Minimal-invasive pain procedures are effective in different degenerative pain generators in CLBP: A prospective cohort study.

BACKGROUND: It is estimated that a significant number of spinal surgeries are performed, but many patients do not often benefit. OBJECTIVE: Our aim was to determine how effective minimally invasive pain procedures (MIP) are in chronic low back pain (CLBP) patients with proven degenerative causes (specific low back pain). METHODS: 386 eligible patients with CLBP/sciatica resistant to conservative therapy and scheduled for open surgery were screened, and 167 could be enrolled in this study. Indications for MIP in the remaining 150 individuals were made by one experienced spinal surgeon. Before

Interventional Pain Procedures: A Narrative Review Focusing on Safety and Complications. Part 1 Injections for Spinal Pain.

In recent years, there has been a considerable increase in the number of image-guided interventional procedures performed for the management of acute and chronic pain. Concomitantly, there has also been an increase in the complication rate related to these procedures. The aim of this narrative review is to summarize the primary complications associated with commonly performed image-guided (fluoroscopic- or ultrasound-guided) interventional procedures. We conclude that although complications from interventional pain procedures can be mitigated to a certain degree, they cannot be eliminated alto

[Premedical treatment of children and adolescents prior to painful procedures and general anaesthesia].

Preoperative anxiety and pain in children and adolescents are associated with postoperative complications. Despite relevant non-pharmacological treatment, the addition of premedical treatment may be needed. Traditionally, oral midazolam has been a favourite premedical treatment. However, several alternatives are available. Additionally, simultaneous administration of the medications to achieve synergy may be advantable. The choice of premedical treatment and method of administration depends on the individual child, the situation, and the preferences of the medical staff.

PROSPECT methodology for developing procedure-specific pain management recommendations: an update.

The procedure-specific postoperative pain management (PROSPECT) working group develops evidence-based pain management recommendations. PROSPECT methodology is unique and rigorous. However, several limitations were recognised that needed to be addressed, and several new factors were identified that improved PROSPECT methodology. The aim of this article is to present updated PROSPECT methodology for development of recommendations for procedure-specific pain management, focusing on the methodological revisions we will implement. In future, included randomised clinical trials will need to be prosp

Clinical study on the efficacy of LED phototherapy for pain control in an orthodontic procedure.

Pain is an unpleasant and emotional subjective sensory experience that occurs during orthodontic procedures. Currently, LED phototherapy is an alternative to the use of laser light as analgesic agent due to similarity of response and lower cost. This case-control, quantitative, qualitative, and longitudinal study aimed to investigate the effect of IR LED phototherapy (λ846 ± 20 nm) in pain during the process of tooth separation during orthodontic treatment. After approval by the Institution Ethics Committee, 40 patients (30 female/10 male, 20-30 years old, average age 24.5 ± 2.6 years old) ful

Evaluating comfort measures for commonly performed painful procedures in pediatric patients.

INTRODUCTION: Management of pediatric pain from medical procedures is of great importance for improving both patient care and experience. In this study, we investigated methods of managing acute pain in infants and children by studying the correlation between the number of attempts to complete painful procedures, given different comfort measures. METHODS: The study is a retrospective review of 74,276 procedures performed at two pediatric hospitals in an integrated academic children's health system between 2013 and 2016. We compared three comfort measures most frequently offered: positions of

Reducing risk of spinal haematoma from spinal and epidural pain procedures.

BACKGROUND AND AIMS: Central neuraxial blocks (CNB: epidural, spinal and their combinations) and other spinal pain procedures can cause serious harm to the spinal cord in patients on antihaemostatic drugs or who have other risk-factors for bleeding in the spinal canal. The purpose of this narrative review is to provide a practise advisory on how to reduce risk of spinal cord injury from spinal haematoma (SH) during CNBs and other spinal pain procedures. Scandinavian guidelines from 2010 are part of the background for this practise advisory. METHODS: We searched recent guidelines, PubMed (MEDL

Pain Management Interventions for Needle Stick Procedures: An Ambulatory EBP Project.

Pain is a prevalent global health concern, and pain assessment and treatment is a patient right. This evidence-based practice project targeted translating pain management interventions into practice for adult and pediatric patients undergoing needle stick procedures in ambulatory settings. Evidence-based interventions should consistently be offered to patients who often experience procedural pain or discomfort. Implementation of the practice change included multiple interactive, reinforcing strategies. Pre/post-implementation evaluation measures included clinician knowledge, perceptions, and c…

Interventional spine procedures for management of chronic low back pain-a primer.

Chronic low back pain is a common clinical condition. Percutaneous fluoroscopic-guided interventions are safe and effective procedures for the management of chronic low back pain, which can be performed in an outpatient setting. Interventional radiologists already possess the technical skills necessary to perform these interventions effectively so that they may be incorporated into a busy outpatient practice. This article provides a basic approach to the evaluation of patients with low back pain, as well as a review of techniques used to perform the most common interventions using fluoroscopic

[Cinematography, a new diagnostic procedure in evaluation of the injured painful wrist joint].

By the X-ray Cineradiografie we are able to examine and to judge the dynamic of the wrist bones by 50 pictures/sec. in comparison to one another and also depending on their ligaments. We did an investigation of 170 patients with painful wrist. With the method we were able to make up a clear diagnosis and to propose the therapy. I.e.: If consecutive shortening of the radius after distal radius fracture resulting ingruency of the wrist joint is relevant, or a scaphoid pseudarthrosis is fixed elastically, or a scaphoic dissociation is effective. The variations were shown in comparison to normal c

Impact of 2-site cryoablation on pain control after the Nuss procedure.

BACKGROUND: Intercostal nerve cryoablation (cryo) reduces opioid use and hospital length of stay (LOS) following the Nuss procedure. Traditionally, cryoablation targets only the main branch of the intercostal nerve (ICN). A 2-site technique has been proposed to additionally target the collateral ICN branch, potentially improving postoperative analgesia. METHODS: We retrospectively analyzed patients ≤21 years of age who underwent the Nuss procedure with the Park System from 01/2023 to 01/2026. Group 1 underwent standard single-site cryoablation inferior to the rib, targeting the main ICN (1-si

A narrative review on pain control interventions for non-surgical pleural procedures.

Pleural diseases are common and frequently result in disabling symptoms, impaired health-related quality of life and hospitalisation. Both diagnosis and management often require pleural procedures and despite a variety of pain control strategies available for clinicians to employ, many procedures are still complicated by pain and discomfort. This can interfere with procedure success and can limit patient satisfaction. This review examines the evidence for pain control strategies for people undergoing non-surgical pleural procedures. A systematic literature search was undertaken to identify pub

The incidence of neuropathic pain after intercostal cryoablation during the Nuss procedure.

PURPOSE: Intercostal nerve cryoblation during the Nuss procedure for pectus excavatum decreases pain, opiate requirement, and hospital length of stay (LOS) compared to thoracic epidural analgesia. However, long-term complications of cryoablation, including neuropathic pain development, are not well studied. METHODS: We conducted a multi-institutional retrospective review of patients following intercostal nerve cryoablation during Nuss bar insertion (11/2015-7/2018). Patients completed the Leeds Assessment of Neuropathic Symptoms and Signs, a validated questionnaire for detecting neuropathic s
