High intervention, low impact? A retrospective cohort study of management and outcomes of women presenting with decreased fetal movement.

PROBLEM: Women are encouraged to monitor fetal movements to reduce stillbirth, however the relationships between decreased fetal movements, birth interventions and perinatal outcomes are unclear. BACKGROUND: Decreased fetal movement is considered a risk factor for stillbirth, and clinical guidelines recommend assessment and management of affected women. AIM: To describe the characteristics, management and clinical outcomes of women presenting with decreased fetal movements at one Australian tertiary referral hospital. METHODS: Data on women reporting decreased fetal movements in the third t…

Movement Behavior during Pregnancy and Adverse Maternal-Fetal Outcomes in Women with Gestational Diabetes: A Pilot Case-Control Study.

Gestational diabetes mellitus (GDM) is a major complication in pregnancy. GDM is associated with a higher risk for adverse maternal-fetal outcomes. Associations between movement behavior, including physical activity (PA) and sedentary behavior (SB), and maternal-fetal outcomes are still unclear. The objective of this study was to investigate associations between movement behavior and adverse maternal-fetal outcomes in women with GDM. A total of 68 women with GDM (20-35 weeks, 32.1 ± 5.8 years) were included in this pilot case-control study. The cases were defined by the presence of an adverse

Fetal breathing movements as predictor of favourable pregnancy outcome after oligohydramnios due to membrane rupture in second trimester.

In 11 pregnancies complicated by oligohydramnios due to spontaneous rupture of the membranes in the second trimester of pregnancy fetal breathing movements were assessed regularly by ultrasonographic examinations. In the 6 cases in which fetal breathing movements were detected the babies were liveborn and there was no evidence of pulmonary hypoplasia or the other non-renal features of Potter's syndrome. In the other 5 cases there were no fetal breathing movements. 1 pregnancy was terminated electively, and 1 ended in an intrauterine death; the remaining 3 infants died in the neonatal period. A

Women and health professionals' views and experiences of reduced fetal movements in low- and middle-income countries: A scoping review.

BACKGROUND: Raising awareness of reduced fetal movement (RFM) is a potential approach to prevent stillbirth. To date, research regarding fetal movements has largely been conducted in high-income countries. However, conclusions drawn from this setting may not be applicable to other settings with a higher burden of stillbirth due to differences in sociodemographic factors and healthcare systems. AIM: To explore women's and health professionals' experiences regarding fetal movements, and interventions on encouraging the awareness of RFM in low- and middle-income countries. METHODS: A comprehens…

The association between increased subjective sensation of fetal movements and pregnancy outcome-a prospective cohort and a retrospective comparative analysis.

OBJECTIVE: To determine pregnancy outcomes in women with subjective sensation of increased fetal movements (IFM). METHODS: A prospective cohort study of women after 20 weeks of gestation who were referred with subjective sensation of IFM (April 2018-April 2019) for assessment. Pregnancy outcome was compared to pregnancies with a normal sensation of fetal movements all through pregnancy who underwent obstetrical assessment at term (37-41 weeks of gestation) matched by maternal age and pre-pregnancy BMI in a 1:2 ratio. RESULTS: Overall, out of 28,028 women referred to the maternity ward during…

Excessively delayed maternal reaction after their perception of decreased fetal movements in stillbirths: Population-based study in Japan.

BACKGROUND: Fetal movement is the most common method to evaluate fetal well-being. Furthermore, maternal perception of decreased fetal movements is associated with perinatal demise. Previously, we showed that perception of decreased fetal movements was the most common reason for mothers visiting the outpatient department among those who had stillbirths in our region. Further investigation of stillbirths with decreased fetal movements is essential to find a possible way of preventing stillbirth. AIM: To investigate maternal reaction time after their perceiving decreased fetal movements among s…

Fetal breathing movements are not a good indicator of lung development after premature rupture of membranes and oligohydramnios--a preliminary study.

The effect of severe oligohydramnios (due to prolonged premature rupture of the membranes (PROM)) on breathing movements and lung development was studied longitudinally in 11 human fetuses. Prenatally, fetal breathing movements (FBM) were scored off-line from weekly, 1 h during ultrasound recordings (n = 47). In each recording, the incidence of FBM was scored according to 4 different methods. Postnatally, the cases were retrospectively assigned to a group with normal (n = 4), partially hypoplastic (n = 3), or hypoplastic (n = 4) lungs. Compared to control fetuses, the percentage of time spent

Comparison of chronic morphine and placebo infusion in late gestation fetal lambs: effect upon survival and breathing movements.

A placebo-controlled, randomized study of the effect of a 10-day infusion of morphine (MOR) upon fetal survival and fetal breathing movements was done in late gestation lambs. MOR infusion at a rate of 3 mg.hr-1 did not affect fetal survival or the response of fetal breathing movements to hypercapnia. Chronic exposure to MOR increased the height of the integrated diaphragmatic electromyogram signal (IDIA), respiratory drive and inspiratory effort during periods of eucapnia. Respiratory drive was determined by the product of IDIA height and breathing frequency, and inspiratory effort was the qu

Awareness of fetal movements and care package to reduce fetal mortality (AFFIRM): a trial-based and model-based cost-effectiveness analysis from a stepped wedge, cluster-randomised trial.

BACKGROUND: The AFFIRM intervention aimed to reduce stillbirth and neonatal deaths by increasing awareness of reduced fetal movements (RFM) and implementing a care pathway when women present with RFM. Although there is uncertainty regarding the clinical effectiveness of the intervention, the aim of this analysis was to evaluate the cost-effectiveness. METHODS: A stepped-wedge, cluster-randomised trial was conducted in thirty-three hospitals in the United Kingdom (UK) and Ireland. All women giving birth at the study sites during the analysis period were included in the study. The costs associa…

Effect of encouraging awareness of reduced fetal movement and subsequent clinical management on pregnancy outcome: a systematic review and meta-analysis.

OBJECTIVE: Reduced fetal movement, defined as a decrease in the frequency or strength of fetal movements as perceived by the mother, is a common reason for presentation to maternity care. Observational studies have demonstrated an association between reduced fetal movement and stillbirth and fetal growth restriction related to placental insufficiency. However, individual intervention studies have described varying results. This systematic review and meta-analysis aimed to determine whether interventions aimed at encouraging awareness of reduced fetal movement and/or improving its subsequent cl…

Immediate Neural Plasticity Involving Reaction Time in a Saccadic Eye Movement Task is Intact in Children With Fetal Alcohol Spectrum Disorder.

BACKGROUND: Saccades are rapid eye movements that bring an image of interest onto the retina. Previous research has found that in healthy individuals performing eye movement tasks, the location of a previous visual target can influence performance of the saccade on the next trial. This rapid behavioral adaptation represents a form of immediate neural plasticity within the saccadic circuitry. Our studies have shown that children with fetal alcohol spectrum disorder (FASD) are impaired on multiple saccade measures. We therefore investigated these previous trial effects in typically developing ch

Risk Factors, Associations, and Outcomes of Reduced Fetal Movements: A Preliminary Cross-Sectional Study at Port Sudan Maternity Hospital.

BACKGROUND: Reduced fetal movements (RFM) are a significant concern in obstetric care. They often indicate fetal distress and are associated with adverse perinatal outcomes such as stillbirth and intrauterine growth restriction (IUGR). While RFM is recognized as a critical marker of fetal well-being, there is a limit to the data available on its risk factors and outcomes in the Port Sudan region. This study aimed to estimate risk factors and outcomes of pregnancies at risk due to RFM. METHODS: This was a cross-sectional hospital-based study, conducted from February to August 2022 at Port Suda…

Development of a core outcome set (COS) for studies relating to awareness and clinical management of reduced fetal movement: study protocol.

BACKGROUND: Concerns regarding reduced fetal movements (RFM) are reported in 5-15% of pregnancies, and RFM are associated with adverse pregnancy outcomes including fetal growth restriction and stillbirth. Studies have aimed to improve pregnancy outcomes by evaluating interventions to raise awareness of RFM in pregnancy, such as kick counting, evaluating interventions for the clinical management of RFM, or both. However, there is not currently a core outcome set (COS) for studies of RFM. This study aims to create a COS for use in research studies that aim to raise awareness of RFM and/or evalua…

Fetal muscle-type nicotinic acetylcholine receptor activation in TE-671 cells and inhibition of fetal movement in a day 40 pregnant goat model by optical isomers of the piperidine alkaloid coniine.

Coniine is an optically active toxic piperidine alkaloid and nicotinic acetylcholine receptor (nAChR) agonist found in poison hemlock (Conium maculatum L.). Coniine teratogenicity is hypothesized to be attributable to the binding, activation, and prolonged desensitization of fetal muscle-type nAChR, which results in the complete inhibition of fetal movement. However, pharmacological evidence of coniine actions at fetal muscle-type nAChR is lacking. The present study compared (-)-coniine, (+)-coniine, and nicotine for the ability to inhibit fetal movement in a day 40 pregnant goat model and in …

Identifying placental dysfunction in women with reduced fetal movements can be used to predict patients at increased risk of pregnancy complications.

Maternal perception of fetal movements has historically been used to indicate fetal wellbeing, and has been used with varying success in recent years to identify those pregnancies at increased risk of stillbirth, and other placental pathologies. We present a hypothesis that links reduced fetal movements (RFM) to fetal growth restriction (FGR) and stillbirth through placental dysfunction, and suggests the possibility that this can allow development of a reliable method to identify those women experiencing RFM who are at increased risk of adverse outcome. Reduced fetal movement is thought to rep…
