Cardiomyopathy with an LMNA Genetic Variant Affecting Three Consecutive Generations: A Case Series.

We report the case of a family afflicted with cardiac laminopathy who showed atrial fibrillation (AF) and complete atrioventricular block across three generations. Implantable cardioverter defibrillators (ICDs) implantation, or cardiac resynchronization therapy (CRT) were delivered to the three patients (proband; 61 years old, proband's mother: 84 years old, and proband's daughter; 38 years old) to prevent sudden cardiac death or suppress heart failure progression. A novel frameshift mutation (LMNA Exon 9: c.1550dupA;p. N518Efs*34) was found in all three cases through genetic testing, and this…

[Left Hemothorax without Pericardial Effusion due to Pacing Lead Penetration Through Right Ventricular Wall:Report of a Case].

Cardiac perforation by a pacemaker lead that causes left hemothorax without pericardial effusion is rare and life threatening. So, we report a surgically salvaged case. A 55-year-old man underwent insertion of adevice for cardiac resynchronization therapy and defibrillation. On the second postoperative day, the patient fell into shock state after defecation. Electrocardiogram showed pacing failure and computed tomography (CT) showed left hemothorax without pericardial effusion. A drainage tube was placed in the left pleural cavity, and bleeding was massive and continuous. We then performed eme…

Suppression of acute heart failure rehospitalization by biventricular pacing in wide QRS and mid-range ejection fraction.

We present a 66-year-old male patient with heart failure, mid-range ejection fraction and QRS widening suffering from recurrent hospitalizat … g of mitral regurgitation). Because the patient was considered as refractory to optimal medication, cardiac resynchronization therapy (CRT) was performed. After CRT implantation, these factors were improved, and the patient has not experienced recurrent hospitalization for >2 years.

[Implantation of CRT/ICD in women with heart failure – do we have sufficient evidence?].

There are several gender differences in epidemiology, the course, prognosis and treatment of heart failure which is called the epidemic of 21st century. In women heart failure appears at a later age and this can be a sufficient reason for incidence of concomitant diseases. Clinical symptoms of heart failure in females are caused at a higher percentage by non-ischaemic cardiomyopathy and heart failure with preserved ejection fraction of the left ventricle. The history of development of current guidelines for ICD has significantly influenced different, gender-dependent access of patients to ICD

Interventional Management of Atrial Fibrillation in the Chronic Heart Failure Population.

Atrial fibrillation (AF) and heart failure (HF) synergistically interact to exacerbate each other. However, treatment of one entity can greatly improve management of the other. Although historically, permissive medical therapy was the mainstay of AF management in the HF population, recent data strongly favor early, often invasive, intervention for AF to reduce hard HF outcomes. It seems that intervening earlier in the time course of AF, though still not excluding persistent AF from treatment, may have more pronounced effects.

Treatment of atrial fibrillation with third-degree atrioventricular block by pacing His bundle and left bundle branch: Case report.

INTRODUCTION: Substantial advances in cardiac pacing technology have been developed in the past decades. However, efforts to improve pacing technology to achieve physiological electrical activity, such as with cardiac resynchronization therapy, are underway. Permanent His bundle pacing, which directly stimulates the His-Purkinje network and electrically activates both ventricles, simulates physiological electric activity in the heart, and has been considered an ideal pacing strategy to treat arrhythmias. For patients with atrial fibrillation complicated by third-degree atrioventricular block (…

[Cardiac resynchronization in narrow QRS and less affected exercise capacity].

Cardiac resynchronization (CRT) through atrio-biventricular pacemaker therapy is now a well established treatment option for severely symptomatic patients with advanced systolic heart failure (HF) and intraventricular conduction delay, especially left bundle branch block. The purpose of this article is to address possible applications of biventricular pacing beyond the classical CRT indication. In particular, a summary is given of available information on CRT in patients with symptomatic systolic HF, but narrow QRS complex, and those with wide QRS, but less pronounced or even absent clinical H

Physiology of cardiac resynchronization.

Dyssynchronous ventricular contraction, often associated with delayed electrical activation, contributes to worsened clinical status in pati … sed efficiency of contraction, increased mitral regurgitation, and abnormal ventricular remodeling. Cardiac resynchronization therapy (CRT) can improve ventricular performance in two thirds of patients selected based on QRS duration alone. Improved understanding of the pathophysiology of cardiac dyssynchrony will aid in patient selection and in assessment and optimization of respo …

A case report of pacemaker lead perforation causing late pericardial effusion and subacute cardiac tamponade.

Despite medical therapy, the overall prognosis for heart failure (HF) remains poor with high rates of sudden death and death from progressive HF. Device-based therapies offer considerable promise both for the relief of symptoms and for improving prognosis. Cardiac resynchronization therapy is a relatively new and effective treatment for patients with moderate to severe systolic HF and ventricular dyssynchrony. Clinical trials have demonstrated an improvement in quality of life, improved exercise tolerance, decreased HF hospitalizations, and improved survival. Complication rates for patients re…

New insights from a computational model on the relation between pacing site and CRT response.

AIMS: Cardiac resynchronization therapy (CRT) produces clinical benefits in chronic heart failure patients with left bundle-branch block (LBBB). The position of the pacing site on the left ventricle (LV) is considered an important determinant of CRT response, but the mechanism how the LV pacing site determines CRT response is not completely understood. The objective of this study is to investigate the relation between LV pacing site during biventricular (BiV) pacing and cardiac function. METHODS AND RESULTS: We used a finite element model of BiV electromechanics. Cardiac function, assessed as…

Physiological pacing with a DF-1 single chamber defibrillator in a patient with permanent atrial fibrillation and heart block: A case report.

We report a case report of a 68-year-old man with chemotherapy-induced cardiomyopathy and uncontrolled permanent atrial fibrillation. Cardiac resynchronization therapy implantation and atrioventricular junction ablation were planned. DF-1 single chamber defibrillator was connected to lead's defibrillation and a lead destinated to left bundle branch area pacing. This system leads to reduce costs by one-third, improve battery longevity, and provide a more physiological pacing.

Interventional approach to CRT in a patient with drainage of the superior vena cava into the coronary sinus.

A persistent left superior vena cava markedly increases the size of the coronary sinus (CS), which can increase the difficulty of left ventricular (LV) lead placement in patients receiving cardiac resynchronization therapy (CRT). We present a case where the entire superior vena cava drains into the coronary sinus, creating a massive CS. We also describe an interventional approach to LV lead implantation utilizing a combination of delivery systems from different vendors.

Functional behavior of patients with conventional pacemakers undergoing cardiac resynchronization.

BACKGROUND: Cardiac resynchronization therapy (CRT) is an efficient treatment for patients with heart failure (HF), severe ventricular dysfunction and intraventricular block. Conventional pacemakers (CPM) implanted in the right ventricular apical area cause alterations in the normal sequence of cardiac activation similar to those induced by LBBB (left bundle-branch block). Therefore, patients with CPM and advanced HF could be candidates to undergo CRT, but as only small numbers of patients have been evaluated so far, definitive conclusions are lacking. OBJECTIVE: To assess the clinical and fu…

Electrical and mechanical ventricular activation during left bundle branch block and resynchronization.

Cardiac resynchronization therapy (CRT) aims to treat selected heart failure patients suffering from conduction abnormalities with left bundle branch block (LBBB) as the culprit disease. LBBB remained largely underinvestigated until it became apparent that the amount of response to CRT was heterogeneous and that the therapy and underlying pathology were thus incompletely understood. In this review, current knowledge concerning activation in LBBB and during biventricular pacing will be explored and applied to current CRT practice, highlighting novel ways to better measure and treat the electric…

Percutaneous intervention for mitral regurgitation.

Percutaneous treatment of mitral regurgitation (MR) is a promising alternative for patients with functional MR (FMR) who are not appropriate for surgery and are not responding to optimal medical therapy and cardiac resynchronization therapy. Unlike degenerative MR, where repair therapy is clearly preferred, the optimal approach for FMR has not been defined. Challenges for novel mitral repair devices are to demonstrate safety and superior efficacy to medical management in higher risk patients. Transcatheter mitral valve replacement is emerging as a feasible therapy, but requires significant add…
