PURPOSE: To describe a case of intraoperative mortality because of air embolism during resection of a choroidal melanoma by pars plana vitrectomy.
METHODS: Retrospective interventional case report.
RESULTS: A 69-year-old man died unexpectedly at the time of pars plana vitrectomy. The operative technique involved the use of high-pressure air (60 mmHg) in the presence of traumatically exposed choroidal vasculature. Autopsy revealed a large air embolus in the right ventricle, which resulted in sudden cardiovascular collapse.
CONCLUSION: Air embolism is a rare complication of ophthalmic surgery

Perineal hernia with bowel gangrene is uncommon but known complication of laparoscopic extralevator abdominoperineal excision (ELAPE). We present a rare case of closed loop small bowel obstruction with bowel gangrene secondary to an incarcerated perineal hernia that developed 7 years after an ELAPE. Intraoperatively, we found a definitive transition point due to adhesions in pelvis and a closed loop obstruction of the distal small bowel at different site with gangrenous intestine. Caecal bascule is an unusual type of volvulus that presents a challenging diagnosis for clinicians. We present a case of a forty-two year old female who developed a perforated caecal bascule five days post emergency caesarean section. The diagnosis of caecal bascule was made intraoperatively during a hemicolectomy. Greater awareness of this phenomenon and its clinical and radiological findings is important to avert the development of bowel perforation or gangrene.

LDS is a newly described autosomal dominant connective tissue disease with systemic vascular involvement. Complex maxillofacial trauma is a challenging problem for even the most experienced reconstructive surgeon. However, advances in presurgical planning, utilization of intraoperative technology, occlusal splints, three dimensional (3D) printing, and creation of custom plates can aid in simplifying challenging cases, decreasing operative time, and optimizing patient outcomes. In this report, we assessed whether GOR correlation analysis could depict epileptogenic networks intraoperatively. BACKGROUND AND OBJECTIVES: Many risk factors have been identified in minimally invasive cholecystectomies that lead to higher complications and conversion rates. No study that we encountered looked at nonvisualization of the gallbladder (GB) during surgery as a risk factor. We hypothesized that nonvisualization was associated with an increased risk of complications and could be an early intraoperative identifier of a higher risk procedure.
