Cardiac resynchronization therapy is a novel nonpharmacologic approach to treating patients who have advanced heart failure with left bundle branch block (LBBB). Such a therapy is based on the original theory that synchronous biventricular pacing is able to reduce the interventricular delay caused by LBBB in patients with heart failure. Although there is convincing evidence that biventricular pacing increases the left ventricular ejection fraction, decreases mitral regurgitation, and improves symptoms caused by heart failure, the percentage of nonresponders to such therapy has been described a…

Bundle-branch re-entrant ventricular tachycardia (BBRVT) is usually seen in patients with dilated cardiomyopathy. The diagnosis should be suspected in case the QRS morphology during the tachycardia is identical to that in sinus rhythm. Identifying patients with BBRVT is important because they can be easily treated by catheter ablation. This report describes a patient previously implanted with a cardiac resynchronization therapy defibrillator (CRT-D) presenting with an electrical storm due to BBRVT.

Incidence of damage to pacemaker and implantable cardioverter defibrillator leads is an emerging problem that should be prevented. The extrathoracic venipuncture approach has been suggested as a technique for venous access to avoid the problem. This report describes the method of double marker-guided venipuncture of extrathoracic subclavian and/or axillary vein. This approach achieves definite, safe, and speedy extrathoracic venipuncture and may be especially suitable for multiple lead placement for cardiac resynchronization therapy.

The coronary sinus, whose electrical features play an important role in cardiac arrhythmias, is the integral part of the cardiac venous system. Here we describe a 67-year-old male patient with congestive heart failure who was referred to our hospital after the failure of the first cardiac resynchronization therapy defibrillator (CRT-D) implantation. During the cannulation of the coronary sinus, the separate orifice of the posterior cardiac vein was demonstrated by the retrograde filling of the coronary sinus via contrast injection into the posterior cardiac vein. Due to the serious tortuosity …

We report a case of inappropriate implantable cardioverter defibrillator (ICD) therapy due to single ventricular fibrillation (VF) zone programming leading to patient death. A remote transmission was received from a patient with a cardiac resynchronization therapy-defibrillator showing sinus tachycardia in the VF detection zone initiating inappropriate shocks and resulting in shock refractory VF. This case report highlights the importance of manufacturer specific ICD programming. In devices without discrimination in the VF zone, a higher rate single VF detection zone and/or addition of a ventr…

BACKGROUND: Current guidelines indicate electrical dyssynchrony as the major criteria for selecting patients for cardiac resynchronization therapy, and 25-35% of patients exhibit unfavorable responses to cardiac resynchronization therapy (CRT). We aimed to evaluate different cardiac mechanical dyssynchrony parameters in heart failure patients using current echo-Doppler modalities and we analyzed their association with electrical dyssynchrony.
METHODS: The study included 120 heart failure with reduced ejection fraction (HFrEF) who underwent assessments for left ventricular mechanical dyssynchr…

Permanent His bundle pacing prevents ventricular dyssynchrony in patients who depend on ventricular pacing and can provide an alternative means to implementing cardiac resynchronization therapy in patients with bundle branch disease and congestive heart failure. Adoption of His bundle pacing, however, has lagged in part due to perceived and real challenges in performing the procedure well and durably. This article focuses on what is required to get on the learning curve and developing technical competence.

The selection of appropriate candidates for mitral surgery among symptomatic patients with nonischemic, chronic, secondary severe mitral regurgitation (NICSMR) remains a clinical challenge. We studied 50 consecutive symptomatic NICSMR patients for a median follow-up of 2.5 years after mitral surgery and concluded that the pre-operative 2-dimensional speckle tracking echocardiography-derived left ventricular torsional profile and QRS width/cardiac resynchronization therapy are potentially important prognostic indicators for post-surgery survival and reverse remodeling.

Persistent left superior vena cava (PLSVC) is a benign vascular anomaly that complicates pacemaker or implantable cardioverter defibrillator (ICD) lead implantation. Thus far, right ventricular lead implantation via PLSVC has been mostly accomplished by forming a U-shaped stylet, necessitating considerable manoeuvring depending on the heart size and geometry. In this article we report a technique of ICD lead implantation via PLSVC, using a coronary sinus delivery system as used today in cardiac resynchronization therapy.

The treatment of progressive and terminal heart failure follows the principle of causative therapy. Therefore, etiology and pathophysiology of the underlying disease and its hemodynamic conditions are indispensable. This applies to coronary artery disease, hypertension, valvular heart disease, the cardiomyopathies with and without inflammation, and microbial persistence similarly. The classic treatment algorithms both in heart failure with and without reduced ejection fraction are based on measures onloading the heart (angiotensin-converting enzyme inhibitors, angiotensin antagonists, beta-blo

The aim of this study was to review published data on gender differences in cardiac electrophysiology and in the presentation and clinical treatment of arrhythmias. The evidence from studies published to date show that women have a higher mean resting heart rate, a longer QT interval, a shorter QRS duration, and a lower QRS voltage than men. Women have a higher prevalence of sick sinus syndrome, inappropriate sinus tachycardia, atrioventricular nodal reentry tachycardia, idiopathic right ventricular tachycardia, and arrhythmic events in the long-QT syndrome. In contrast, men have a higher prev

We report a 57-year-old male patient with dilated cardiomyopathy, NYHA function class III and complete left bundle branch block (CLBBB) who received cardiac resynchronization therapy using His-bundle pacing (HBP) when a left-ventricular lead could not be placed. In 10 months' follow up, his symptoms resolved rapidly, his QRS, left ventricular and left ventricular ejection fraction normalized. Recent reports support the conception that HBP may be an alternative or even better therapy for heart failure patients with CLBBB, especially if biventricular pacing is not feasible.

We present a case of a 55-year-old man with idiopathic dilated cardiomyopathy, ejection fraction (EF) of 27%, left bundle branch block with QRS duration of 160 ms, NYHA class III, and optimal medical therapy for at least 6 months who was referred to our division for cardiac resynchronization therapy (CRT) using a biventricular pacing device. During the operation, the coronary sinus branch cannulation failed using classical angioplasty guide wire techniques. The pacing lead was successfully implanted into the posterior target vein using a telescopic dual-catheter system.

PURPOSE OF REVIEW: Cardiac resynchronization therapy is a nonpharmacological treatment option in patients with heart failure and left bundle branch block but response rates are still disappointing.
RECENT FINDINGS: Extent of mechanical left ventricular asynchrony as detected by tissue Doppler imaging has emerged as an independent predictor of outcome to CRT. In addition, long-term therapy delivery may be further improved through optimized lead positioning and pacemaker programming.
SUMMARY: Tissue Doppler imaging should be included in the evaluation of potential CRT candidates but standardiz…

BACKGROUND: Atrial lead perforation may lead to pneumopericardium or pneumothorax within a few days of device implantation.
METHODS AND RESULTS: We report a case of atrial lead perforation 6 years after cardiac resynchronization therapy implantation, which resulted in pneumopericardium and pneumothorax.
CONCLUSION: Although pneumopericardium caused by atrial lead perforation can spontaneously resolve with conservative treatment, as it did in this case, treatment should be decided based on the patient's general condition and lead performance.

Left bundle branch block (LBBB) is considered a marker of underlying structural cardiac disease. To determine whether LBBB is cause or consequence of deterioration of left ventricular (LV) function is difficult as both are often diagnosed concomitantly. We discuss a patient where reversal of LBBB and subsequent normalization of LV function was observed after 2 different therapies, first after start of heart failure medication, and years later after implantation of a cardiac resynchronization device. This indicates that LBBB per se may result in the development of non-ischemic cardiomyopathy an
