Outcome of noncardiac and nonvascular surgery in patients with mechanical heart valves.

There is a tendency to avoid noncardiac surgery in patients with mechanical heart valves (MHVs) owing to the increased risk of perioperative thromboembolism, infective endocarditis, and bleeding. We aimed to determine the risk of cardiac and noncardiac complications in patients with MHVs who underwent noncardiothoracic, nonvascular surgery. A total of 140 patients with MHVs (77 aortic, 46 mitral, and 17 double valve) and 1,200 patients with native valves (control group) were prospectively followed up for a minimum of 3 months after noncardiothoracic and nonvascular surgery. Patients with biopr…

Anticoagulation for mechanical heart valves in patients with and without atrial fibrillation.

Surgical replacement of a native valve with a biological or mechanical prosthesis is the definitive treatment for many forms of advanced valvular heart disease. Mechanical heart valves are less prone to structural deterioration compared with bioprostheses, but require chronic oral anticoagulation to prevent thromboembolic events. Thromboembolic risk varies based on patient-related risk factors, including atrial fibrillation, advanced age, low ejection fraction, and hypercoagulability. Other important correlates of high thromboembolic risk include valve design, valve position, anticoagulation v…

Acute coronary syndromes in patients with prosthetic heart valves-a case-series.

BACKGROUND: There are few reports regarding acute coronary syndromes (ACS) in patients with prosthetic heart valves (PHV), mostly attributing the ACS to a PHV-derived coronary embolus. OBJECTIVE: To characterize a case-series of ACS patients with PHV. METHODS: All patients in our institution with previous PHV surgery and ACS during 1996-2005 were retrospectively analysed. RESULTS: We identified 40 patients from the 15,878 patient database, whose mean age was 72.5 +/- 12.5 years and of whom 21 were male. The majority (n=28) had mechanical valves; 24 patients (60%) had an aortic prosthetic va…

Shared gene expression profiles in developing heart valves and osteoblast progenitor cells.

The atrioventricular (AV) valves of the heart develop from undifferentiated mesenchymal endocardial cushions, which later mature into stratified valves with diversified extracellular matrix (ECM). Because the mature valves express genes associated with osteogenesis and exhibit disease-associated calcification, we hypothesized the existence of shared regulatory pathways active in developing AV valves and in bone progenitor cells. To define gene regulatory programs of valvulogenesis relative to osteoblast progenitors, we undertook Affymetrix gene expression profiling analysis of murine embryonic

Age-related changes in collagen synthesis and turnover in porcine heart valves.

BACKGROUND AND AIM OF THE STUDY: Although the six-month-old pig is commonly used as a model to study human heart valve biology and various age-specific valve diseases, the correlation of porcine valve biology and development with that of humans has not been thoroughly assessed. Given the important role of the matrix in valve function, the aim of this study was to characterize porcine valve matrix structure and collagen turnover during development and aging. METHODS: Porcine aortic valves (AV) and mitral valves (MV) were examined throughout fetal development and postnatally at six weeks, six m

Tissue engineering of heart valves in vivo using bone marrow-derived cells.

In this study, we tissue-engineered heart valves in vivo using autologous bone marrow-derived cells (BMCs). Canine BMCs were differentiated into endothelial cell (EC)-like cells and myofibroblast (MF)-like cells. Decellularized porcine pulmonary valves were seeded with BMCs and implanted to abdominal aorta and pulmonary valve of bone marrow donor dogs. Histological examination of the explants identified the regeneration of valvular structures expressing CD31 and smooth muscle alpha-actin, indicating the presence of EC-like and MF-like cells in the grafts at 3 and 1 week, respectively, after im…

Clinical application of tissue engineered human heart valves using autologous progenitor cells.

BACKGROUND: Tissue engineering (TE) of heart valves reseeded with autologous cells has been successfully performed in vitro. Here, we report our first clinical implantation of pulmonary heart valves (PV) engineered with autologous endothelial progenitor cells (EPCs) and the results of 3.5 years of follow-up. METHODS AND RESULTS: Human PV allografts were decellularized (Trypsin/EDTA) and resulting scaffolds reseeded with peripheral mononuclear cells isolated from human blood. Positive stain for von Willebrand factor, CD31, and Flk-1 was observed in monolayers of cells cultivated and differenti…

Early investigation of silver-coated Silzone heart valves prosthesis in 126 patients.

BACKGROUND AND AIM OF THE STUDY: Permanent silver (Silzone) coating of the sewing cuff of St. Jude Medical prosthetic heart valves may reduce the rate of prosthetic valve endocarditis (PVE). However, the incidence of paravalvular leaks and stroke in patients after implantation of Silzone-coated heart valve prostheses is largely unknown. METHODS: Complications were analyzed among 126 consecutive patients (78 males, 48 females; mean age 64.7 years; range: 40-80 years) who received Silzone-coated prostheses at our institution between February 1998 and December 1999. Among patients, 94 had aortic…

Further evidence of gaseous embolic material in patients with artificial heart valves.

OBJECTIVE: We undertook this study to evaluate the hypothesis that most microemboli signals in patients with artificial heart valves are gaseous, assuming that microemboli counts in cerebral arteries would progressively decline with increasing distance from the generating heart valve. METHODS: A total of 10 outpatients with CarboMedics (Sulzer Carbomedics Inc., n = 5) and ATS prosthetic heart valves (n = 5) in the aortic (n = 8), mitral (n = 1), or both aortic and mitral positions (n = 1) were recruited. Monitoring was performed simultaneously over the middle and anterior cerebral arteries an…

Can cerebrovascular microemboli induce cognitive impairment in patients with prosthetic heart valves?

Doppler ultrasonography can be used to detect the presence of emboli in the cerebral arterial circulation. Emboli can be produced by different sources and can be of different nature: solid elements as thrombi, platelet aggregates or atheromatous material, or gaseous when they are produced during the decompression phase of diving or during extracorporeal circulation (ECC) procedures. A more recent source of emboli has been found in the mechanical prostheic heart valves (MHV). The emboli generated by MHV are likely of gaseous nature and are found in the middle cerebral artery blood flow at a var…

[Value of transesophageal echocardiography in diagnosis of diseases of native heart valves].

In the clinical management of patients with valvular heart disease, transthoracic echocardiography (TTE) combined with Doppler has become the central diagnostic tool during the past decades. The development of transesophageal echocardiography (TEE) has led to an improved image quality especially of structures distant to the chest wall. However, since TEE is a semi-invasive technique, its use has to be considered carefully. In aortic valve disease, TEE facilitates a detailed study of valve morphology and allows sufficiently reliable planimetry of aortic valve area, at least when the multiplane

Pregnancy in patients with prosthetic heart valves: an experience with 45 pregnancies.

Between 1974 and 1981, 27 patients became pregnant after prosthetic heart valve replacement with a total of 45 pregnancies. Four pregnancies terminated in therapeutic abortions. Of the remaining 41 pregnancies, 30 were associated with the use of anticoagulants and 11 not. Haemorrhage related to anticoagulant therapy occurred in one patient. The foetal loss in the group with anticoagulant therapy was 33 per cent (10 abortions) and that in the group without was 9 per cent (one abortion). The use of aspirin and dipyridamole during pregnancy (eight pregnancies) did not appear to threaten the outco

Urinary ferritin protein in subjects with prosthetic heart valves and other disorders.

Twenty-four-hour urine ferritin protein excretion (UFPE) was elevated in the great majority of patients with prosthetic cardiac valves. In this group, there was a positive correlation with the presence of haemosiderin in the urine and with another indirect measure of reduced red-cell survival, namely, serum lactic dehydrogenase. In a number of hospital patients with various disorders, UFPE was also increased. In this latter group, however, there was no constant accompanying haemosiderinuria or elevation of lactic dehydrogenase. The measurement of UFPE can be added to the list of screening test

Aspirin and prophylaxis of thromboembolic complications in patients with substitute heart valves.

Anticoagulation therapy with acenocoumarin or with anticoagulants plus aspirin was given to 65 and 57 patients, respectively, with cardiac valve replacement. The follow-up was 1,462 months (22.5 months per patient) for the first group and 1,411 months (24.7 months per patient) for the second group. The frequency of embolic accidents was significantly lower in the group taking aspirin: Thirteen thromboembolic accidents were detected in patients receiving the anticoagulant and 3 in the group receiving the anticoagulant plus aspirin. These figures represent a 20.3 per cent incidence (one each 9.3

Multimodality imaging in the diagnostic approach to a patient with carcinoid heart disease involving four heart valves.

Carcinoid heart disease is a rare condition that occurs in less than half of patients with carcinoid syndrome. The disease mainly affects right-sided heart valves; however, in 5%-10%, it can also involve left-sided valves. This case illustrates the most complicated form of the disease involving four heart valves.

Heart Valve Critical Pathway and Heart Valve Clinic: Novel Benchmarks for Modern Management of Valvular Heart Disease.

The growing burden of valvular heart disease in Western countries represents a challenge for the daily clinical practice, especially in the light of the ever-increasing number of therapeutic options. The Euro Heart Survey showed that, among elderly subjects with severe, symptomatic valve dysfunction, surgery is denied for 33% of patients with aortic stenosis and for 50% of patients with mitral regurgitation. Current management (from diagnosis to follow-up) is often fragmented in multiple-sometimes unnecessary-steps. Such a "patchy" approach may translate into a suboptimal management, especiall

Serial In Vitro Investigation of the Fracture Properties of Mechanical Heart Valves.

BACKGROUND: Mechanical valve prostheses may develop serious dysfunctions, leading to end-stage heart failure. In some cases, a high-risk redo operation is not an option. The first cases of interventional fracturing of mechanical valves have already been successfully performed, but systematic data on mechanical prostheses' fracture properties are lacking. OBJECTIVES: The aim of this study was to assess the properties of mechanical heart valves crucial for interventional cardiologists: the pressure required to fracture valve leaflets; structural weak points of the prostheses; the behavior, velo…

Partial heart transplant valves demonstrate growth in piglets despite prolonged cold storage.

BACKGROUND: Traditional heart valve replacement options lack growth potential, limiting their long-term effectiveness in pediatric patients. Partial heart transplantation is an emerging approach that offers growth-capable valves by transplanting only the valve-containing segment of the heart. We report on the effects of extended cold storage on the viability and growth potential of partial heart transplantation grafts. METHODS: Pulmonary and aortic conduits were harvested from piglets and human donors and stored at 4°C in cold-storage solution, with some samples supplemented with apoptosis in

Review Article: Contemporary Transcatheter Heart Valves for TAVI in Bicuspid Aortic Anatomy.

Bicuspid aortic valve (BAV) is the most common congenital heart disease, affecting 0.5-2% of the population and often leading to early aortic valve degeneration. While surgical aortic valve replacement (SAVR) remains the gold standard for treating severe bicuspid aortic stenosis (AS), transcatheter aortic valve implantation (TAVI) is emerging as a viable alternative in selected BAV anatomies. Initial experiences with first-generation transcatheter heart valves (THVs) showed the feasibility of this technique, but were associated with lower device success rates and higher complications, such as …
