PEEP Generated by High-Flow Nasal Cannula in a Pediatric Model.

BACKGROUND: High-flow nasal cannulas (HFNC) have been increasingly used in the pediatric critical care patient population. There are different theories about the mechanism by which HFNC reduces work of breathing, including diminishing upper airway dead space by the washout of carbon dioxide. However, one of the likely primary mechanisms by which HFNC reduces work of breathing is by generating PEEP. There are limited data assessing the PEEP delivered by moderate flows (8-50 L/min) of HFNC, which are used most commonly in pediatric patients. METHODS: Pediatric upper-airway models were created w…

Septic pulmonary emboli as a complication of peripheral venous cannula insertion.

Septic pulmonary emboli can occur as a complication of many diseases, most common being right sided infective endocarditis. Septic emboli through a peripheral venous cannula are rarely reported in literature though central venous catheter is commonly implicated. We describe a case of widespread cellulitis and septic pulmonary emboli as a complication of peripheral venous cannulation.

PaCO2 measurement in cerebral haemodynamics: face mask or nasal cannulae?

OBJECTIVE: PaCO2 affects cerebral blood flow (CBF) and its regulatory mechanisms, but the effects of CO2 measurement technique on cerebrovascular parameters are unknown. In order to determine if the two most commonly used approaches, face mask (FM) or nasal cannulae (NC), are interchangeable or not, we tested the hypothesis that the use of FM versus NC does not lead to significant differences in CO2-related systemic and cerebrovascular parameters. APPROACH: Recordings of CBF velocity (CBFV), blood pressure (BP), heart rate, and end-tidal CO2 (EtCO2) were performed in 42 subjects during normoc…

Extracorporeal membrane oxygenation cannula malposition in the azygos vein in an infant.

INTRODUCTION: Misplacement of extracorporeal membrane oxygenation (ECMO) venous cannula in the azygos vein has previously been described only in newborns. CASE: For the first time, we report an aberrant ECMO cannula placement in the azygos vein in a child outside neonatal period. History of chronic lung disease and ligation of persistent ductus arteriosus leading to elevated right ventricular pressure was the potential risk factor. CONCLUSIONS: This rare complication should be considered whenever the patient has inadequate venous return on ECMO and it can be easily ruled out with a lateral c…

Use of a chronic ureter cannula in the pig for the determination of renal clearance.

The surgical implantation of chronic ureter cannula to determine the renal clearance was evaluated using 24 pigs. The silicon tubing was surgically implanted into both ureters of each pigs. Two types of thick tubing (the inside diameter 1.0 or 2.5 mm and the outside diameter 4.0 mm) were used for these cannulas. The tubing was exposed out of the pig's body on the flank, on the hypogastric zone near the umbilicus, or near the groin. The following steps were effective to minimize the opportunity of a bacterial infection in the kidney and to maintain the functional integrity of the chronic ureter…

A new method for the artificial raising of infant rats: the palate cannula.

Chronic removal of infant rats from their mother prior to the onset of weaning is complicated by the fact that young rats do not easily suckle from an artificial nipple. Thus, a method of artificial raising is advantageous for developmental investigations of nutrition or ingestive behaviors during the suckling period. The intragastric cannula has become a popular method for this purpose. However, for many studies, it would be advantageous if the diet could be administered to the mouth and actually swallowed by the young rat. We developed a new cannulation procedure which accomplishes these goa…

Technique for trouble-free removal of pars plana infusion cannulas.

We describe a simple procedure that facilitates the removal of a pars plana cannula. A 2-cm 4-O black silk suture is passed under the double-throw knot, which secures the infusion cannula. When the ends of the silk suture are pulled, the double-throw knot opens easily, allowing the cannula to be removed.

Studies on the microbiological safety of the valved side-port of the 'Venflon' cannula.

A randomized prospective study was conducted to assess the effect of inserting a bacteria-retaining filter into the side-port of 'Venflon' cannulae. Observations were made on 109 cannulae with filters and 100 without. Although there was a trend towards a reduced rate of side-port colonization in the cannulae with filters this was not paralleled by any reduction in the incidence of local inflammation or colonization of the intravascular segment (IVS) of the cannula. Of the 37 cannulae with positive cultures from the skin at the cannula insertion site 22 yielded indistinguishable bacteria from t…

A new method for evaluating antitussives in cats using an electrode-cannula.

An emplaced laryngo-tracheal electrode-cannula was employed to induce and to measure cough in cats anesthetized with either sodium pentobarbital or Dial--urethane. Carbetapentane citrate, codeine sulfate and dextromethorphan hydrobromide were tested for antitussive action by this method. Relative antitussive potency obtained in order of decreasing effectiveness was dextromethorphan hydrobromide, codeine sulfate and carbetapentane citrate.

High-Flow Nasal Cannula in Subjects Undergoing Endoscopy With Intravenous Sedation.

BACKGROUND: High-flow nasal cannula (HFNC) is a noninvasive oxygenation method that delivers humidified and heated oxygen mixtures at high flows while generating PEEP. Therefore, its use during endoscopic procedures with intravenous deep sedation may help maintain airway patency and oxygenation. This study aims to compare the incidence of reductions in oxygen saturation in subjects receiving HFNC at different flows with that in subjects receiving conventional oxygen therapy during endoscopic retrograde cholangiopancreatography (ERCP). Secondary outcomes include the lowest observed SpO2, snorin…

Variable-Curvature Bending Characteristic and Kinematic Model of Cannula Flexible Needle.

BACKGROUND: Compared with the conventional bevel-tip flexible needles, the cannula flexible needle has a superiority in terms of agility. To improve the insertion accuracy of the cannula flexible needle in the soft tissue, the variable-curvature bending characteristics were explored and a kinematic model was developed. METHODS: This paper identified the parameters that influence the needle bending using a combination of the dimensional analysis theory and the equivalent stiffness method. Based on this, a kinematic model was established considering the deviation and rebound of the needle shaft…

[High flow oxygen via nasal cannula: Palliative care and ethical considerations].

High flow oxygen via nasal cannula (HFO2NC) has become the first-line reference symptomatic treatment for hypoxemic acute respiratory failure. This non-invasive technique can be addressed, as palliative therapeutic care, to frail patients near end-of-life with a do-not-intubate order. A distinction will be made between those with an imminent and inevitable fatal outcome (pallitative end-of-life management) and those with hope for transient clinical remission (meliorative management). This review focuses on the expected physiological benefits and technical benefits/risks incurred by HFO2NC use …

Adhered ECMO cannula in COVID-19 related severe acute respiratory failure.

BACKGROUND: Crescent cannula adhesion in the setting of COVID-19 respiratory failure requiring extracorporeal membrane oxygenation (ECMO) support is a novel complication. The objective of this case presentation is to highlight this rare complication and to explore potential predisposing factors and our management strategies. CASE PRESENTATION: We present the case of a 25 y.o. patient with COVID-19 respiratory failure requiring ECMO support for 16-days in which a 32 Fr crescent cannula became adherent to the SVC and proximal jugular vein. Attempts to remove the cannula at the bedside failed du…

A Dual-Lumen Percutaneous Cannula for Managing Refractory Right Ventricular Failure.

A right ventricular assist device (RVAD) using a dual-lumen percutaneous cannula inserted through the right internal jugular vein (IJV) might improve weaning in patients with refractory right ventricular (RV) failure. However, the reported experience with this cannula is limited. We reviewed the records of all patients receiving RVAD support with this new dual-lumen cannula at our institution between April 2017 and February 2019. We recorded data on weaning, mortality, and device-specific complications. We compared outcomes among three subgroups based on the indications for RVAD support (postc…

High-flow nasal cannula therapy: clinical practice in intensive care units.

BACKGROUND: Despite the extensive use of high-flow nasal cannula (HFNC) therapy in intensive care units (ICU) for acute respiratory failure (ARF), its daily clinical practice has not been assessed. We designed a regional survey in ICUs in North-west France to evaluate ICU physicians' clinical practice with HFNC. MATERIALS AND METHODS: We sent an observational survey to ICU physicians from 34 French ICUs over a 6-month period in 2016-2017. The survey included questions regarding the indications and expected efficiency of HFNC, practical aspects of use (initiation, weaning) and satisfaction. Co…

Physiological effects of high-flow nasal cannula therapy in preterm infants.

OBJECTIVE: High-flow nasal cannula (HFNC) therapy is increasingly used in preterm infants despite a paucity of physiological studies. We aimed to investigate the effects of HFNC on respiratory physiology. STUDY DESIGN: A prospective randomised crossover study was performed enrolling clinically stable preterm infants receiving either HFNC or nasal continuous positive airway pressure (nCPAP). Infants in three current weight groups were studied: <1000 g, 1000-1500 g and >1500 g. Infants were randomised to either first receive HFNC flows 8-2 L/min and then nCPAP 6 cm H2O or nCPAP first and then H…
