Relapsing polychondritis (RP) is a relatively rare rheumatic condition of unknown aetiology. It is characterized by recurrent episodic inflammation of cartilaginous structures (nose, ear and trachea). The clinical diagnosis of polychondritis can frequently be made with confidence in the absence of histological confirmation. Thoracic splenosis (TS) is autoimplantation of ectopic splenic tissue in the thoracic cavity that occurs following splenic injury. The majority of cases of TS are asymptomatic and are diagnosed in the course of an evaluation of incidentally discovered pulmonary lesions. INTRODUCTION: The objective of this study was to evaluate the necessity of repeat imaging after an initial chest radiograph (CXR) following minimally invasive repair of pectus excavatum (MIRPE).
MATERIALS AND METHODS: A retrospective review was performed on patients who underwent MIRPE from January 2012 to July 2016 at two academic children's hospitals. Data collected included demographics, severity of pectus defect (Haller index [HI]), utilization of CXRs, outpatient follow-up, and clinical outcomes.
RESULTS: A total of 360 patients (171 at Hospital 1 and 189 at Hospital 2) underwent MIRPE.

Subacute thyroiditis may be hard to diagnose, therefore patients are sometimes misdiagnosed and subjected to unnecessary work-up. We report a 37-year-old man with subacute thyroiditis and a high concentration of serum alkaline phosphatase. After aspirin treatment there was clinical improvement and decrease in rapid ESR, and in high serum thyroxin and alkaline phosphatase. The increased alkaline phosphatase, seen in as many as 50% of patients, is of hepatic origin, and is not caused by high serum thyroxin. BACKGROUND: Perigastric appendagitis is a rare diagnosis. Clinical presentation can often mimic an acute surgical abdomen and result in invasive and unnecessary procedures. It occurs as a result of the perigastric ligament appendages spontaneously twisting, causing vascular compromise and local fat necrosis. It is typically self-limited, although potential complications include hemorrhagic necrosis, intestinal obstruction, and localized abscess formation.

The two cases described in this report are examples of a bizarre form of a child abuse. Intentional poisoning with prescription drugs was used by the mother to simulate an underlying disease in her two children. Combined with misleading history and interference with laboratory procedures, she confounded physicians for years, and led to unnecessary hospitalizations, treatment and multiple laboratory procedures for these children. This report may alert physicians to earlier recognition of this form of child abuse.
