programs. Our previous work testing a wearable robotic exoskeleton with precisely timed assistive torque applied at the knee showed immediate increases in knee extension during walking for children with cerebral palsy, which continued to improve over an acute pr

Thrombospondin-4 as potential cerebrospinal fluid biomarker for therapy response in pediatric spinal muscular atrophy.

BACKGROUND AND PURPOSE: Spinal muscular atrophy (SMA) as the second most common neurodegenerative disorder in childhood is characterized by the deficiency of survival of motor neuron (SMN) protein leading predominantly to degeneration of alpha motor neurons and consequently to progressive muscle weakness and atrophy. Besides some biomarkers like SMN2 copy number therapeutic biomarkers for SMA with known relevance for neuromuscular transmission are lacking. Here, we examined the potential of Thrombospondin-4 (TSP4) to serve as a cerebrospinal fluid (CSF) biomarker, which may also indicate treat
