Nutrition in brain development and aging: role of essential fatty acids.

The essential fatty acids (EFAs), particularly the n-3 long-chain polyunsaturated fatty acids (LCPs), are important for brain development during both the fetal and postnatal period. They are also increasingly seen to be of value in limiting the cognitive decline during aging. EFA deficiency was first shown over 75 years ago, but the more subtle effects of the n-3 fatty acids in terms of skin changes, a poor response to linoleic acid supplementation, abnormal visual function, and peripheral neuropathy were only discovered later. Both n-3 and n-6 LCPs play important roles in neuronal growth, dev

Dietary recommendations for children and adolescents with diabetes: an implementation paper. British Diabetic Association's Professional Advisory Committee.

General Recommendations: 1. Children with diabetes mellitus have the same basic nutritional requirements as other children. 2. Dietary recom … not differ from that of infants without diabetes. Breast feeding should be encouraged or a standard infant formula-feed used. Solids may be introduced from 3-6 months, but breast milk or a modified infant formula is encouraged as part of the increasingly mixed diet to at least the end of the first year. 10. Diabetes is rare in infancy so expert advice should be …

Maximization of calcium and phosphate in neonatal total parenteral nutrition.

BACKGROUND: Appropriate calcium and phosphate supplementation is essential for bone growth in preterm infants. Using Rehabix-K2™ (AY Pharmaceuticals, Tokyo, Japan) and Pleamin-P Injection™ (Fuso Pharmaceutical Industries, Osaka, Japan) as the total parenteral nutrition (TPN) and amino acid solution, respectively, we investigated ways of maximizing calcium and phosphate in the TPN solution. METHODS: Rehabix-K2, Pleamin-P, calcium gluconate, sodium phosphate, 50% glucose, and water were mixed in varying proportions to create 16 formulations. Precipitation assessment was done three times for eac

Effective lens position and pseudophakic refraction prediction error at 10½ years of age in the Infant Aphakia Treatment Study.

BACKGROUND: The refraction prediction error (PE) for infants with intraocular lens (IOL) implantation is large, possibly related to an effective lens position (ELP) that is different than in adult eyes. If these eyes still have nonadult ELPs as they age, this could result in persistently large PE. We aimed to determine whether ELP or biometry at age 10½ years correlated with PE in children enrolled in the Infant Aphakia Treatment Study (IATS). METHODS: We compared the measured refraction of eyes randomized to primary IOL implantation to the "predicted refraction" calculated by the Holladay 1

Complementary Feeding in Developed Countries: The 3 Ws (When, What, and Why?).

While a wide knowledge exists on the effects of breast milk or infant formula on growth and infant development, less attention has been paid to the importance of complementary feeding (CF). This review focuses on current recommendations for optimal introduction of CF in healthy full-term European infants and discusses the potential impact of this type of feeding on health outcomes. Overall, exclusive breastfeeding is recommended at least for 4 months and preferable for 6 months, followed by the introduction of CF alongside breast milk; infants' nutrient requirements must meet the differences b…

Nutrition beyond the neonatal intensive care unit: Best practices and optimal outcome measures.

Despite advancements in nutritional management during the neonatal intensive care unit hospitalization, preterm infants remain smaller than term-born counterparts at the time of hospital discharge. The goal for nutritional management after discharge from the neonatal intensive care unit is to address ongoing nutrient and growth catch-up requirements and support growth and neurodevelopment while balancing the potential risks of too much growth. The current standard of care approach to post-hospitalization nutrition includes an individualized approach to nutritional supplementation with human mi

Distal Ileal Atresia in a Preterm Infant with Minor Omphalocele: A Case Report.

INTRODUCTION: Omphalocele is a congenital abdominal wall defect that is characterized by herniation of the abdominal viscera through the umbilical ring. Compared with gastroschisis, omphalocele is less frequently associated with ileal atresia. This report describes a preterm newborn with a minor omphalocele complicated by ileal atresia, a complication that may have been previously underestimated. CASE PRESENTATION: A 5-day-old preterm male infant (gestational age 34 weeks, birth weight 2005 g) presented with delayed meconium passage and persistent bilious gastric aspirates. Know the Flow: Milk Flow Rates From Bottle Nipples Used in the Hospital and After Discharge.

BACKGROUND: Milk flow rate may play an important role in an infant's ability to safely and efficiently coordinate sucking, swallowing, and breathing during feeding. PURPOSE: To test milk flow rates from bottle nipples used in the hospital and after discharge. METHODS: Bottle nipples used in hospitals (10 unique types) and available nationwide at major retailers (15 unique types) were identified. For each of the 25 nipple types, 15 nipples of that type were tested by measuring the amount of infant formula extracted in 1 minute by a breast pump. Mean milk flow rate (mL/min) and coefficient of …

Congenital maltase-glucoamylase deficiency associated with lactase and sucrase deficiencies.

BACKGROUND: Multiple enzyme deficiencies have been reported in some cases of congenital glucoamylase, sucrase, or lactase deficiency. Here we describe such a case and the investigations that we have made to determine the cause of this deficiency. METHODS AND RESULTS: A 2.5 month-old infant, admitted with congenital lactase deficiency, failed to gain weight on a glucose oligomer formula (Nutramigen). Jejunal mucosal biopsy at 4 and 12 months revealed normal histology with decreased maltase-glucoamylase, sucrase-isomaltase, and lactase-phlorizin hydrolase activities. Testing with a C-starch/bre

A Study on the Identification of High-Risk Factors and the Establishment of a Prediction Model for Uterine Rupture in an Unscarred Uterus During Pregnancy.

OBJECTIVE: The diagnosis of uterine rupture was often delayed or misdiagnosed, leading to maternal morbidity and fetal mortality. The current study was performed to retrospectively identify high-risk factors of uterine rupture in an unscarred uterus (USU), and to establish a model to predict uterine rupture in USU during pregnancy. METHODS: This was a retrospective multi-center study including five regional medical centers from the inception of each medical center to December 31 2021. Out of 547 325 deliveries, 28 patients with USU who had a uterine rupture during pregnancy were recruited. Th

Lactoferrin infant feeding trial_Canada (LIFT_Canada): protocol for a randomized trial of adding lactoferrin to feeds of very-low-birth-weight preterm infants.

BACKGROUND: In Canada alone, almost 3000 VLBW infants are born and treated annually with almost 1200 going onto death or survival with severe brain injury, chronic lung disorders, aggressive retinopathy of prematurity, late-onset sepsis, or significant necrotizing enterocolitis. Lactoferrin is an antimicrobial, antioxidant, anti-inflammatory iron-carrying, bifidogenic glycoprotein found in all vertebrates and in mammalian milk, leukocytes and exocrine secretions. Lactoferrin aids in creating an environment for growth of beneficial bacteria in the gut, thus reducing colonization with pathogenic

Neonatal metabolic acidosis caused by food proteininduced enterocolitis syndrome: a case report.

Food protein-induced enterocolitis syndrome (FPIES) is a non-IgE mediated allergic reaction with gastrointestinal symptoms, such as vomiting and diarrhea. FPIES diagnosis is based on clinical criteria and on a food challenge test. It is an unknown disease in neonatal units due to its nonspecific symptoms in newborn infants. An elevated methemoglobin level is a simple way to approach diagnosis. Here we describe a clinical case of a newborn admitted to the emergency department because of dehydration, lethargy, vomiting, diarrhea, severe metabolic acidosis, and a high methemoglobin level. Clinica

Bacteremia in a Newborn with Hypocalcemic Seizures and Vitamin D Deficiency.

Infants with neonatal hypocalcemia often present with seizures, and neonatal hypocalcemia can be due to parathyroid (PTH) insufficiency or resistance. Causes of hypocalcemia with PTH elevation include increased phosphate load, vitamin D deficiency (VDD) or defects in metabolism, renal dysfunction, hypomagnesemia, genetic mutations resulting in end-organ resistance to PTH, or critical illness. Hypocalcemia has also been shown to be associated with Gram-negative bacteremia and sepsis in adults. We present the case of a full-term, formula-fed newborn presenting with late-onset hypocalcemic seizur
