Assessing access barriers to tuberculosis care with the tool to Estimate Patients' Costs: pilot results from two districts in Kenya.

BACKGROUND: The poor face geographical, socio-cultural and health system barriers in accessing tuberculosis care. These may cause delays to timely diagnosis and treatment resulting in more advanced disease and continued transmission of TB. By addressing barriers and reasons for delay, costs incurred by TB patients can be effectively reduced. A Tool to Estimate Patients' Costs has been developed. It can assist TB control programs in assessing such barriers. This study presents the Tool and results of its pilot in Kenya. METHODS: The Tool was adapted to the local setting, translated into Kiswah

Leadership and Communication Skills Used by Pediatric Residents in the First 5 Minutes of Simulated High-Acuity Events.

OBJECTIVE: Effective leadership enhances clinical team performance through collaboration, resource management, and communication. Currently, most resident physicians learn leadership skills through informal apprenticeship. METHODS: This observational study quantifies the leadership and communication skills (LCS) demonstrated by pediatric residents leading interdisciplinary teams in simulated life-threatening events at a single institution using retrospective video review. Using a checklist of LCS, 3 authors each scored the occurrence of 12 LCS skills within the first 5 minutes of response, pl

ICED OUT: integrated cold-chain eliminating disease outreach. Piloting a motorcycle-enabled laboratory transport system combatting visceral leishmaniasis in rural Kenya.

The fight against neglected tropical diseases (NTDs) in sub-Saharan Africa, particularly visceral leishmaniasis (VL) among Kenya's Pokot tribe, exemplifies the need for improved last-mile healthcare delivery systems. The Pokot face high disease burdens due to their pastoral lifestyle, socioeconomic hardship, and comorbidities such as malnutrition and high exposure to communicable diseases. Despite the availability of VL diagnostics and treatments, limited healthcare access in remote Baringo County exacerbates the disease's endemicity. This study describes a motorcycle-based cold chain transpor

Fear, stigma and Uncertainty: The short and long-term effects of Ebola on survivors, affected families, and community in Bundibugyo, Western Uganda.

In 2014, Uganda was identified as a high-risk country for Ebola Virus Disease (EVD), with a series of outbreaks recorded since 2000. In 2007, the second outbreak in Bundibugyo district resulted in 149 reported cases and 37 confirmed Ebola deaths. Through the outbreak response, a new strain of the Ebola virus (Bundibugyo ebolavirus) was discovered. Although much is known about the nature of Ebola, including disease signs and symptoms, transmission and management, there is limited understanding of the short and long-term sociocultural impacts of the disease in communities. The study team conduct

Unmasking metabolic disruptors: The NEMESIS project's quest for Novel Biomarkers, Evidence on Adverse Effects, and Efficient Methodologies.

Metabolism disrupting chemicals (MDCs) elicit negative effects on metabolically active organs such as the liver and the pancreas, altering normal metabolic processes. Chemicals that are known, or suspected MDCs include compounds found in everyday consumer products and food, making low-dose, continuous exposure inevitable for humans. Through the discovery of chemically induced metabolic disruption, a concern has surfaced whether and how MDCs impact human health and the development of metabolic diseases. This has accelerated research around the topic, and it has been found that exposure to MDCs

Epidemiological profiles and causes of sudden deaths of various ages in Ethiopia: an autopsy-based study.

BACKGROUND: Sudden death is an important global public health issue. An autopsy is an important source of epidemiological data, as the considerable causes of sudden death remain hermetic until postmortem examination. This study is devoted to evaluating the sociodemographic, behavioral, clinical and pathological characteristics of sudden deaths of various ages in Ethiopia. METHODS: This is an observational, prospective, descriptive study that included all sudden deaths observed over 1 year at St. Paul's Hospital and Millennium Medical College, Addis Ababa, Ethiopia. RESULTS: Sudden death (n =

Psychometric performance of tools to measure internalized drug use stigma among people who use drugs and people on medication-assisted treatment in Dar es Salaam, Tanzania.

BACKGROUND: People who use drugs (PWUD) and people who use medication-assisted treatment (MAT) to treat opioid use disorder face severe stigma. Stigma may manifest in delayed health-seeking behaviors, which adversely affects health and increases disease risk. Few validated measures assess internalized drug use stigma or MAT disclosure concerns in sub-Saharan Africa. This study examines the performance of internalized drug use stigma scales and characterizes the prevalence of these stigmas among PWUD and people who use MAT in Dar es Salaam, Tanzania. METHODS: We conducted a cross-sectional sur

Treating young adult cannabis use disorder with text message-delivered peer network counseling.

BACKGROUND: Approximately 16.5% of U.S. young adults have a cannabis use disorder (CUD) and are at risk for negative outcomes. Treatment can reduce cannabis use, but young adults are less likely to seek help than older adults. Peer Network Counseling-txt (PNC-txt) is a brief, text-delivered, Motivational Interviewing-informed substance use intervention focusing on peer relations and activity spaces as mechanisms for behavioral change. PNC-txt has shown evidence of reducing tobacco and cannabis use with adolescents and young adults, but it has not been tested in the context of legal cannabis us

Validation of a New Patient-Reported Outcome Measure of the Functional Impact of Essential Tremor on Activities of Daily Living.

BACKGROUND: The Essential Tremor Rating Assessment Scale (TETRAS) is a popular scale for essential tremor (ET), but its activities of daily living (ADL) and performance (P) subscales are based on a structured interview and physical exam. No patient-reported outcome (PRO) scale for ET has been developed according to US regulatory guidelines. OBJECTIVE: Develop and validate a TETRAS PRO subscale. METHODS: Fourteen items, rated 0-4, were derived from TETRAS ADL and structured cognitive interviews of 18 ET patients. Convergent validity analyses of TETRAS PRO versus TETRAS ADL, TETRAS-P, and the

Case report: Bilateral damage to the immature optic radiation and secondary massive loss of retinal ganglion cells causing tunnel vision.

We describe the case of a 30-year-old woman, who needed a formal report on her visual impairment to seek support from society. She was born preterm, and during her neonatal period, she suffered from bilateral intraventricular hemorrhage (IVH) grade 3, a condition that can cause cerebral visual impairment (CVI) due to damage to the retro-geniculate visual pathways. Individuals with such brain damage of this severity are often restricted by cerebral palsy (CP) and intellectual disability, and thus have a limited ability to cooperate in the assessment of visual function. However, our patient was

Adverse childhood experiences are associated with increased overdose risk in predominately Latinx adults seeking treatment for substance use disorders.

INTRODUCTION: Almost no previous studies explored the relationship between adverse childhood experiences (ACEs) and overdose risk for individuals with substance use disorders (SUDs), and these did not focus on a Latinx population. This study examined the relationship between ACEs, reporting PTSD symptoms, and lifetime experience of overdose in a sample (n = 149) of primarily Latinx adults seeking treatment for substance use disorder (SUD). MATERIALS AND METHODS: Administrative data from an integrated behavioral health and primary care treatment system in Massachusetts were analyzed through bi

Roles of Psychological Distress and Social Support in the Relationship Between Childhood Maltreatment and Perceived Needs for Mental Health Care.

Childhood maltreatment is a major public health issue worldwide. It increases a range of health-risk behaviors, psychological and physical problems, which are associated with an increased need for mental health services in adulthood. Identification of mediating factors in the relationship between maltreatment and seeking mental health care may help attenuate the negative consequences of childhood maltreatment and promote more appropriate treatment. This study aims to examine whether the relationship between childhood maltreatment and perceived need for mental health care is mediated by psychol

Social Media and HPV Vaccine Hesitancy: How Misinformation Affects Latino Parents.

Human papillomavirus (HPV) vaccine coverage in the United States remains suboptimal, with Latino communities facing disproportionately lower rates and a higher burden of HPV-associated cancers. Health misinformation and disinformation (M&D) on social media represents a barrier to HPV vaccine uptake, yet little empirical evidence exists on how such content shapes vaccine decision-making. We conducted a cross-sectional study of 98 Latino parents drawn from two randomized, community-based prospective trials conducted across four U.S. metropolitan areas. Eligible participants confirmed their HPV v
