[The cytofluorometric opinion of expression of adhesion molecules CD11a and CD54 on lymphocytes T and B in children's hypertrophied adenoids, which are sick on the otitis media with effusion].

INTRODUCTION: Otitis media with effusion (WZU) is highly prevalent among young children. The hypertrophied adenoid favorables to development (WZU). The pharyngeal tonsil delivers the essential lymphocytes for developed immunological answer. The migrations of lymphocytes to pharyngeal tonsil and to places inflammatory proceed with part of adhesive molecules (CD11a, CD54) presented on lymphocytes. The aim of this study was examination the percentage of the lymphocytes CD4+, CD8+ and lymphocytes B with expression of adhesion molecules ICAM-1 (CD54) and LFA-1 (CD11a). We examined their expression

The effects of chin lift and jaw thrust while in the lateral position on stridor score in anesthetized children with adenotonsillar hypertrophy.

Obstruction of the upper airway is a major challenge for anesthesiologists administering general anesthesia in spontaneously breathing patients, especially in pediatric anesthesia with adenotonsillar hypertrophy. Lateral positioning is a simple treatment for obstructive sleep apnea and also decreases collapsibility of the pharynx in anesthetized adults with obstructive sleep apnea. In this study, we examined the effects of body position shifting and common airway maneuvers, such as chin lift and jaw thrust, on airway patency (stridor score) in anesthetized children scheduled for adenotonsillec

Acoustic reflectance of pharyngeal structures in children.

The aim of this study was to examine whether Eccovision Reflectance Pharyngometer could assess the anatomical structure of the upper airway in young children. Secondary aims were to assess changes in pharyngeal volume in children with tonsillar (Group A, n=13) and adenoidal hypertrophy (Group B, n=17) at pre- and post- surgical procedures, respectively and further compare them to children who underwent myringotomy (Control Group C, n=10). In all 40 children (aged 3-9 years, median 6 years) enrolled in this pilot prospective study, six recordings (equally dispersed at pre- and 3 month post- ope

A prospective randomized controlled study of the efficacy of ketamine for postoperative pain relief in children after adenotonsillectomy.

BACKGROUND: Adenotonsillectomy is commonly needed by children with obstructive sleep apnoea syndrome. This population is at high risk of life threatening airway obstruction in the postoperative period. METHODS: Fifty children were studied to test the efficacy of an alternative analgesic to the use of opioids in providing analgesia in the immediate postoperative period. Patients were randomized to receive either 0.1 mg.kg-1 morphine or 0.5 mg.kg-1 ketamine at induction. RESULTS: Ketamine was as effective as morphine with no additional side-effects. CONCLUSIONS: Ketamine is a safe and effecti

Assessment of adenoidal obstruction in children: clinical signs versus roentgenographic findings.

OBJECTIVE: As part of a comprehensive study of indications for tonsillectomy and adenoidectomy, we investigated the reliability of standardized clinical assessments and standardized roentgenographic assessments of adenoidal obstruction of the nasopharynx, and the degree of correlation between clinical assessments and roentgenographic assessments. METHODS: We rated the degree of patients' mouth breathing and patients' speech hyponasality on a 4-point scale (none = 1; mild = 2; moderate = 3; marked = 4), we averaged the ratings for each child to obtain a Nasal Obstruction Index, and we determin…

[Oro-pharyngeal burn during electrodissection of the adenoid and tonsil].

We present a case of oro-pharyngeal burn which occurred during electrodissection of the adenoid and tonsil in a 5-year-old boy. We intubated the patient with an uncuffed spiral tube of appropriate size and noticed a slight gas leak during positive-pressure ventilation. Anesthesia was maintained with a mixture of 60% nitrous oxide, 40% oxygen and 2.5% sevoflurane. During manipulations of the right tonsil, orange-colored flame blew out about 5 cm from the mouth. Fortunately, the patient underwent the operative procedures without any further troubles, recovered fully from the grade 1 burn in the

Do systemic corticosteroids effectively treat obstructive sleep apnea secondary to adenotonsillar hypertrophy?

To determine if pediatric obstructive sleep apnea syndrome (OSAS) caused by adenotonsillar hypertrophy (ATH) could be treated by a short course of systemic corticosteroids, we conducted an open-label pilot study in which standardized assessments of symptomatology, OSAS severity, and adenotonsillar size were performed before and after a 5-day course of oral prednisone, 1.1+/-0.1 (+/-SE) mg/kg per day. Outcome measures included symptom severity, adenotonsillar size, and polysomnographic measures of OSAS. Selection criteria included age from 1 to 12 years, ATH, symptomatology suggesting OSAS, an

A prospective study of factors which may predispose to post-operative tonsillar fossa haemorrhage.

Adeno-tonsillectomy and tonsillectomy are frequently performed and haemorrhage remains an important post-operative complication. A prospective study of 1090 patients undergoing surgery over a 12-month period was undertaken investigating whether the following factors influenced post-operative haemorrhage: age and sex of patients, method of tonsillectomy, haemostasis and premedication, month of operation and experience of surgeon. A significantly higher incidence of post-operative haemorrhage occurred in the group aged 16-25 years and significantly more males suffered post-operative haemorrhage.

Risk of influenza and COVID-19 illness and pediatric obstructive sleep apnea: a TriNetX cohort with 5-year follow-up.

BACKGROUND: Obstructive sleep apnea (OSA) is associated with impaired immune responses. Prior small-scale investigations suggested that pediatric OSA leads to a more severe clinical course of common childhood viruses. Whether OSA increases the risk of infection remains unknown. OBJECTIVES: To examine whether children with OSA are at increased risk of influenza or COVID-19, and to assess the effect of adenotonsillectomy. METHODS: Using TriNetX, we compared children aged 2-18 years with incident OSA and matched controls without OSA. Outcomes were ICD-10-coded influenza and COVID-19 over a five

The application of xylocaine 10% pump-spray to improve immediate post-adenotonsillectomy pain in children: A randomized controlled trial.

INTRODUCTION: Post-adenotonsillectomy pain is often severe, requiring substantial analgesia in the first 48-72 h. This pain is not only distressing to the patient and his or her parents, but often reflects poorly on an otherwise well performed procedure. Safe, simple and effective post-adenotonsillectomy pain control is still clinically elusive, even though a multitude of surgical and analgesic interventions have been proposed. OBJECTIVES: To investigate the analgesic properties of immediate post-operative application of xylocaine 10% pump spray to the tonsillar fossae in children having unde
