Patterns of iron deposition and classification in parkinsonian syndromes: a quantitative susceptibility mapping study.

Differential diagnosis of parkinsonian syndromes remains challenging because of overlapping clinical symptoms. We characterized brain iron deposition using whole-brain quantitative susceptibility mapping (QSM) and developed internally validated exploratory machine learning (ML) classifiers. This single-center retrospective study included 119 patients with Parkinson's disease (PD), 62 with multiple system atrophy-parkinsonian type (MSA-P), 25 with progressive supranuclear palsy (PSP), and 56 healthy controls (HC). Voxel-wise and region-of-interest (ROI) analyses were performed, and classifiers

Elevated levels of cerebrospinal fluid soluble triggering receptor expressed on myeloid cells 2 in multiple system atrophy: a marker of disease-associated microglial activation.

Multiple system atrophy (MSA) is increasingly being recognized to be characterized by neuroinflammation, especially involving the activation of microglia. Soluble triggering receptor expressed on myeloid cells 2 (sTREM2) in cerebrospinal fluid (CSF) has been proposed as a biomarker of microglial activation; however, its role in MSA remains unclear. Levels of glycoprotein nonmetastatic melanoma protein B (GPNMB) and neurofilament light chain (NfL) may also indicate MSA-related neuroinflammatory and neurodegenerative processes. In this cross-sectional study, we analyzed CSF samples from 44 MSA p

Serum NFL discriminates Parkinson disease from atypical parkinsonisms.

OBJECTIVE: To investigate the diagnostic value of serum neurofilament light chain (NFL) in patients with clear signs of parkinsonism but whose specific diagnosis was yet uncertain. METHODS: Serum samples were collected from patients with clear signs of parkinsonism but with uncertain diagnosis at the inclusion. Clinical diagnoses of Parkinson disease (PD) and atypical parkinsonism disorders (APDs) were established after 3 years of follow-up and updated again after a maximum of 12 years in case longer follow-up data were available. Serum NFL was quantified by single molecule array in patients

Motor, cognitive and behavioral differences in MDS PSP phenotypes.

INTRODUCTION: Movement Disorder Society (MDS) new diagnostic criteria for Progressive Supranuclear palsy (PSP) identifying different disease phenotypes were recently released. The aim of the present study is to report on the cognitive and behavioral features of the different phenotypes diagnosed according to the MDS criteria. METHODS: Forty-nine PSP patients underwent an extensive battery of clinical assessments. Differences between PSP subtypes were computed with χ2 or ANOVA tests. Using the z scores, subjects were classified as having normal cognition, mild cognitive impairment, single or m

Pathological correlations of [F-18]-AV-1451 imaging in non-alzheimer tauopathies.

OBJECTIVE: Recent studies have shown that positron emission tomography (PET) tracer AV-1451 exhibits high binding affinity for paired helical filament (PHF)-tau pathology in Alzheimer's brains. However, the ability of this ligand to bind to tau lesions in other tauopathies remains controversial. Our goal was to examine the correlation of in vivo and postmortem AV-1451 binding patterns in three autopsy-confirmed non-Alzheimer tauopathy cases. METHODS: We quantified in vivo retention of [F-18]-AV-1451 and performed autoradiography, [H-3]-AV-1451 binding assays, and quantitative tau measurements

[Dementia with motor and language disorders].

Memory is not the only core diagnostic criteria in Alzheimer's disease and many dementias are characterized by other cognitive deficits. Moreover dementias are often associated with multiple and complex motor signs. The first part of this reviewcovers parkinsonism in diffuse Lewy Body Disease and other neurodegenerative diseases, corticobasal syndrome, or motor deficit in the motoneurone disease-frontotemporal dementia spectrum. In the second part, primary progressive aphasia and its three variants including basic clinical evaluation are described. These complex clinical syndromes involving mo

Automated Differential Diagnosis of Early Parkinsonism Using Metabolic Brain Networks: A Validation Study.

UNLABELLED: The differentiation of idiopathic Parkinson disease (IPD) from multiple system atrophy (MSA) and progressive supranuclear palsy (PSP), the most common atypical parkinsonian look-alike syndromes (APS), can be clinically challenging. In these disorders, diagnostic inaccuracy is more frequent early in the clinical course when signs and symptoms are mild. Diagnostic inaccuracy may be particularly relevant in trials of potential disease-modifying agents, which typically involve participants with early clinical manifestations. In an initial study, we developed a probabilistic algorithm t

FDG-PET in pathologically confirmed spontaneous 4R-tauopathy variants.

The 4-repeat (4R)-tauopathies can be clinically heterogeneous and difficult to diagnose. An FDG-PET pattern of hypometabolism has been previously reported in clinically suspected 4R-tauopathies. Considering that pathological confirmation has not been used as inclusion criteria in these studies, however, the possibility exists that atypical cases were excluded. We studied pathologically confirmed cases of 4R-tauopathies to determine if FDG-PET patterns of hypometabolism different than those previously described exist. We identified all autopsy confirmed 4R-tauopathies with FDG-PET imaging perfo

Incidence and pathology of synucleinopathies and tauopathies related to parkinsonism.

IMPORTANCE: The frequency and distribution of synucleinopathies and tauopathies manifesting with parkinsonism in the general population are poorly understood, thus affecting health care planning and research. OBJECTIVE: To investigate the incidence and distribution of specific types of parkinsonism and related proteinopathies. DESIGN: We used the medical records-linkage system of the Rochester Epidemiology Project to identify all subjects who received a screening diagnostic code related to parkinsonism in Olmsted County, Minnesota, from January 1, 1991, through December 31, 2005 (15 years).

The odor stick identification test for Japanese differentiates Parkinson's disease from multiple system atrophy and progressive supra nuclear palsy.

BACKGROUND: Progressive supranuclear palsy (PSP) and parkinsonian variant of multiple system atrophy (MSA-P) are clinically difficult to differentiate from idiopathic Parkinson's disease (PD), particularly in the early stages of the disease. Previous reports indicated that the olfactory function is relatively intact or slightly reduced in patients with PSP and MSA-P, suggesting that the odor stick identification test for Japanese (OSIT-J), which is a short and simple noninvasive test that is potentially useful clinically for detecting early-stage PD in Japan, may be useful in the differential

The relationship between clinical and pathological variables in Richardson's syndrome.

In order to determine the relationship between regional neuropathology and severity of clinical features in Richardson's syndrome (PSP-RS), the following hypotheses were tested: (1) executive dysfunction relates to prefrontal pathology; (2) language difficulties to pathology in Broca's area and/or the perirhinal cortex; and (3) visuospatial impairment to pathology in the supramarginal region. A prospectively studied case series of brain donors at a specialist clinic in Addenbrooke's Hospital Cambridge, UK, were examined. All those fulfilling postmortem criteria for PSP-RS and their last cognit

Incidence of Parkinson's disease and atypical parkinsonism: Russian population-based study.

Data on the incidence of Parkinson's disease (PD) and atypical parkinsonian syndromes (APS) in East European countries and Asia are limited. The objective of this prospective population-based study was to determine the incidence of PD and APS in the Russian population. The study area was a large district of Moscow with a population of 1,237,900 inhabitants. Multiple sources of case ascertainment were used to identify incident cases of PD and APS between July 2006 and December 2008. All incident cases were examined by a specialist and followed up prospectively to confirm the diagnosis. The age-

PET and SPECT functional imaging in Parkinson's disease.

Neuroimaging studies of the dopaminergic system with positron emission tomography and single photon emission computed tomography radioligands are useful in the assessment of neuronal degeneration in Parkinson's disease. Abnormalities can be identified early in the disease process and used to monitor progression as well as the effect of treatment. Studies of dopamine receptors and brain metabolism can help identify those patients who have other Parkinsonian syndromes, such as multiple system atrophy, progressive sopranuclear palsy or diffuse Lewy body dementia. The current available diagnostic

Steele-Richardson-Olszewski syndrome in a patient with a single C212Y mutation in the parkin protein.

Steele-Richardson-Olszewski syndrome (SROS) is a neurodegenerative disorder of unknown aetiology, most frequently sporadic. Familial cases of SROS have been described. An intronic polymorphism of the tau gene is associated with sporadic SROS and mutations of the tau gene are present in atypical cases of SROS. The role of tau has been excluded in other families with pathology proven SROS, suggesting that this syndrome may have multiple causes. An 82-year-old patient, father of 3 children with autosomal recessive juvenile parkinsonism due to combined heterozygous mutations of the parkin gene, de

Positron emission tomography in parkinsonism.

Positron emission tomography provides researchers and clinicians with information on cerebral blood flow, metabolic pathways or neurotransmission systems, which may help to understand the physiopathology of movement disorders. Studies of the nigrostriatal dopaminergic pathway and data on cerebral glucose metabolism contribute to discriminate between parkinsonian syndromes. New techniques for image processing and statistical analysis, and brain activation studies better emphasize dysfunction of cortico-subcortical neuronal networks responsible for movement disorders, or functional modification

Asymmetry in Atypical Parkinsonian Syndromes-A Review.

Background/Objectives: Atypical parkinsonian syndromes (APSs) are a group of neurodegenerative disorders that differ from idiopathic Parkinson's disease (IPD) in their clinical presentation, underlying pathology, and response to treatment. APSs include conditions such as multiple system atrophy (MSA), progressive supranuclear palsy (PSP), corticobasal syndrome (CBS), and dementia with Lewy bodies (DLB). These disorders are characterized by a combination of parkinsonian features and additional symptoms, such as autonomic dysfunction, supranuclear gaze palsy, and asymmetric motor symptoms. Many
