Aortic perforation due to cardiac resynchronisation therapy defibrillator lead placement: Case report and medicolegal considerations.

A 45-year-old woman with dilated cardiomyopathy was admitted for the upgrade of a previously implanted pacemaker. Echocardiography showed intraventricular dyssynchrony and a low ejection fraction (0.35). Treatment with a cardiac resynchronization therapy defibrillator (CRT-D) was selected and the device was implanted. CRT-D interrogation revealed proper function. Following procedure termination, the patient went into cardiac arrest and died despite resuscitation attempts. An autopsy revealed that the medial aspect of the right atrium was pierced by an active lead and that the aorta had a deep …

A rare homozygous variant of CHKB induced severe cardiomyopathy and a cardiac conduction disorder: a case report.

BACKGROUND: The CHKB (choline kinase beta) gene plays a crucial role in regulating mitochondrial function and choline metabolism. Mutations … blocks in the proband were considered to be associated with choline metabolic impairment, and thus cardiac resynchronization therapy would benefit the patient. Furthermore, the missense homozygous or compound heterozygous variants of CHKB, as well as the combined compound heterozygous missense and nonsense variants of CHKB, usually lead to neurological impairments and muscular we …

Restoration of Cardiac Function in Left Bundle Branch Block-Induced Cardiomyopathy via Conduction System Pacing: A Case Report.

BACKGROUND Left bundle branch block (LBBB) is not uncommon. In certain cases, patients with LBBB exhibit a deterioration of cardiac function … therapy was initiated; however, her symptoms persisted after 6 months of therapy. Following CSP for cardiac resynchronization therapy, the patient's LVEF improved to 54% at 6 months and further increased to 56% at 30 months, approaching normalization. CONCLUSIONS LIC is a recently identified condition wherein patients with LBBB experience left ventricular dysfunction and mechanica …

[Conduction system pacing (CSP) for every patient? : Should CSP replace classical right ventricular and biventricular stimulation?].

Direct stimulation of the conduction system (conduction system pacing, CSP) can be performed at multiple levels and for different indication … tion with the established stimulation modalities of the right ventricle in isolated bradycardia and cardiac resynchronization therapy (CRT) in heart failure. A clear distinction and recommendation in the guidelines are not yet definitive, also due to pending and constantly accumulating data and an evolving portfolio of sheaths and leads from all manufacturers; however, overall the …

[Painful left bundle branch block].

The author presents a case where the frequency-dependent left bundle branch block causes angina complaints with typical effort in normal coronary artery. The exercise ECG test triggered angina pectoris and a synchronous complete left bundle branch block appeared on the ECG. After the exercise, both the left bundle branch block and the angina pectoris disappeared. The angina pectoris and left bundle branch block with combined treatment with ivabradine and trimetazidine did not occur later. The possible pathomechanism of painful left bundle branch block and treatment options are discussed. Orv H

Conduction System Pacing: Hope, Challenges, and the Journey Forward.

PURPOSE OF THE REVIEW: Cardiac pacing has evolved in recent years currently culminating in the specific stimulation of the cardiac conduction system (conduction system pacing, CSP). This review aims to provide a comprehensive overview of the available literature on CSP, focusing on a critical classification of studies comparing CSP with standard treatment in the two fields of pacing for bradycardia and cardiac resynchronization therapy in patients with heart failure. The article will also elaborate specific benefits and limitations associated with CSP modalities of His bundle pacing (HBP) and …

A shorter R wave peak time in left bundle branch pacing may not be a marker of more physiological ventricular activation: A case report.

Left bundle branch pacing (LBBp) is a promising alternative to conventional biventricular pacing cardiac resynchronization therapy. The left anterior fascicle (LAF) is adjacent to the left ventricular outflow tract, while the left posterior fascicle (LPF) dominates a broader area of the left ventricle. Whether LAF or LPF dominates ventricular activation has not been determined. We present the case of a 76-year-old man who underwent LBBp implantation and propose the left ventricular activation domination in LPF pacing, an alternative when LBBp is unavailable.

Chronically altered ventricular activation causes pro-arrhythmic cardiac electrical remodelling in the chronic AV block dog model.

AIMS: Altered ventricular activation (AVA) causes intraventricular mechanical dyssynchrony (MD) and impedes contraction, promoting pro-arrhythmic electrical remodelling in the chronic atrioventricular block (CAVB) dog. We aimed to study arrhythmogenic and electromechanical outcomes of different degrees of AVA. METHODS AND RESULTS: Following atrioventricular block, AVA was established through idioventricular rhythm (IVR; n = 29), right ventricular apex (RVA; n = 12) pacing or biventricular pacing [cardiac resynchronization therapy (CRT); n = 10]. After ≥3 weeks of bradycardic remodelling, Tors…

Pacing of Specialized Conduction System.

Right ventricular pacing for bradycardia remains the mainstay of pacing therapy. Chronic right ventricular pacing may lead to pacing-induced cardiomyopathy. We focus on the anatomy of the conduction system and the clinical feasibility of pacing the His bundle and/or left bundle conduction system. We review the hemodynamics of conduction system pacing, the techniques to capture the conduction system and the electrocardiogram and pacing definitions of conduction system capture. Clinical studies of conduction system pacing in the setting of atrioventricular block and after AV junction ablation ar

Electrocardiographic parameters associated with pacemaker induced cardiomyopathy.

BACKGROUND: Chronic right ventricular (RV) pacing can induce left ventricular (LV) dyssynchrony and cause pacemaker induced cardiomyopathy (PiCM). Identifying which patients are at risk for PiCM is limited. METHODS: Patients receiving RV-only permanent pacemakers (PPMs) at Duke University Medical Center between 2011 and 2017 who had normal baseline ejection fractions (EFs) were identified. Patients who developed a subsequent decrease in EF, died, or underwent cardiac resynchronization therapy, left ventricular assist device, or heart transplant without a competing cause were considered as the…

Current Challenges in Cardiac Device Management: Guidance for Practicing Clinicians: A Canadian Journal of Cardiology White Paper.

Cardiac implantable electronic devices (CIED) are medical devices that play a significant role in the management of heart disease by regulating cardiac rate and rhythm. CIEDs encompass pacemakers, implantable cardioverter defibrillators, cardiac resynchronisation devices (CRT), and implantable cardiac monitors. While robust evidence supports the use of these devices, there are several areas where technical developments have outpaced robust clinical trials, including: the role of primary prevention implantable cardioverter-defibrillators in the era of contemporary heart failure (HF) therapy, ri

PURPOSE OF THE REVIEW: This review focuses on broader perspectives of mitral regurgitation (MR) in patients with heart failure. RECENT FIND … new visual markers of LV mechanical dyssynchrony, appear to be predictive of MR reduction following cardiac resynchronization therapy. Optimal guideline-directed medical therapy has been shown to decrease the severity of secondary MR effectively. A theoretical framework to characterize secondary MR as it relates to the onset of MR is proposed. Type A: Early onset of MR contemporan …

Electrical therapies in heart failure: Evolving technologies and indications.

Device therapy for heart failure has rapidly evolved over 2 decades. The knowledge of indications, assessment lead and device technology has expanded to include CRT, leadless pacing and conduction system pacing such as His bundle and left bundle branch area pacing. But there is still a lack of evidence for these new technologies as well as for common indications such as atrial fibrillation and upgrading from a previous device. The role of personalized medicine will become increasingly important when selecting candidates for CRT, primary preventive ICD ablation procedures and emerging new devic

Natural history and clinical significance of isolated complete left bundle branch block without associated structural heart disease.

OBJECTIVE: Left bundle branch block (LBBB), which is associated with underlying cardiac disease, is believed to play a role in the pathogenesis of cardiomyopathy through delays in interventricular conduction, leading to dyssynchrony. However, this has not been established in previous studies. It is unclear whether LBBB indicates clinically advanced cardiac disease or is an independent factor responsible for increased mortality and the development of heart failure. We investigated the natural history of isolated LBBB without any associated structural heart disease in order to determine its clin

Left Bundle Branch Block: Characterization, Definitions, and Recent Insights into Conduction System Physiology.

Left bundle branch block (LBBB) is not just a simple electrocardiogram alteration. The intricacies of this general terminology go beyond simple conduction block. This review puts together current knowledge on the historical concept of LBBB, clinical significance, and recent insights into the pathophysiology of human LBBB. LBBB is an entity that affects patient diagnosis (primary conduction disease, secondary to underlying pathology or iatrogenic), treatment (cardiac resynchronization therapy or conduction system pacing for heart failure), and prognosis. Recruiting the left bundle branch with c…
