Using guideline-directed medical therapies to improve kidney and cardiovascular outcomes in patients with chronic kidney disease.

PURPOSE: An estimated 37 million people currently live with chronic kidney disease in the US, which places them at increased risk for kidney disease progression, cardiovascular disease, and mortality. This review discusses current standard-of-care management of patients with chronic kidney disease, identifies key gaps in care, and briefly highlights how pharmacists can address gaps in care as members of the multidisciplinary care team. SUMMARY: Recent advances in guideline-directed medical therapies for patients with chronic kidney disease, including agents from the sodium-glucose cotransport

From Roots to Canopy: Understanding Chronic Pelvic Pain Through a Tree-Inspired Model.

INTRODUCTION AND HYPOTHESIS: Chronic pelvic pain affects approximately 25% of women. Despite its prevalence, health care providers often find it challenging and may feel underprepared to offer effective care. METHODS: Using the metaphor of a tree provides a simplified, systematic approach to better understanding and thus managing this condition. The tree model unites both the biopsychosocial and neuroinflammatory models of chronic pain. In this metaphor, the tree's roots represent nociceptive inputs, the trunk represents nociplastic changes, and the canopy signifies psychosocial factors. The

Flourishing in chronic disease care: a narrative review and proposed conceptual framework.

The growing burden of chronic diseases has increased interest in both psychological distress and positive psychological functioning among people living with chronic conditions. In this context, flourishing may be understood as the capacity to maintain positive functioning, meaning, social connectedness, and personal growth while adapting to long-term illness. This narrative review examines the conceptualization, theoretical foundations, current evidence, measurement instruments, and factors associated with flourishing in chronic disease populations. Insights from Keyes' Mental Health Continuum

Interdisciplinary Teams in Endometriosis Care.

Endometriosis-associated chronic pelvic pain can at times be a complex problem that is resistant to standard medical and surgical therapies. Multiple comorbidities and central sensitization may be at play and must be recognized with the help of a thorough history and physical examination. If a complex pain problem is identified, most endometriosis expert reviews and guidelines recommend multidisciplinary care. However, there are no specific recommendations about what should be the components of this approach and how that type of team care should be delivered. There is evidence showing the effe

Mental health hospitalisations of care-experienced and general-population children with chronic conditions: evidence from the Children's Health in Care in Scotland (CHiCS) study, a population-wide longitudinal study using administrative data.

BACKGROUND: Care experience and chronic physical conditions in children are associated with an increased risk of mental health (MH) problems. These issues can compound, leading to poorer prognosis for both MH and chronic conditions. Examining the relationship between care experience and mental ill-health, and exploring how chronic conditions may influence this, is crucial for informing policy and mitigating the burdens on children, families and healthcare systems. METHODS: Routine health and social care data were linked for 13,830 care-experienced children (CEC) and 649,771 general-population

Addressing the Poverty Barrier in Collaborative Care for Adults Experiencing Homelessness: A Case-Based Report.

The Collaborative Care (CC) model of integrated care is an evidence-based, systematic approach in which primary care and behavioral health teams work together to deliver effective treatment for depression and other common mental illnesses in primary care settings. Because people experiencing homelessness have high rates of chronic medical conditions, mental illness and substance use disorders, interventions that integrate the physical, mental and social determinants of health have been shown to be effective to provide healthcare for this population in primary care outpatient settings. In this

Establishing a Net-Zero Emissions Kidney Care Center: A Model Proposal for Taiwan.

Green nephrology has emerged as a crucial strategy to address the health care sector's role in the climate crisis, particularly due to the high carbon intensity of dialysis-related services. Aligned with global net-zero commitments, sustainable kidney care can reduce environmental impact while maintaining high standards of patient care. This viewpoint paper proposes a net-zero carbon emissions kidney care center model to address global climate change challenges and advance health care sustainability goals. Based on the United Nations Sustainable Development Goals, we developed a 4D framework:

A scoping review on community health care centers: Evidence in support of Italian policymakers to redesign the Italian primary care system.

BACKGROUND: The COVID-19 pandemic underscored the central role of primary health care in managing patients with complex and chronic health conditions. In response, the Italian government launched a reform of the National Health Service, introducing Community Health Care Centers ("Case della Comunità") as a key pillar of the new primary care model. Evidence is needed on which organizational features of such centers best support integrated, high-quality care. OBJECTIVE: This scoping review synthesized international evidence on community health care centers characterized by integrated care and c

Current perspectives on congenital obstructive nephropathy.

Congenital obstructive nephropathy is the leading cause of chronic renal disease in children. As a result, it represents a tremendous societal burden in terms of morbidity and mortality, as well as in health care expenses of caring for children with chronic kidney disease and end-stage renal disease. The various diagnostic, prognostic, and therapeutic challenges associated with congenital obstructive nephropathy highlight the importance of developing effective experimental models for studying this disease process. In this review, we define the clinical entity that is congenital obstructive nep

Healthcare professionals' challenges and solutions in providing palliative care to patients with severe chronic obstructive pulmonary disease.

Palliative care significantly improves the quality of life for individuals with advanced chronic diseases. However, access in Malaysia remains limited, particularly for patients with non-malignant conditions such as chronic obstructive pulmonary disease (COPD). Here, we discuss the barriers and solutions identified through a nine hour in-person stakeholder workshop to co-develop practical initiatives for integrating palliative care into the management of severe COPD. Key challenges in the assessment of severe COPD included limited training in holistic care, resource constraints, and fragmented

Traumatic brain injury as a chronic disease: insights from the United States Traumatic Brain Injury Model Systems Research Program.

Traumatic brain injury (TBI) is a global health priority, associated with substantial burden. Historically conceptualised as an injury event with finite recovery, TBI is now recognised as a chronic condition that can affect multiple domains of health and function, some of which might deteriorate over time. Many people who have had a TBI remain moderately to severely disabled at 5 years, are rehospitalised up to 10 years post-injury, and have a reduced lifespan relative to the general population. Understanding TBI as a chronic disease process can be highly informative for optimising care, which

Proactive and Integrated Management and Empowerment in Parkinson's Disease: Designing a New Model of Care.

Parkinson's disease is the second most common neurodegenerative condition after Alzheimer's disease. The number of patients will rise dramatically due to ageing of the population and possibly also due to environmental issues. It is widely recognised that the current models of care for people with Parkinson's disease or a form of atypical parkinsonism lack continuity, are reactive to problems rather than proactive, and do not adequately support individuals to self-manage. Integrated models of care have been developed for other chronic conditions, with a range of positive effects. A multidiscipl

The value of personal health records for chronic disease management: what do we know?

BACKGROUND AND OBJECTIVES: Electronic personal health records (PHRs) allow patients access to their medical records, self-management tools, and new avenues of communication with their health care providers. They will likely become a valuable component of the primary care Patient-centered Medical Home model. Primary care physicians, who manage the majority of chronic disease, will use PHRs to help patients manage their diabetes and other chronic diseases requiring continuity of care and enhanced information flow between patient and physician. In this brief report, we explore the evidence for th

The Harris Center: Expanding Integrated Care with Partnerships Beyond Traditional Clinic Walls.

Integrated behavioral health care has advanced from simple service colocation to a coordinated, system-wide approach that connects behavioral health, primary care, emergency medicine, social services, and community support. Health systems increasingly rely on partnerships to address the complex medical and social needs driving population health outcomes. The Harris Center for Mental Health and IDD (The Harris Center) illustrates this evolution, operating as a comprehensive population health partner that serves nearly 90,000 individuals annually across Harris County. Through deep integration wi

The healthy learner model for student chronic condition management--part I.

A significant number of children have chronic health conditions that interfere with normal activities, including school attendance and active participation in the learning process. Management of students' chronic conditions is complex and requires an integrated system. Models to improve chronic disease management have been developed for the medical system and public health. Programs that address specific chronic disease management or coordinate school health services have been implemented in schools. Lacking is a comprehensive, integrated model that links schools, students, parents, health car
