[Tuberculosis-associated posterior uveitis].

The commercially available interferon gamma release assays T-SPOT.TB and QuantiFERON-TB Gold/QuantiFERON-TB Gold In-Tube may be important, besides the direct detection with culturing, PCR or the Mantoux skin test, for the diagnosis of tuberculosis as a possible cause of uveitis. It is important to know the limitations of these tests to get a fast and reliable diagnosis which may necessitate initiation of causative treatment of the tuberculosis to prevent further damage to the eye and to avoid multi-drug resistant tuberculosis strains.

Malignant uveitis masquerade syndromes.

The term "Masquerade Syndrome" was first used in ophthalmology in 1967 by Theodore, to describe a case of conjunctival carcinoma that manifested as chronic conjunctivitis. The Uveitis Masquerade Syndromes (UMS) are a group of various ocular diseases that may mimic chronic intraocular inflammation. Many malignant pathologies may result in an appearance masquerading as uveitis. The article reviews most common malignant conditions which may be considered masquerades such as primary intraocular lymphoma, leukemias, uveal melanoma, retinoblastoma, metastatic lesions, and paraneoplastic syndromes, a…

[Listeria monocytogenes kerato-uveitis].

An 86-year-old patient with corneal gutae and diabetes presented with a Listeria monocytogenes serpent ulcer on her right eye. Smears taken from the ulcer revealed Listeria monocytogenes serotype 4b. Despite specific antibiotic therapy including cephalosporins and systemic administration of amikacin, as well as topical gentamycin drops, the eye had to be enucleated on the 16th day after hospitalization. Massive fibrinous reaction and extreme pain causing the patient to reject alimentation were the clinical signs at this time. The anterior chamber aspirate taken perioperatively was sterile.

Tofacitinib in uveitis and scleritis.

Janus kinases (JAKs) are a family of intracellular, non-receptor tyrosine kinases that play a crucial role in the JAK signaling pathway invo … ibitors in ophthalmology is still limited, evidence is gradually emerging on their role in managing uveitis and scleritis. One such agent is tofacitinib, a small-molecule pan-JAK inhibitor. Its therapeutic profile differs from that of biologics due to its partial and reversible inhibition of cytokines. Additionally, owing to its smaller molecular size, th …

Pathogenesis and targets in uveitis.

Uveitis is the most common ocular finding in systemic rheumatic diseases. It is a potentially sight threatening disease seen on its own or in association with systemic rheumatic diseases. Whilst significant advances have been made with biological and small molecular therapies for conditions such as rheumatoid arthritis (RA), ankylosing spondylisitis (AS), juvenile idiopathic arthritis (JIA) and psoriatic arthritis (PsA), the evidence base for managing the ocular complications from these diseases, particularly uveitis, is still limited.

Uveitis in Adults: A Review.

IMPORTANCE: Uveitis is characterized by inflammation of the uvea-the middle portion of the eye composed of the iris, ciliary body, and choroid-causing eye redness, pain, photophobia, floaters, and blurred vision. Untreated uveitis may cause cataracts, glaucoma, macular edema, retinal detachment, optic nerve damage, and vision loss. OBSERVATIONS: Uveitis predominantly affects individuals aged 20 to 50 years. Anterior uveitis affects the iris and ciliary body (41%-60% of cases); intermediate uveitis affects the pars plana (attachment point of vitreous humor) and peripheral retina (9%-15%); post…

[Viral uveitis in the tropics].

Tropical regions are characterized by a hot and humid climate that promotes the emergence or resurgence of endemic or epidemic levels of microorganisms such as viruses. Emerging viral infections are mostly inapparent, mild, or non-specific (flu-like syndrome), but they may include serious, potentially disabling or life-threatening complications. A wide array of ocular manifestations has been recently described in association with specific emerging viral infections, including arboviruses, Human T-cell Lymphotropic Virus Type 1 (HTLV-1), and Ebola. Emerging viral disease should be considered in

Sarcoid Uveitis in Children.

Sarcoidosis is a multi-system granulomatous disease that often presents with uveitis. Although sarcoidosis and sarcoid uveitis typically occur in adulthood, children also may be affected. There are two distinct clinical presentations of the pediatric disease, associated with younger and older age groups, and having different causations. "Early-onset sarcoidosis", beginning at age 5 years or less, is an autosomal dominant genetic disease, caused by a mutation in the NOD2 gene. It is also known as sporadic Blau syndrome or Jabs syndrome. "Adult-type sarcoidosis", usually beginning between the ag…

Pathogenesis of Bacterial Uveitis.

PURPOSE: To describe the pathogenesis and the general immune mechanisms of the most frequent causes of bacterial uveitis. METHODOLOGY: Narrative review. RESULTS: Both extra- and intracellular bacteria can induce uveitis, whereas intracellular bacteria are generally transported into the inner eye via cells of the innate immune system, mainly macrophages. Systemic adaptive immunity is usually induced before the bacteria are localized to the inner eye, and once T and B cells have detected the pathogens behind the blood-eye barriers they elicit an acute and/or chronic inflammatory response deter…

Progress in the genetics of uveitis.

Uveitis is the most common form of intraocular inflammatory disease and is a significant cause of visual impairment worldwide. Aetiologically, uveitis can also be classified into infectious uveitis and non-infectious uveitis. The common non-infectious forms of uveitis include acute anterior uveitis (AAU), Behçet's disease (BD), Vogt-Koyanagi-Harada (VKH) disease, birdshot chorioretinopathy (BSCR), sarcoid uveitis. In addition, a few monogenic autoinflammatory disorders can also cause uveitis, such as Blau Syndrome and haploinsufficiency of A20 (HA20). Although the exact pathogenesis of non-inf…

Treatment of noninfectious uveitis.

Uveitis is a broad term that refers to a large group of eye disorders categorized by intraocular inflammation, a leading cause of visual impairment. Historically, treatment of noninfectious uveitis has depended on corticosteroid drugs. Owing to the myriad of side effects caused by corticosteroids, immunomodulatory therapy has become the preferred treatment for chronic noninfectious intraocular inflammation. Recently, biological response modifiers have established a new era in uveitis therapy, with the range of targets continuing to expand. In this review, we aimed to convey up-to-date informat…

Uveitis in Children and Adolescents.

Childhood noninfectious uveitis leads to sight-threatening complications. Idiopathic chronic anterior uveitis and juvenile idiopathic arthritis-associated uveitis are most common. Inflammation arises from an immune response against antigens within the eye. Ophthalmic work-up evaluates anatomic involvement, disease activity, ocular complications, and disease course. Local and/or systemic glucocorticoids are initial treatment, but not as long-term sole therapy to avoid glucocorticoids-induced toxicity or persistent ocular inflammation. Children with recurrent, refractory, or severe disease requi…

Intraocular biopsy in uveitis.

An intraocular biopsy is performed for diagnostic, prognostic and investigational purposes. Biopsies help to confirm or exclude malignancies and differentiate inflammatory from infectious processes. Histopathological analysis is the final verdict in unresponsive uveitis, atypical inflammation, metastases and masquerade syndromes. Advances and refinement of techniques in cytopathology, immunohistochemistry, microbiological and molecular biologic study offer much more than just diagnosis. They provide prognosis based on cell characteristics and are helpful in planning treatment and intervention.…
