Load distribution on the radio-carpal joint for carpal arthrodesis.

BACKGROUND AND OBJECTIVE: Carpal fusions are useful for treating specific carpal disorders, maximizing postoperative wrist motion, hand strength, reducing pain and instability of the joint. The surgeon selects the appropriate treatment by considering the degree of stability, the chronicity of the injury, functional demands of the patient and former patient's outcomes as well. However there are not many studies regarding the load distribution provided by the treatment. So, the purpose of this study is to analyze the load distribution through the wrist joint with an arthrodesis treatment and com

The Effect of Distal Scaphoid Excision and Triquetrum Excision on Radioscapholunate Fusion: A Systematic Review.

PURPOSE: Radioscapholunate (RSL) fusion is a motion-preserving limited wrist fusion treatment option for symptomatic radiocarpal arthritis; however, there is no consensus regarding the need for excision of the distal scaphoid or triquetrum. This study aimed to determine the impact of distal scaphoid excision (DSE) and triquetrum excision (TE) in RSL fusion and compare the outcomes and complications. METHODS: A systematic review of the literature was performed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, and relevant clinical studies on RSL fusio

Arthroscopic repair of the scapholunate interosseous ligament.

Scapholunate injuries are the most frequent of the intercarpal ligament injuries in the wrist. Current repair methods generally involve an open approach the dorsal capsule of the wrist. Arthroscopic repair of the dorsal portion of the scapholunate interosseus ligament would carry the advantages of less stiffness and would preserve the important dorsal capsular stabilizers. In the development of this technique, we first sought to determine the anatomic location and accessibility of the dorsal scapholunate ligament and the site in which a suture anchor would be placed. Ten fresh-frozen cadaver l

Optimal fixation of acute scaphoid fractures: finite element analysis.

PURPOSE: The hypothesis of this study was that more stable fixation of acute scaphoid fractures may be achieved by a screw placed perpendicular to the fracture plane than along the long axis of the scaphoid, as previously suggested. We examined this assumption on different fracture patterns using a finite element analysis model. METHODS: A computed tomography scan of an intact scaphoid of a young man provided the data set for all fracture models. We used semiautomatic segmentation to create 3-dimensional computer models of the 3 simple fracture configurations: oblique, transverse waist, and p

Coronal fractures of the proximal scaphoid: the proximal ring sign.

We present two coronal fractures of the proximal scaphoid which were both missed in the acute stage as interpretation of initial radiographs was difficult. In both cases, recognition of the so-called "Proximal Ring Sign" on the PA ulnar deviation radiographs may have helped diagnosis. CT scans were necessary to fully demonstrate the fractures. Open reduction and internal fixation, performed 2 and 4 months after the injury, resulted in union in both cases.

Pressure distribution over the palm region during forward falls on the outstretched hands.

Falls on the outstretched hands are the cause of over 90% of wrist fractures, yet little is known about bone loading during this event. We tested how the magnitude and distribution of pressure over the palm region during a forward fall is affected by foam padding (simulating a glove) and arm configuration, and by the faller's body mass index (BMI) and thickness of soft tissues over the palm region. Thirteen young women with high (n=7) or low (n=6) BMI participated in a "torso release experiment" that simulated falling on both outstretched hands with the arm inclined either at 20° or 40° from t

Technique of Carbon Dioxide Insufflation of the Wrist During Arthroscopic Bone-Grafting Procedures.

Chronic scaphoid fractures are conventionally treated with arthroscopic bone grafting. Traditional dry wrist arthroscopy avoids graft displacement caused by fluid irrigation but faces challenges such as obscured visibility from bleeding and limited operative space. This study introduces a carbon dioxide insufflation technique using pulsed gas perfusion at 5 kPa pressure and 10 mL/min flow rate. By replacing the dry environment, carbon dioxide insufflation dynamically clears intraoperative bleeding, stabilizes the visual field, and expands the joint cavity for improved instrument maneuverabilit

Spider plate fixation: no significant improvement in limited wrist arthrodesis.

Using the Spider plate fixation device, limited wrist arthrodesis was performed on 30 patients over a 2-year period. Developed as an alternative to Kirschner wires and compression bone screws in limited intercarpal fusions, its rigid fixation was expected to yield better results. Five hand surgeons treated 24 men and 6 women with an average age of 45 years. Diagnosis included the following: scaphoid nonunion advanced collapse, scapholunate advanced collapse, scapholunate dissociation, midcarpal arthrosis, and Kienböck disease. The 8-hole regular Spider and 6-hole mini-Spider plates were applie

Scaphoid fractures in children: problems and treatment.

From 1984 to 1999, the authors treated 64 cases of fracture of the scaphoid in children. Causes of injury were sports (n = 27), punching game machines or fighting (n = 22), and traffic accident or other trauma (n = 15). Most (46 cases) were nonunion cases. Eighteen cases were acute. Cast immobilization was performed in 10 acute cases and two nonunion cases. Screw fixation was performed in 52 cases, including 35 cases of bone graft. In 10 of these operated cases, freehand screw insertion was used. Ultimately, good bony fusion was achieved in all cases, but in two nonunion cases a secondary bone

Multiple carpometacarpal dislocations and an ipsilateral scapho-trapezium-trapezoid fracture-dislocation: a rare pattern of injury.

We report a rare case of simultaneous dorsal dislocation of 4 ulnar carpometacarpal joints and dorsoradial dislocation of the trapezium with an associated fracture of the scaphoid tuberosity. The injuries were diagnosed early and treated successfully with closed reduction and transfixation using Kirschner wires. The functional results were excellent at 17-month follow-up.

Problems With Medium-Sized Joints: Wrist Conditions.

Common wrist conditions include fractures and other injuries, osteoarthritis, radial epiphysitis, joint instability, de Quervain tenosynovitis, carpal tunnel syndrome, ganglion cyst, and ulnar neuropathy. The initial history and physical examination, with particular focus on the anatomic structures of the wrist, can narrow the differential diagnosis. Magnetic resonance imaging study can be used to identify soft tissue masses and occult osseous processes, particularly with scaphoid fractures. Computed tomography scan is useful in cases of bony abnormalities, high clinical suspicion of occult fr

[Muscular control of scapholunate instability. An experimental study].

PURPOSE: As long as the neuromuscular stabilizers are intact, a lesion of the scapholunate ligament may or may not progress to a carpal instability. The mechanisms by which the muscles compensate this defect are not very well known. We designed an experimental study with the aim of clarifying these mechanisms. MATERIAL AND METHOD: Using 10 fresh wrists, with no pre-existing lesions, we studied the movements of the scaphoid, triquetrum and capitate produced by the isometrical loading of the muscles which move the wrist, each of them isolated or combined, before and after cutting off the scapho

Clinical, radiological and dynamic CT results of scapholunate intercarpal ligamentoplasty for scapholunate dissociation.

A clinical, radiological and four-dimensional computed tomography (4DCT) assessment of the outcomes of scapholunate intercarpal ligamentoplasty (SLICL) was done with a minimum follow-up of 2 years. Twenty-nine patients (23 men and 6 women) with a mean age of 40 years (22-57) who had chronic scapholunate dissociation were treated with the SLICL procedure. There were 18 cases of dynamic instability and 11 of static instability. The patients were evaluated with a mean follow-up of 61 months (24-94). SLICL significantly reduced pain and increased grip strength and wrist function. On radiographs, t

Carpal bone dislocations: an analysis of twenty cases with relative emphasis on the role of crushing mechanisms.

Twenty cases of carpal bone dislocation were encountered during a 7-year period, with an average of 27 months of follow-up. There were ten types of dislocation in this series; the most common type was transscaphoid perilunate dislocation which was seen in nine cases. In addition, there were two scaphoid subluxations; one volar lunate dislocation; one dorsal perilunate dislocation; one scaphoid perilunate dislocation; one hamate and pisiform dislocation; one transhamate pisiform dislocation; one trapezoid dislocation with dislocation of carpometacarpal joints two to five; one dislocation of the

Upper extremity radionuclide bone imaging: the wrist and hand.

Bone scintigraphy of the hands and wrists represents an important adjunct imaging technique that complements plain film radiographic examination. The use of the three-phase bone scan provides clinical information not only regarding osseous uptake but the blood flow and extravascular distribution of the radiotracer as well. Scintigraphic evaluation of the hands and wrists is employed in acute and chronic conditions. In the event of an equivocal or negative plain film, the bone scan can identify occult fractures. Of particular concern is the identification of scaphoid fractures due to the higher

The use of vascularized grafts from the distal radius for the treatment of Preiser's disease.

PURPOSE: To examine the outcome of vascularized bone grafts (VBGs) used for the treatment of Preiser's disease. METHODS: Over a 10-year period (1993-2003) 8 pedicled VBGs were performed as the primary treatment for Preiser's disease. All patients had preoperative magnetic resonance imaging (MRI) scans that confirmed the diagnosis of avascular necrosis of the scaphoid. Bone revascularization was evaluated by examining preoperative and postoperative MRIs. All patients had a reverse-flow pedicled VBG from the distal radius. The average patient age was 40 years and the mean follow-up period was 3

Atraumatic Scaphoid Avascular Necrosis After Repeated Steroid Injections.

Atraumatic avascular necrosis of the scaphoid is an extremely rare pathology. We present a case of avascular necrosis of the scaphoid following repeat local glucocorticoid injections. A 70-year-old, right-handed woman presented to our clinic with 2 years of atraumatic, progressively worsening left wrist pain and loss of range of motion. Imaging demonstrated fragmentation and avascular necrosis of the scaphoid. The patient underwent uncomplicated scaphoid excision, pisiform excision, and intercarpal arthrodesis, with a good functional outcome. This case highlights both the risk of repeated intr

A Failed 4-Corner Arthrodesis Using a Polyether-Ether-Ketone Implant: A Case Report.

CASE: A 59-year-old man underwent scaphoidectomy and 4-corner arthrodesis with a polyether-ether-ketone (PEEK) circular plate for scapholunate advanced collapse of the wrist. Five years later, he presented with a symptomatic nonunion and radiocarpal arthritis. Total wrist arthrodesis with a dorsal plate was performed. During revision surgery, considerable synovitis was encountered. Histological evaluation revealed a foreign body response likely secondary to PEEK particles. CONCLUSION: Use of PEEK implants can result in an adverse local tissue reaction if particles are generated. This is a rar
