Laminoplasty versus laminectomy and fusion for cervical spondylotic myelopathy: a cost analysis.

BACKGROUND CONTEXT: Laminectomy with fusion (LF) and laminoplasty (LP) are commonly used to treat cervical spondylotic myelopathy (CSM). The decision regarding which procedure to perform is largely a matter of surgeon's preference, while financial implications are rarely considered. PURPOSE: We aimed to better understand the financial considerations of LF compared to LP in the treatment of CSM. STUDY DESIGN: Retrospective comparative study. PATIENT SAMPLE: Adult patients, 18 years of age or older, who had undergone LF or LP for CSM from 2017 to 2019 at 2 large academic centers were included

Safe Margin beyond Dens Tips to Ventral Dura in Anterior Odontoid Screw Fixation : Analysis of Three-Dimensional Computed Tomography Scan of Odontoid Process.

OBJECTIVE: Anterior odontoid screw fixation is a safe and effective method for the treatment of odontoid fractures. The surgical technique is recommended for perforation of the apical cortex of the dens by the lag screw. However, overpenetration of the apical cortex may lead to potentially serious complications such as damages of adjacent vascular and neural structures. The purpose of this study was to assess the role of three-dimensional computed tomography (CT) scan to evaluate the safe margin beyond dens tip to ventral dura for anterior odontoid screw fixation. METHODS: We retrospectively

Feasibility of occipital condyle screw placement for occipitocervical fixation: a cadaveric study and description of a novel technique.

STUDY DESIGN: Occipital-cervical (OC) stabilization using occipital condyle fixation with a polyaxial screw-rod construct is described. OBJECTIVES: To describe a novel technique and initial radiographic results for posterior OC fixation using the occipital condyles for cranial fixation. SUMMARY OF BACKGROUND DATA: Stabilization of the OC junction remains a challenge. Owing to the regional anatomy and the poor occipital bone purchase, multiple attachment points to the occipital bone are required to increase construct rigidity. To address these issues, we propose a novel OC fixation technique

Mediastinal tracheostomy: techniques, outcomes and quality of life-a systematic review.

BACKGROUND: Mediastinal tracheostomy (MT) is a rare, technically demanding salvage procedure in which the distal trachea is redirected into the upper mediastinum. It is indicated when extensive resection of the proximal trachea precludes conventional tracheal reconstruction or cervical tracheostomy. It is performed for a variety of underlying diseases. The aim of this systematic review was to summarize the available evidence on operative techniques, perioperative outcomes, disease-specific outcomes, and quality of life. METHODS: A systematic review was conducted. PubMed was searched for case

Lateral sclerosis in amyotrophic lateral sclerosis is an upper motor neuron distal axonopathy ("dying back"): a clinical-neuropathological study.

The hallmark neuropathological feature of upper motor neuron (UMN) degeneration in amyotrophic lateral sclerosis (ALS) is the "sclerosis" of the lateral corticospinal tract (CST)-the major axonal pathway connecting UMNs to lower motor neurons (LMNs). However, few studies since Charcot's original descriptions in the nineteenth century have directly correlated clinical UMN phenotypes with corresponding neuropathological findings. We examined the pathology of the lateral CST at the cervical, thoracic, and lumbar levels of the spinal cord from ALS patients with predominantly UMN (UMN-p) phenotypes

Spinal cord swelling and intradural compression predict neurological recovery after acute cervical traumatic spinal cord injury.

Intradural spinal cord compression impairs perfusion pressure and is putatively rate-limiting for recovery after traumatic spinal cord injury (tSCI). After cervical tSCI, even minimally improved tissue preservation may help promote neurological recovery. To assess the nature and extent of spinal cord swelling and compression post-acute cervical tSCI, we evaluated several baseline MRI parameters including BASIC score, intramedullary lesion (IML) length, maximal canal compromise (MCC), maximal spinal cord compression (MSCC), extent of cord compression (ECC), maximal swollen anteroposterior diame

Biomechanics and Physiology for Propelling Wheelchair Uphill Slope.

A vertical slope of sidewalks significantly inhibits to the mobility of manual wheelchair users in their daily life. International guidelines of the vertical slope are specified approximately 4% or 5% (1:20) gradient or less as preferred, and allow 8.3% (1:12) as its maximum when it is impossible. Relevant research of the physical strain for wheelchair users with pushing on slopes, and the validity assessment of slope guidelines have been investigated. However, the analysis for the effect of a slope distance and their transient performance are still remained. The purpose of this study is to cl

Artificial discs for lumbar and cervical degenerative disc disease -update: an evidence-based analysis.

OBJECTIVE: To assess the safety and efficacy of artificial disc replacement (ADR) technology for degenerative disc disease (DDD). CLINICAL NEED: Degenerative disc disease is the term used to describe the deterioration of 1 or more intervertebral discs of the spine. The prevalence of DDD is roughly described in proportion to age such that 40% of people aged 40 years have DDD, increasing to 80% among those aged 80 years or older. Low back pain is a common symptom of lumbar DDD; neck and arm pain are common symptoms of cervical DDD. Nonsurgical treatments can be used to relieve pain and minimize

The impact of over-distraction on adjacent segment pathology and cage subsidence in anterior cervical discectomy and fusion.

Over-distraction has been shown to be a risk factor for cage subsidence and postoperative neck pain after anterior cervical discectomy and fusion (ACDF). Biomechanical studies have demonstrated increased adjacent segment intradiscal pressure after ACDF. The purpose of this study is to determine if over-distraction of the index disc has an effect on adjacent segment pathology. A consecutive series of 145 patients who received primary ACDF for cervical degenerative pathologies from January 2010 to December 2017 were retrospectively reviewed. The patients were divided into: (1) Over-distraction g

Rationale & design of the PROMISES study: a prospective assessment and validation study of salivary progesterone as a test for preterm birth in pregnant women from rural India.

BACKGROUND: In India, 3.6 million pregnancies are affected by preterm birth annually, with many infants dying or surviving with disability. Currently, there is no simple test available for screening all women at risk of spontaneous PTB in low income setting, although high resource settings routinely use cervical length measurement and cervico-vaginal fluid fetal fibronectin for identification and care of women at risk due to clinical history. In rural India, where the public health system has limited infrastructure, trained staff and equipment, there is a greater need to develop a low-cost scr…
