Outcomes of low-weight patients with avoidant/restrictive food intake disorder and anorexia nervosa at long-term follow-up after treatment in a partial hospitalization program for eating disorders.

OBJECTIVE: To assess long-term outcomes of patients with avoidant/restrictive food intake disorder (ARFID) treated in a partial hospitalization program (PHP) for eating disorders (ED). METHOD: A cross-sectional study comparing patients with ARFID to those with anorexia nervosa (AN) who had been discharged from a PHP for at least 12 months was performed. Percent median body mass index (%MBMI), scores on the Children's Eating Attitudes Test (ChEAT), and treatment utilization were assessed, with intake and discharge data collected via retrospective chart review. RESULTS: Of the 137 eligible pat

Gastrointestinal symptomatology, diagnosis, and treatment history in patients with underweight avoidant/restrictive food intake disorder and anorexia nervosa: Impact on weight restoration in a meal-based behavioral treatment program.

OBJECTIVE: Gastrointestinal (GI) concerns are often presumed to complicate nutritional rehabilitation for restrictive eating disorders, yet their relationship to weight restoration outcomes is unclear. This retrospective chart review examined GI history and weight-related discharge outcomes in primarily adult, underweight inpatients with anorexia nervosa (AN, N = 107) or avoidant/restrictive food intake disorder (ARFID, N = 22) treated in a meal-based, behavioral eating disorder program. METHOD: Lifetime GI symptomatology, diagnoses, diagnostic tests, and procedures were abstracted from medic

Risk of avoidant/restrictive food intake disorders in patients with inflammatory bowel disease: a matched cross-sectional case-control study.

BACKGROUND: Individuals with inflammatory bowel disease (IBD) often modify their diet to manage symptoms; however, these behaviors may evolve into eating disorders, including avoidant/restrictive food intake disorder (ARFID). We assessed the risk of eating disorders and ARFID in patients with IBD compared with healthy controls (HC), explored differences by age at diagnosis, and examined associations with malnutrition and disability. METHODS: In this cross-sectional study, adult patients with confirmed IBD, stratified by pediatric- vs adult-onset, were matched with HC. ARFID risk was assessed

Screening of Avoidance/Restrictive Food Intake Disorder (ARFID) in Patients with Quiescent Inflammatory Bowel Disease and Irritable Bowel Syndrome-Like Symptoms.

PURPOSE: Even in the absence of active inflammation, patients with inflammatory bowel diseases (IBD) frequently report symptoms consistent with irritable bowel syndrome (IBS), which may lead to restrictive dietary patterns and ultimately increase the risk of avoidant/restrictive food intake disorder (ARFID). The aim was to evaluate the prevalence of patients at risk of ARFID among individuals with quiescent IBD and IBS-type symptoms, and to evaluate its association with the FODMAP intake. METHODS: A cross-sectional study was conducted on adult IBD patients followed in a tertiary Belgian cente

A comparison of food avoidant behaviours and sensory sensitivity in adults with and without Tourette syndrome.

Food selectivity has been shown to be more persistent and severe in children with Tourette syndrome (TS) compared to their typically developing peers. The current study aimed to examine differences in food selectivity, food neophobia and Avoidant Restrictive Food Intake Disorder (ARFID)-associated behaviours, between adults with and without TS. Fifty-three adults diagnosed with TS were compared to 53 neurotypical adults and completed the following measures online: Adult Eating Behaviour Questionnaire (AEBQ), Nine-Item Avoidant/Restrictive Food Intake disorder screen (NIAS), Food Neophobia Scal…

Validation study on the child, adult, and parent version of the ARFID module 2.0 for the Eating Disorder Examination.

OBJECTIVE: This study presents a psychometric evaluation of the avoidant/restrictive food intake disorder (ARFID) module 2.0 for the Eating Disorder Examination (EDE), its child (ChEDE), and parent version. Within a pediatric sample seeking treatment for restrictive feeding or eating disorders and non-treatment-seeking controls, the module's interrater reliability, parent-child agreement, and its convergent, divergent, and discriminant validity were examined. METHOD: The child, adult, and/or parent version of the German ARFID module was administered to N = 176 children and adolescents (0-17 y

Multiple Hypovitaminoses Presenting as Optic Disc Swelling in a Child with Autism Spectrum Disorder and Restrictive Eating.

Optic disc swelling, frequently associated with vitamin A toxicity, is infrequently linked to vitamin A deficiency. This report describes a 6-year-old male with autism spectrum disorder (ASD) and avoidant restrictive food intake disorder who presented with xerophthalmia, optic disc swelling, vision changes, and deficiencies in vitamins A, B1, and iron. The patient's behavioral dysregulation posed important challenges for the evaluation, diagnosis, and treatment of his hypovitaminoses. This case underscores the importance of considering multiple nutritional deficiencies as the etiology of optic…

Franz Kafka: the hunger artist.

Franz Kafka has often been posthumously diagnosed with anorexia nervosa, but recent research impels a re-evaluation of his disordered eating. This study presents a qualitative analysis of Kafka's diaries, correspondence, medical reports and fiction, focusing on appetite, diet, somatic symptoms, and cognitions concerning food and fasting. Diagnostic indicators are matched against DSM-5 criteria for anorexia nervosa and avoidant/restrictive food intake disorder. Kafka's clinical profile, which is characterised by diminished appetite, anorexia, selective intake, obsessive mastication, low body we

Medical Management of Restrictive Eating Disorders in Adolescents and Young Adults.

The medical provider plays an important role in the management of adolescents and young adults (AYAs) with restrictive eating disorders (EDs), including anorexia nervosa (AN), atypical anorexia nervosa, and avoidant/restrictive food intake disorder. The focus of this article is the medical management of AYAs with restrictive EDs, which can be performed by a number of different medical providers, including pediatricians, family physicians, internists, nurse practitioners, and, in some countries, psychiatrists. This position paper clarifies the role of the medical provider in diagnosing and mana

When Medically Required Food Avoidance Goes Awry: A Conceptual Framework of ARFID as an Underrecognized Clinical Complication of Food Allergy.

Pediatric patients with medical/developmental conditions face increased risk for avoidant/restrictive food intake disorder (ARFID). Diet-managed chronic illnesses may heighten risk for disordered eating, with emerging research finding medical symptoms predating disordered eating behaviors. Pediatric-onset immunoglobulin E-mediated food allergy ("food allergy") is on the rise, affecting an astonishing 8% of US children. The relationship between food allergy and ARFID is puzzlingly understudied despite food allergy clearly altering the individual's relationship with food, posing continuous dange

Effective Treatment Approaches for Eating Disorders in Children and Adolescents: A Review Article.

Eating disorders are serious psychiatric illnesses marked by disordered behaviors toward food and eating due to dissatisfactory body shape and weight, which impact the physical and psychological growth of children and adolescents. This review aims to recognize the effectiveness of psychotherapy and pharmacotherapy in treating eating disorders. The most common type of eating disorder is anorexia nervosa characterized by severe restriction of energy intake and an intense fear of gaining weight. Bulimia nervosa is characterized by episodes of binging and purging followed by compensatory behaviors

A causality dilemma: ARFID, malnutrition, psychosis, and hypomagnesemia.

We present a novel case of a woman with coincident occurrence of auditory and visual hallucinations, electrolyte disturbances, chloride unresponsive alkalosis, and an eating disorder. The patient was ultimately diagnosed with Gitelman syndrome comorbid with schizophreniform disorder and avoidant restrictive food intake disorder. Eating disorders are often associated with electrolyte abnormalities which, in turn, can cause or contribute to other neuropsychiatric symptoms. At the same time, psychotic disorders can lead to food intake aversions or overconsumption of fluids with associated effects…

Low-Dose Risperidone for an Autistic Child with Comorbid ARFID and Misophonia.

Avoidant/Restrictive Food Intake Disorder and misophonia seem to be overrepresented in autism spectrum disorder. Literature is mute on psychopharmacotherapy in these complex presentations. Here, authors report on a challenging case of low-functioning ASD child with comorbid ARFID and misophonia that responded favorably to a low-dose risperidone. This is followed by a brief discussion of purported pharmacodynamic mechanisms and relevant literature.
