The influence of defaecating proctograms on clinical decision-making in pelvic floor disorders.

AIM: Defaecating proctogram (DP) studies have become an integral part of the evaluation of patients with pelvic floor disorders. However, their impact on treatment decision-making remains unclear. The aim of this study was to assess the concordance of decision-making by colorectal surgeons and the role of the DP in this process. METHOD: Four colorectal surgeons were presented with online surveys containing the complete history, examination and investigations of 106 de-identified pelvic floor patients who had received one of three treatment options: physiotherapy only, anterior Delorme's proce

The value of preliminary clinical evaluation for decision making in injuries of the hand and wrist.

INTRODUCTION: Recent research suggests that up to 20% of minor trauma patients admitted to the emergency department (ED) will suffer from non-specific chronic conditions over the subsequent several months. Thus, the present study assessed the correlates of symptoms that persisted at 4 months after an ED visit and, in particular, evaluated the associations between these symptoms and self-reported stress levels at ED admission and discharge. METHOD: This study was a prospective observational investigation conducted in the ED of Bordeaux University Hospital that included patients admitted for mi

A clinical score to guide in decision making for monogenic type I IFNopathies.

OBJECTIVE: To develop a set of clinical criteria that identifies patients with a potential autoinflammatory IFNopathy. METHODS: Based on a literature review, a set of clinical criteria identifying genetically confirmed monogenic IFNopathies was selected. For validation, the clinical score was assessed in healthy controls (HCs) and 18 disease controls, including 2 known autoimmune IFNopathies, juvenile systemic lupus erythematosus (JSLE, n = 4) and dermatomyositis (JDM, n = 4); adenosine deaminase 2 deficiency (DADA2, n = 4); and oligoarticular juvenile idiopathic arthritis (oJIA, n = 6). [Clinical reasoning and decision-making in practice. A man with hip pain and fever].

A 42-year-old man was admitted to the hospital because of pain in the left hip. On examination he was febrile at 38 degrees C and he walked with a limp. The chest, abdomen and extremities were normal. Laboratory tests showed an elevated ESR and CRP. The ANA test was positive. CT-scan of the abdomen revealed a mass in the psoas region and some dilatation of the left renal pelvis. Following the histological results of the first and second diagnostic percutaneous biopsies, the clinicians suspected idiopathic retroperitoneal fibrosis. They treated the patient with corticosteroids for a period of 4

[Clinical reasoning and decision-making in practice. A patient with oliguria following prostatectomy].

A 70-year-old man with clinically localised prostate carcinoma underwent extraperitoneal endoscopic radical prostatectomy. His medical history revealed hypertension, renal colic, hypogonadotropic hypogonadism and recurrent deep venous thrombosis in the legs. The operation was uneventful with 500 ml blood loss and no periods ofhypotension. The patient developed oliguria within 12 h after surgery. A hypovolemic state was initially suggested to explain the oliguria and increasing amounts of intravenous fluids were administered. The oliguria persisted, however, and the patient did not respond to a

[Clinical thinking and decision making in practice. A young women with muscle cramps].

A 23-year-old woman with a history of episodic stiffening of the limbs since her early adolescence, reported attacks of muscle contraction accompanied by feelings of panic but without loss of consciousness. Epileptic seizures, e.g. progressive myoclonic epilepsy, metabolic encephalopathy, dystonia and tetany were suggested. During the examination, muscle contraction could be provoked whilst measuring the blood pressure. Tetany based on primary hypoparathyroidism was diagnosed from the medical history as well as the neurological examination. This was confirmed by laboratory tests. She was succe

[Clinical thinking and decision making in practice. A patient with flank pain and anuria].

A 49-year-old man was admitted because of acute anuria. He had bilateral loin pain and was said to have been suffering from renal calculi seven years earlier. He used diclofenac as a pain killer. The renal pyela were mildly dilated. Eventually it became clear that the patient suffered from retroperitoneal fibrosis. This case illustrates (a) ureteral patency does not ensure actual urine flow; (b) non-steroidal anti-inflammatory drugs (NSAIDs) are a frequent cause of renal function loss, but only if there is an additional renal disease causing increased renal prostaglandin synthesis; (c) pattern

The influence of computed tomography on clinical decision making in the evaluation of a nonvisualizing kidney.

Nonvisualization of a kidney in an elderly man was investigated by computed tomography (CT). Simple low lying ureteral obstruction was excluded, multiple pathologic abnormalities were identified and causes for the nonvisualization are postulated. This case report demonstrates the important effect CT scanning produced on the clinical management of the patient in question.

Perioperative strategies for resectable EGFR-Mutant NSCLC: evidence hierarchy and clinical decision-making.

The management of resectable non-small cell lung cancer (NSCLC) with EGFR-sensitizing mutations has changed substantially over the past decade, with adjuvant osimertinib now representing the most mature perioperative strategy because it has demonstrated durable disease-free survival, overall survival, and central nervous system (CNS) benefit in ADAURA. This review examines resectable EGFR-mutant NSCLC through the lens of evidence maturity rather than novelty, emphasizing the biologic features that distinguish this subgroup from unselected NSCLC, including CNS tropism, limited perioperative imm

Molecular Profiling of Urothelial Bladder Cancer: Bridging Tumor Biology and Clinical Decision-making.

CONTEXT: Bladder cancer ranks among the most common malignancies worldwide, with urothelial carcinoma as the predominant histological type. Its development from normal urothelium to non-muscle-invasive bladder cancer (NMIBC) or muscle-invasive bladder cancer (MIBC) is driven by complex molecular events, including chromosomal aberrations, somatic mutations, and epigenetic dysregulation. Advances in next-generation sequencing (NGS) and transcriptomic profiling have transformed the understanding of urothelial tumor biology and opened new perspectives for precision oncology. OBJECTIVE: To provide

[Staging of middle ear cholesteatoma surgery and its value for clinical decision-making].

Middle ear cholesteatoma is characterized by squamous epithelial accumulation within the middle ear cavity, which can lead to severe complications such as hearing loss, facial nerve paralysis, and intracranial infection. A scientific staging system is essential for guiding surgical planning and improving the comparability of clinical research. This review systematically examines the main classification frameworks and their core principles, evaluating evidence regarding preoperative applicability, interobserver reliability, and impact on clinical decision-making. Although existing systems each …

TSH suppression therapy from an individualized perspective: from mechanism to clinical decision-making.

Thyroid-Stimulating Hormone (TSH) suppression therapy is a key strategy in managing differentiated thyroid cancer (DTC) after surgery, but its clinical use has long faced the challenge of a "one-size-fits-all" approach. Previous studies mainly focus on balancing risks and benefits, yet the process of individualized decision-making still lacks systematic integration and synthesis. This article offers a comprehensive review of this issue from an individualized perspective. Mechanistically, it details the networked regulatory mechanisms by which TSH promotes the proliferation, invasion, and dedif

Dopaminergic Radiopharmaceutical Imaging in Parkinsonian Syndromes: From Molecular Targets to Clinical Decision-Making.

Parkinsonian syndromes comprise overlapping neurodegenerative and non-degenerative disorders, making aetiological diagnosis difficult, while existing procedural guidance does not fully integrate tracer-specific biology with clinical decision-making, multimodal strategies, and emerging quantitative and pathology-directed biomarkers. This review synthesises evidence on the molecular targets, radiopharmaceutical characteristics, biological interpretation, and clinical applications of dopaminergic single-photon emission computed tomography (SPECT) and positron emission tomography (PET), focusing o…

Using the Electronic Health Record and Artificial Intelligence to Guide Clinical Decision-Making.

Electronic medical records (EMR) have transformed how clinical information is documented, shared, and utilized over the past 60 years, and the addition of artificial intelligence (AI) has vastly broadened EMR's capabilities. Targeted comprehensive prehabilitation services can be provided to frail patients that mitigate mortality risks. Tools for early detection of sepsis and deterioration are now embedded within the EMR, with predictive models improving their accuracy compared with traditional methodology. The EMR enables protocol-driven care and templated documentation to streamline workflow
