[Noninfectious uveitis in childhood : Clinical aspects, diagnostics, treatment and transition].

Intraocular inflammation in childhood represents a major clinical challenge as if untreated the disease can lead to severe complications and permanent vision loss. The onset is often paucisymptomatic, resulting in delayed detection and consequently a high risk of amblyopia. Pediatric uveitis is classified into idiopathic and associated forms. In juvenile idiopathic arthritis, it is considered an extra-articular manifestation. Among the most common systemic associations are sarcoidosis, tubulointerstitial nephritis and uveitis (TINU) syndrome, Behçet's disease and Vogt-Koyanagi-Harada syndrome.…

Progression From Soft Lens to Piggyback Soft-Scleral Contact Lens System to Facilitate Scleral Lens Use in a Pediatric Patient.

Traumatic injury in the pediatric population often results in significant refractive errors that if left untreated can lead to amblyopia. Scleral contact lenses (ScCLs) have recently gained popularity in this population for efficacy in treatment of significant refractive errors, including irregularity astigmatism and corneal abnormalities. Scleral contact lens intolerance may limit the ability of practitioners to use this modality. Although a soft contact lens (SCL) is generally well tolerated, it cannot treat refractive error as effectively as an ScCL. We recently used an SCL followed by an S…

[Unilateral ptosis in children].

Unilateral ptosis in children. Childhood ptosis is a common condition, mostly congenital, and of varying severity. It is characterized by a lowering of the upper eyelid. Unilateral it can affect visual development producing amblyopia. Before a unilateral ptosis of the child, pseudoptosis must be eliminated as the etiology can be a tumour. The diagnosis of congenital ptosis is mainly performed in infants. Clinical examination is based on the evaluation of the eyelid height and the visual axis, eyelid crease, the levator muscle function, Charles Bell sign and the pupil size. Some more common eti…

Periventricular leukomalacia with -16 DS high myopia in a 2½ months old infant: A rare case presentation.

Periventricular leukomalacia (PVL) is coagulation necrosis of white matter seen in preterm, low birth weight (LBW) infants and associated with a range of ocular manifestations. We report a case of 2½ months old (47 weeks gestational age) female infant with PVL associated with bilateral high myopia (-16 DS). On examination, child had difficulty in tracking objects and inferior field defect with preference for superior gaze. She was prescribed spectacles and visual stimulation exercises to preserve existing vision and prevent amblyopia. Periventricular leukomalacia can affect full-term infants a…

Historical evidence supports El Greco's depiction of a neurological condition in his attributed self-portrait.

Icono-diagnosis, the retrospective image-based diagnosis of pathologies, was applied to the canvas "Portrait of an Old Man" (1595-1600), an attributed self-portrait painted by El Greco. The presence of congenital enophthalmos, strabismus, probable amblyopia and signs of left neglect were found. We assume these sign may be consistent an ischemic event affecting the right middle cerebral artery supply territory. Historically, motor activity was not compromised and the painter was able to return to portraiture. Documental evidence indicates, that a few years later (1608), El Greco suffered anothe…

A patient with bilateral persistent pupillary membrane: a conservative approach.

The author describes the case of a girl with bilateral persistent pupillary membranes and good visual acuity. An otherwise healthy 10-year-old girl presented with persistent pupillary membranes in both eyes since childhood. The patient had ametropic amblyopia, which was treated with spectacles and part-time occlusion therapy, with the patient responding conventionally. After therapy, her best-corrected visual acuity was 20/25 in both eyes. She had stereopsis of 50 seconds of arc, orthophoria, and normal extraocular movements. The remainder of the examination was normal. Patients with bilateral…

We report a 72 year-old male, that after a coronary angiogram presented pain, reduced distal skin temperature of both limbs, cyanosis of toes with preserved peripheral pulses and a rapidly progressive renal failure. Afterwards, the patient suffered a sudden bilateral amblyopia and hematochezia. Cholesterol embolism was confirmed with a skin biopsy and fundoscopy. A colonoscopy showed a possible ischemic colitis. After six months of follow up, the patient lost the distal phalanges of three toes, and renal failure stabilized, with a serum creatinine of 2.5 mg/dl. The diagnosis of cholesterol emb…

Area-specific amblyopic effects in human occipitotemporal object representations.

The role of early visual experience in the establishment of human high-order visual areas is poorly understood. Here we investigated this issue using human amblyopia--a developmental visual disorder, which manifests a central vision (acuity) deficit. Previous fMRI studies of amblyopes have described abnormal functional activations in early retinotopic areas. Here we report the surprising finding of a selective object-related abnormality in high-order occipitotemporal cortex. Specifically, we found that face-related cortical areas show a severe disconnection from the amblyopic eye, while buildi…

Lens-induced astigmatism after perforating scleral injury.

Within 6 weeks of a penetrating scleral injury that included vitreous prolapse, a 6-year-old boy developed lenticular astigmatism with a regular component of 5.5 diopters (D). Visible indentational folds in the posterior lens capsule, caused by anterior vitreous fibers and anterior hyaloid, were presumed to be the origin of the astigmatism. Because of decreased visual acuity and the suspicion of early amblyopia, a pars plana vitrectomy with removal of the anterior hyaloid and the critical anterior vitreous fibers was performed. Dense fibrotic tissue between the lens equator and the site of the…

Office Ophthalmology: What is Urgent?

The author describes eye problems in general practice, from problems appearing in neonates to those in the elderly. Refractive problems are most common. Visually threatening eye disease that requires urgent treatment or referral is important, despite its rarity. Early detection and treatment of amblyopia is important in children. In diabetic retinopathy, a potentially blinding disease, maculopathy and proliferative retinopathy require immediate referral. Chronic simple glaucoma and cataracts cause gradual loss of vision in the older adult. Sudden loss of vision can be caused by optic neuritis,…

[The prognosis for the visual outcomes in the surgical treatment of children with congenital partial cataracts].

The authors suggest a method for assessment of the severity of visual function impairment in obscuration amblyopia (OA) by objective (based on recording the visual evoked potentials--VEP) examination of space contrast sensitivity in low-frequency band. Forty-one normal children and 60 children with ophthalmic diseases (96 eyes, of these 55 with aphakia and artiphakia after removal of congenital cataracts and 41 with congenital partial cataracts) were examined. Three types of VEP were distinguished, with different degrees of manifestation. A correlation was demonstrated between VEP type and OA …
