Cross validation of USARIEM heat strain prediction models. U.S. ARMY Research Institute of Environmental Medicine.

HYPOTHESIS: This study was a cross validation of three heat strain prediction models developed at the U.S. Army Research Institute of Environmental Medicine: the ARIEM, HSDA, and ARIEM-EXP models ability to predict core temperature. METHODS: Seven heat-acclimated subjects completed twelve experimental tests, six in each of two hot climates, at three exercise intensities and two uniform configurations in each climate. RESULTS: Experimental results showed physiological responses as expected with heat strain increasing with work load and level of protective clothing, but with similar heat strai

+Gz associated stenosis of the cervical spinal canal in fighter pilots.

Previous magnetic resonance imaging (MRI) studies have shown that repeated exposure to +Gz forces can cause premature degenerative changes of the cervical spine (i.e. a work-related disease). This paper reports on two clinical cases of +Gz-associated degenerative cervical spinal stenosis caused by dorsal osteophytes in fighter pilots. Conventional x-rays and MRI were used to demonstrate narrowing of the cervical spinal canal. The first case was complicated by a C6-7 intervertebral disk prolapse and a congenitally narrow spinal canal. The second case involved progressive degenerative spinal ste

Effects of sustained low-level elevations of carbon dioxide on cerebral blood flow and autoregulation of the intracerebral arteries in humans.

Cerebral blood flow velocity (CBFv) was measured by insonating the middle cerebral arteries of four subjects using a 2 Mhz transcranial Doppler. Ambient CO2 was elevated to 0.7% for 23 d in the first study and to 1.2% for 23 d in the same subjects in the second study. By non-parametric testing CBFv was elevated significantly by +35% above pre-exposure levels during the first 1-3 d at both exposure levels, after which CBFv progressively readjusted to pre-exposure levels. Despite similar CBFv responses, headache was only reported during the initial phase of exposure to 1.2% CO2. Vascular reactiv

Tolerance to head-up tilt and suspension with elevated legs.

OBJECTIVE: Orthostatic hypotension is usually a benign event. However, some patients are disabled by frequent syncopal events, and vertical transportation during helicopter rescue, for example, may even be fatal. Normal orthostatic tolerance is poorly defined, so we evaluated the response to 50 degrees head-up tilt. Also, the effect of leg elevation was examined in order to establish the influence of venous return, and a fatal accident associated with orthostasis is reported. METHODS: There were 79 volunteers who were subjected to 50 degrees head-up tilt, and 9 subjects performed 1 h of suspe

The "Space Cycle" Self Powered Human Centrifuge: a proposed countermeasure for prolonged human spaceflight.

The Self Powered Human Centrifuge, or Space Cycle, is a countermeasure to the adverse physiologic effects of prolonged human exposure to spaceflight microgravity. This unique device simultaneously provides exercise, impact loading and gravity analogous acceleration to emulate conditions on Earth. One or two crewmembers pedal themselves about a shaft mounted to the space craft located "above" their heads. This creates a short arm centrifuge with a head-to-toe acceleration orientation. The potential advantages of the Space Cycle include: a) reversal of cephalad fluid shift, minimizing post fligh

Drinking milk or water has no effect on the severity of optokinetic rotation-induced symptoms of motion sickness.

BACKGROUND: The present study investigated the effects of drinking milk or water before viewing an optokinetic rotation drum on the severity of motion sickness and abnormal gastric myoelectric activity. METHODS: There were 27 subjects who drank 300 ml of milk or water, or drank nothing, in three separate sessions with 2-d intervals between every two sessions. Subjects in each session then viewed a vertically striped optokinetic rotation drum for 16 min. Symptoms of motion sickness and electrogastrograms (EGGs) were measured. RESULTS: Subjects reported no significant difference in symptoms of

Probable decompression sickness in a trainee with atopic dermatitis.

Hypobaric chamber training has a potential risk of inducing decompression sickness (DCS). A case of a patient with an atopic dermatitis who complained of paresthesia and numbness in his left arm and shoulder during the altitude exposure is presented here. His symptoms were severe enough for the attending medical officer to diagnose Type II DCS, but it turned out to be a probable case of simple skin bends requiring no treatment. The author can find no better explanation for this discrepancy than the contribution of dermatitis. The possibility of atopic dermatitis confounding the correct diagnos

Suppression of the reactive hyperemic response in the forearm due to local hand cooling.

METHODS: Forearm blood flow (FBF) was measured by electrical impedance plethysmography in order to identify the influence of local cooling of the contralateral hand after 3 min of arterial occlusion. Ten volunteers (six females, four males) were randomly assigned to the following three experimental conditions on separate days: a) 6 min of local hand cooling (5-7 degrees C); b) 3 min of arterial occlusion of the forearm; and c) combined arterial occlusion and local cooling. RESULTS: All subjects experienced the typical pressor response during 6 min of hand cooling as evidenced by the significa

Neurological manifestation of arterial gas embolism following standard altitude chamber flight: a case report.

In the course of a decompression at flight level 280 (28,000 ft) in an altitude chamber flight, a 45-yr-old cabin air traffic controller developed sudden numbness in his left upper and lower extremities and, soon after, complete paralysis in the left side, dysarthria and left facial palsy. A presumptive diagnosis of arterial gas embolism (AGE) was made and hyperbaric oxygen therapy (HBO) was given after airevac of the patient to the closest compression facility. Complete resolution of the symptoms was obtained after treatment Table VI-A (extended), plus 3 consecutive HBO treatments (90 min of

Effects of hypovolemia on aortic baroreflex control of heart rate in humans.

INTRODUCTION: To test the hypothesis that hypovolemia can acutely increase the sensitivity of chronotropic baroreflex response, eight men (21-45 yr old) underwent measurements of heart rate response to aortic baroreceptor stimulation under normovolemic and hypovolemic conditions. METHODS: Hypovolemia was acutely induced by a bolus injection of 30 mg of furosemide. The sensitivity of the aortic-cardiac baroreflex was determined with a approximately 15 mmHg elevation in mean arterial pressure (MAP) induced by steady-state infusion of 30 to 97 micrograms.min-1 phenylephrine (PE) combined with ap

Disorientation and postural ataxia following flight simulation.

BACKGROUND: Motion sickness-like symptoms can afflict pilots training in military simulators. This simulator sickness involves symptoms of gastrointestinal distress, eyestrain and disorientation. A simulator sickness Questionnaire (SSQ) with subscales available for each of these dimensions has been developed to assess the problem. HYPOTHESIS: This study examined the hypothesis that there is a strong correlation between the SSQ subscale which summarized self report of disorientation symptoms and an objective measure of post-simulation postural instability. METHODS: Data from two Navy simulato

Effect of direction of head movement on motion sickness caused by Coriolis stimulation.

BACKGROUND: During constant speed rotation of the body, head rotation about an axis other than the axis of rotation of the body (i.e., Coriolis is stimulation) induces motion sickness. HYPOTHESES: The position of the body relative to the center of rotation will influence the sickness caused by Coriolis stimulation; the direction of head movement will not affect the sickness caused by Coriolis stimulation. METHOD: There were 24 seated subjects (12 male, 12 female) who made 30 degrees pitch motions of the head every 30 s while rotating about a vertical axis at 10 r.p.m. on a turntable at two s

Hypoxic syncope.

INTRODUCTION: Syncope at altitude of otherwise healthy individuals is a well-known phenomenon (22). We report on the cardiovascular effects observed in subjects exposed to hypoxia to illustrate the role of the sympathetic-adrenergic system in hypoxic syncope. This study describes unexpected episodes of (near) syncope during two crossover trials at simulated altitude in a low pressure chamber. METHODS: In study A, 30 healthy male volunteers underwent 4 exposures to short-term (20 min) acute severe hypoxia (20,000 ft or 6096 m) to assess psychological performance. In study B, five volunteers we

Hyperthermia-induced cardiac arrest in monkeys: limited efficacy of standard CPR.

BACKGROUND: Successful resuscitation from heatstroke cardiopulmonary arrest has been only partially explored and the data covering the post resuscitation pathophysiology leading to secondary arrest is, in most cases, insufficient. HYPOTHESIS: Following heatstroke-cardiopulmonary arrest, successful resuscitation may be achieved by standard CPR with surface cooling and administration of glucose. We ponder the sequence of early circulatory responses and the pathophysiological changes following successful resuscitation. METHODS: We exposed 12 pigtail monkeys to total-body hyperthermia (cerebral
