Diaphragm paralysis from cervical disc lesions.

An opera singer, who "made her living with her diaphragm", developed a post-traumatic unilateral radiculopathy due to cervical disc lesions, C3 to C6. During one year of severe neck and left arm pain she gradually lost the ability to sing difficult operatic passages which brought an end to her music career. Following a three level anterior cervical decompression and fusion, the neck and arm pain was immediately relieved. One week later her voice and singing ability returned to its full strength and power permitting her to resume her activities as a vocalist. The diagnosis of paresis of the lef…

Prepyloric antral diaphragm: delays in treatment.

Six cases of prepyloric antral diaphragm were encountered at Children's Hospital Medical Center of Akron between 1965 and 1985, with approximately 150 cases reported in the literature during this time. In our series, a delay from onset of symptoms to treatment existed for a period of 4 months to 6 years. We suggest that prompt endoscopic confirmation and surgical therapy for this lesion, when suspected by clinical and radiologic abnormalities, would decrease the physical and psychologic morbidity of unexplained recurrent vomiting in these children.

Toxic shock syndrome and diaphragm use.

A case of toxic shock syndrome (TSS) associated with diaphragm use is presented. Adolescents who use a diaphragm should be informed of the symptoms of TSS, and those with the syndrome should be questioned about diaphragm use.

Obstruction due to prepyloric mucosal diaphragm.

Antral mucosal diaphragm is uncommon, and presents with manifestations of obstruction to the pyloric outlet. The diagnosis is usually established only by gastrotomy. Excision of the diaphragm or pylorophasty affords permanent cure and unnecessary partial gastrectomy should be avoided.

Porous Diaphragm Syndrome: A Literature Review.

Porous diaphragm syndrome (PDS) is a rare and underrecognized condition characterized by the transdiaphragmatic migration of peritoneal contents into the pleural cavity through defects in the diaphragm, most often affecting the right hemidiaphragm. It is commonly associated with hepatic hydrothorax, peritoneal dialysis-related hydrothorax, and peritoneal carcinomatosis. Despite its anatomical simplicity and the availability of effective surgical treatment, PDS is frequently misdiagnosed, leading to repeated pleural drainage, inappropriate therapies, and treatment delays. This review synthesize…

Compensatory effects following unilateral diaphragm paralysis.

Injury to nerves innervating respiratory muscles such as the diaphragm muscle results in significant respiratory compromise. Electromyography (EMG) and transdiaphragmatic pressure (Pdi) measurements reflect diaphragm activation and force generation. Immediately after unilateral diaphragm denervation (DNV), ventilatory behaviors can be accomplished without impairment, but Pdi generated during higher force non-ventilatory behaviors is significantly decreased. We hypothesized that 1) the initial reduction in Pdi during higher force behaviors after DNV is ameliorated after 14 days, and 2) changes …

Small bowel diaphragm disease mimicking malignancy.

Non-steroidal anti-inflammatory drugs (NSAIDs) can produce diaphragm disease where multiple strictures develop in the small bowel. This typically presents with anaemia and symptoms of small bowel obstruction. The strictures develop as a result of circumferential mucosal ulceration with subsequent contraction of rings of scar tissue. We report a case of a 47-year-old woman with a 6-month history of NSAIDs abuse who presented with subacute small bowel obstruction 1 year after stopping NSAIDs. CT and MRI showed multiple ileal strictures with florid locoregional lymphadenopathy. A malignant diagno…

Diaphragm motor unit recruitment in rats.

We hypothesized that considerable force reserve exists for the diaphragm muscle (DIAm) to generate transdiaphragmatic pressures (Pdi) necessary to sustain ventilation. In rats, we measured Pdi and DIAm EMG activity during different ventilatory (eupnea and hypoxia (10% O(2))-hypercapnia (5% CO(2))) and non-ventilatory (airway occlusion and sneezing induced by intranasal capsaicin) behaviors. Compared to maximum Pdi (Pdi(max) generated by bilateral phrenic nerve stimulation), the Pdi generated during eupnea (21+/-2%) and hypoxia-hypercapnia (28+/-4%) were significantly less (p<0.0001) than that …

Diaphragm development and congenital diaphragmatic hernia.

Advances in the understanding of normal diaphragm embryogenesis have provided the necessary foundation for novel insights into the pathogenesis of congenital diaphragmatic hernia (CDH). Although diaphragm formation is still not completely understood, we have identified key structures and periods of development that are clearly abnormal in animal models of CDH. The pleuroperitoneal fold (PPF) is a transient structure which is the target for the neuromuscular component of the diaphragm. The PPF has been shown to be abnormal in multiple animal models of Bochdalek CDH; specifically, a malformation…

Perioperative use of temporary diaphragm pacing in surgical patients with unilateral diaphragm dysfunction to decrease mechanical ventilation.

BACKGROUND: Unilateral diaphragmatic dysfunction, most commonly from phrenic nerve injury, is associated with difficult ventilator weaning and increased postoperative pulmonary complications. Diaphragm pacing prevents ventilator-induced diaphragm dysfunction and supports ventilation during periods of perioperative vulnerability. We report perioperative utilization of temporary diaphragm pacing in patients with pre-existing or anticipated postoperative unilateral diaphragmatic dysfunction. METHODS: This cohort analysis was derived from a prospective, institutional review board-approved databas…

Single port thoracoscopic diaphragm plication: A novel treatment approach to bilateral phrenic nerve palsy and diaphragm paralysis.

INTRODUCTION AND IMPORTANCE: The diaphragm is the primary muscle of respiration. Bilateral paralysis of the diaphragm due to phrenic nerve palsy causes severe dyspnoea and is life threatening. Diaphragmatic Plication has shown great promise in treating diaphragm paralysis and has evolved as operative treatment from an open thoracotomy to multiport and robotic video assisted thoracoscopic surgery. CASE PRESENTATION: Here we present a case of idiopathic bilateral diaphragm paralysis resulting significant deterioration in lung function tests, supplemental oxygen, and ventilator dependence. The p…

Ultrasonographic assessment of the diaphragm in systemic lupus erythematosus: cut-off values and predictive indicators of diaphragm thickness.

BACKGROUND: Respiratory complications significantly result in morbidity and mortality in patients with Systemic Lupus Erythematosus (SLE). However, the impact of SLE on respiratory muscle function, particularly diaphragm thickness and strength, has been the subject of limited investigation. AIMS: The study aims to investigate the key physiological and respiratory parameters affecting diaphragm thickness in SLE patients and to assess its role as a predictive indicator of respiratory dysfunction. METHODS: The cross-sectional study included 29 patients with SLE and 48 healthy individuals. Diaph…

The effect of the diaphragm stretching technique in the diaphragm contraction rate and trunk muscle activity in back pain patients.

BACKGROUND: The diaphragm plays an important role in trunk stability. Therefore, diaphragmatic dysfunction is associated with low back pain. OBSECTIVE: This study aimed to confirm the effectiveness of diaphragm stretching technique as a treatment method for low back pain by evaluating the diaphragm contraction rate, trunk muscle activity in patients with low back pain. METHODS: Thirty-four patients with low back pain were randomly divided into two group: an experimental group and a control group. The diaphragm stretching technique was conducted in the experimental group and the placebo inter…
