Battery longevity of a helix-fixation dual-chamber leadless pacemaker: results from the AVEIR DR i2i Study.

AIMS: A dual-chamber leadless pacemaker (LP) system that employs distinct atrial and ventricular LP devices (ALP, VLP) has been introduced to clinical practice. Proprietary, low-energy, implant-to-implant (i2i) communication at each beat enables the devices to maintain synchronous atrioventricular sensing and pacing. We evaluated device longevities and contributing factors, such as i2i communication. METHODS AND RESULTS: Patients meeting dual-chamber pacing indications received the dual-chamber LP system as part of a prospective, multi-centre, international clinical trial (Aveir DR i2i Study,

Utility of Surveillance Ambulatory Rhythm Monitoring in the Pediatric Fontan Population.

Our institution established a Fontan surveillance plan, which included ambulatory rhythm monitoring (ARM) at 6, 10, 13, 16 and 19 years old, for early detection of Fontan-associated complications. We conducted a retrospective chart review of Fontan patients followed at our institution 2014-2018 to determine the utility of surveillance ARMs. 139 ARMs from 83 patients were included. ARMs with supraventricular tachycardia, sinus node dysfunction, accelerated junctional rhythm, > 1st degree atrioventricular block, and complex ventricular ectopy were classified as positive for arrhythmia. Arrhythmi

Surgical Outcomes of Permanent Epicardial Pacing in Neonates and Young Infants Less Than 1 Year of Age.

BACKGROUND: Open surgical implantation of epicardial leads in neonates and infants remains the first option of treatment. We reviewed the long-term outcomes after epicardial pacemaker implantation in neonates and infants. METHODS: From 1989 to 2016, 48 patients (16 neonates) underwent pacemaker implantation within the first year of life. Their median age and weight were 66.5days (range: 0∼319 days), and 4.2kg (range: 1.9∼9.3kg), respectively, at the time of first pacemaker implantation. The indications for pacemaker implantation were postoperative or congenital atrioventricular block, sinus n

Complete Atrioventricular Block Complicating Mitral Infective Endocarditis Caused by Streptococcus Agalactiae.

BACKGROUND Infective endocarditis (IE) involving the mitral valve can but rarely lead to complete atrioventricular block (CAVB). CASE REPORT A 74-year-old man with a history of infective endocarditis caused by Streptococcus gordonii (S. gordonii) presented to our emergency room with fever and loss of appetite, which had lasted for 5 days. On admission, results of serologic tests pointed to severe infection. Electrocardiography showed normal sinus rhythm with first-degree atrioventricular block and incomplete right bundle branch block, and transthoracic echocardiography and transesophageal echo

Right or left ventricular pacing in young minipigs with chronic atrioventricular block: long-term in vivo cardiac performance, morphology, electrophysiology, and cellular biology.

BACKGROUND: Left ventricular (LV) dyssynchrony may occur as a result of right ventricular (RV) pacing and is a known risk factor for the development of heart failure. In children with complete atrioventricular block, pacing-induced dyssynchrony lasting for decades might be especially deleterious for LV function. To determine the hemodynamic and ultrastructural remodeling after either RV free wall or LV apical pacing, we used a chronic minipig model. METHODS AND RESULTS: Fourteen piglets 8 weeks of age underwent atrioventricular node ablation and were paced from either the RV free wall or the …

Radiofrequency catheter ablation for tachyarrhythmias in children and adolescents. The Pediatric Electrophysiology Society.

BACKGROUND: Although radiofrequency catheter ablation has been used extensively to treat refractory supraventricular tachycardia in adults, few data are available on its safety and efficacy in children and adolescents. We reviewed registry data obtained from 24 centers to evaluate the indications, early results, complications, and short-term follow-up data in young patients who underwent this procedure. METHODS: Standardized data were submitted for 652 patients who underwent 725 procedures between January 1, 1991, and September 1, 1992. The mean length of follow-up was 13.5 months. RESULTS:

Conversion of paroxysmal atrial fibrillation to sinus rhythm by intravenous pirmenol. A placebo controlled study.

The efficacy of pirmenol (a class I antiarrhythmic agent) as a converter of paroxysmal atrial fibrillation was investigated. Forty patients without congestive heart failure or a history of sinus node disorder were randomly allocated to receive either intravenous pirmenol (50-100 mg) or placebo in a double blind trial. In 12 of 20 patients sinus rhythm was restored 2-16 minutes after pirmenol, and in 3 of 20 patients in the control group it returned within one hour. A nodal escape rhythm was seen during sinus slowing in one patient, but in other patients there was no sinus arrest, atrioventricu

A simple catheter technique for production of AV block.

Atrioventricular (AV) block was produced in animals for experimental purposes by use of a newly devised cannula. It has bipolar electrodes; one electrode on the needle tip and the other 5.0 mm proximally from the tip. Otherwise, the cannula was electrically completely insulated. The cannula was inserted from the right external jugular vein and advanced close to the AV ring. While His bundle electrogram was obtained with the electrodes, the AV node was accessed. The injection site was determined as the point where His bundle electrogram can be well recorded and the atrial electrogram is close t

[Ventricular fibrillation and asystole precursors in myocardial infarct].

Ventricular fibrillation and asystole are a frequent cause of death in myocardial infarction. The data of continuous monitoring the heart in 134 patients with acute myocardial infarction and ventricular fibrillation and asystole. The immediate precursors of ventricular fibrillation were predominantly ventricular extrasystoles. A frequent precursor of ventricular fibrillation consists in paroxysmal ventricular tachycardia. In some patients the ventricular fibrillation was preceded by the block of the bundle of His, non-paroxysmal ventricular tathycardia, escaping contractions and some other arr

Introduction of the Low Voltage Bridge Mapping for the AVNRT RF Ablation in Children - Early Follow-Up Results.

BACKGROUND: Low voltage bridge mapping (LVBM) is an approach to atrio-ventricular nodal reentry tachycardia (AVNRT) ablation that has been postulated to facilitate the procedure. In our center, AVNRT ablations were performed with the traditional radio frequency current (RF) current approach using the CARTO mapping system. The use of LVBM in children has been well described with cryoablation but not with RF. LVBM was introduced in our center in April 2023 and is now routinely used for all AVNRT RF ablations. The aim of this study was to analyze how the introduction of the LVBM influenced the re

Impact of Early Atrioventricular Nodal Blocker Use Post-Transcatheter Aortic Valve Replacement.

BACKGROUND: High-degree atrioventricular block (AVB) and permanent pacemaker (PPM) implantation remain common complications following transcatheter aortic valve replacement (TAVR). Limited evidence exists on whether early postprocedural administration of atrioventricular (AV) nodal blocking medications impacts these outcomes. OBJECTIVES: This study aimed to evaluate the safety of AV nodal blocker use within the first 48 h after TAVR. METHODS: A retrospective cohort study was conducted in adults who underwent TAVR from January 1, 2016 to April 2, 2024. The population was divided into two coho

Peak frequency annotation algorithm-guided slow pathway ablation in typical atrioventricular nodal reentrant tachycardia.

BACKGROUND: The slow pathway potential is difficult to annotate because it is buried within the atrial potential. Omnipolar technology near field can automatically annotate the peak frequency potential associated with acquired intracardiac electrograms. OBJECTIVES: This study aimed to visualize the junction between the transitional cells and the slow pathway using a peak frequency map with omnipolar technology near field and evaluate whether the high-frequency site around the tricuspid annulus (TA) is an effective target for slow pathway ablation. METHODS: This prospective observational stud

Slow Potential at the Entrance of the Slow Conduction Zone in the Reentry Circuit of a Verapamil-Sensitive Atrial Tachycardia Originating From the Atrioventricular Annulus.

BACKGROUND: Slow conduction zone in a verapamil-sensitive reentrant atrial tachycardia originating from atrioventricular annulus is composed of calcium channel-dependent tissue. We examined whether there was a slow potential (SP) at the entrance of the slow conduction zone. METHODS AND RESULTS: We first identified the pacing site from where manifest entrainment and orthodromic capture of the earliest atrial activation site were demonstrated in 40 atrioventricular annulus patients with atrioventricular annulus. Radiofrequency energy was then delivered 2 cm proximal to the earliest atrial activ

Changes in atrioventricular conduction and predictors of pacemaker need after percutaneous implantation of the CoreValve®. Aortic valve prosthesis.

INTRODUCTION AND OBJECTIVES: Although changes in atrioventricular conduction frequently occur after percutaneous implantation of an aortic valve prosthesis, little is known about the mechanisms involved or how these changes progress. We investigated ECG abnormalities and predictors of pacemaker need after percutaneous implantation of the CoreValve® aortic valve prosthesis. METHODS: Between April 2008 and October 2009, 65 patients with symptomatic severe aortic stenosis received a CoreValve® prosthesis. Clinical and ECG parameters were recorded and predictors of pacemaker need due to advanced

Effect of pacing site on the atrial electrogram at target sites for slow pathway ablation in patients with atrioventricular nodal reentrant tachycardia.

Atrial electrograms recorded from target sites during radiofrequency catheter ablation of the slow atrioventricular (AV) nodal pathway are often fractionated and may be associated with a late, high frequency component (the slow pathway potential). The purpose of the current study was to assess the effects of slow pathway ablation on the morphology of the atrial electrogram and to determine whether target site electrograms display direction dependent changes in morphology during atrial pacing maneuvers. Twenty-six patients with typical AV nodal reentry had electrograms recorded from target site
