Fungal keratitis following laser in situ keratomileusis (LASIK) is extremely rare. Except in cases that follow trauma, the source of fungal organisms is unknown in published reports. We report a case of keratitis following LASIK that could be directly traced to a fungal skin infection of the patient's pet cat.

Neoscytalidium dimidiatum is a mold known to cause onychomycosis and dermatomycosis; however, it is an extremely rare cause of systemic infection. We report a case of Lasiodiplodia theobromae pneumonia in a patient who died 14 days after cadaveric-liver transplantation. His condition was complicated by Enterococcus faecium peritonitis. Direct microscopy analysis of the bronchoalveolar lavage specimens showed septate hyphae. A dematiaceous mold was recovered and identified as L. theobromae by microscopic morphology and EF1alpha gene sequencing.

PURPOSE: To report a case of bilateral endogenous Phialemonium curvatum endophthalmitis, secondary to intrapenile injections for erectile dysfunction.
DESIGN: Observational case report.
METHODS: A 71-year-old man with P. curvatum endocarditis and bilateral decreased vision was diagnosed as having bilateral endogenous endophthalmitis. P. curvatum was identified in cultures that were performed on samples of the vitreous. Treatment consisted of bilateral vitrectomy and intraocular and systemic antifungals.
