Successful graft replacement of the descending aorta after an extended reconstruction of the ascending and transverse aorta in a patient with Marfan's syndrome.

A 27-year-old man with Marfan's syndrome underwent a total aortic graft replacement in three separate stages. Initially the abdominal aorta was replaced, followed by the ascending aorta and aortic arch, and finally the residual portion. The extensive reconstruction of both the ascending and transverse aorta at the second operation, even though no dissection was present in the aortic arch, reduced the risk of the subsequent operation since the same surgical approach did not have to be used.

Acute dissection of the aorta in a kidney transplant patient.

Acute type IIIb dissection of the aorta was observed in a patient who had undergone kidney transplantation 5 years previously. Arteriograms showed that the true channel was compressed by the false channel, excluding the right common iliac artery to which the renal artery of the transplanted kidney had been anastomosed. The patient was treated successfully by fenestration of the abdominal aorta.

[Subtotal aortic replacement in a patient with Marfan's syndrome--replacement of the ascending aorta and entire aortic arch for acute type I aortic dissection 4 years after the thoracoabdominal replacement].

We report a patient with Marfan's syndrome who underwent replacement of the ascending aorta and the entire aortic arch for acute type I aortic dissection 4 years after the thoracoabdominal replacement. As a result, she underwent subtotal aortic replacement from the ascending aorta to the bilateral common iliac arteries in three stages. A 48-years-old female was admitted to our service because of acute heart failure and cardiogenic shock caused by acute DeBakey type I aortic dissection. She had undergone replacement of the descending thoracic aorta at the age of 44 and replacement of the remain

[Total aortic replacement in a patient with extensive thoracoabdominal aortic aneurysm--a case report].

A 35-year-old woman with a form fruste of Marfan's syndrome, presented the extensive thoracoabdominal aortic aneurysm. CT scan, MRI and aortogram revealed that she had a true aneurysm of ascending aorta and aortic arch and chronic DeBakey IIIb dissecting aneurysm extending throughout the descending thoracic and abdominal aorta. The surgical treatment was consisting of two staged operations. At the first operation, ascending aorta and aortic arch were reconstructed under cardiopulmonary bypass and selective cerebral perfusion. Elephant trunk method was employed for the second operation. The sec

Transcarotid Transcatheter Aortic Valve Replacement in an Octogenarian With Complex Type B Aortic Dissection and Aneurysm: A Case Report.

We report the case of an octogenarian with severe aortic valve stenosis, chronic kidney disease (CKD) and heart failure. Due to advanced CKD, we used a 3-dimensional transesophageal echocardiogram for sizing the device before transcatheter aortic valve replacement (TAVR). Noncontrast computed tomography found complex aortic dissection involving the arch, descending thoracic aorta, and abdominal aorta. TAVR was approached via the right carotid artery using a CoreValve. There was no cerebral vascular event. Renal function was well preserved. Transcarotid TAVR can be performed safely with complex

[Acute dissection of the descending aorta with ischemia of the limbs. Value of an axillofemoral bypass operation (author's transl)].

The authors describe a case of acute dissection of the descending aorta with continuation into the abdominal aorta, producing severe ischemia of the lower limbs and sensory-motor disturbances. An axillo-bi-femoral by-pass operation was performed leading to a revascularization syndrome with anuria and local edema. Good results were still apparent after one and a half years and the patient had started work again. Though a palliative operation, not treating the dissection, axillofemoral by-pass surgery appears to be indicated for distal ischemic lesions when the thoracic affection does not requir

Total Endovascular Aortic Repair in a Jehovah's Witness Due to Chronic Postdissection Aortic Aneurysm with Endovascular Aortic Septotomy with Electrosurgery.

A 63-year-old male Jehovah's Witness with a history of Stanford type B aortic dissection presented after a stable course over the years with a rapid expansion of the thoraco-abdominal aorta, necessitating intervention. Due to religious beliefs prohibiting blood transfusions, open surgical treatment of the aortic arch was not viable. Instead, endovascular aortic septotomy with electrosurgery of the abdominal aorta and endovascular repair of the aortic arch were successfully performed, followed by thoracic endovascular aortic repair to ensure sufficient lumen expansion for further prosthetic dep

Multiple vascular anomalies involving testicular, suprarenal arteries and lumbar veins.

Testicular arteries arise from the abdominal aorta and the inferior suprarenal artery from the renal artery. There are reports about variant origin and course of these arteries. Accessory testicular artery is also a common finding but its providing origin to inferior suprarenal artery is an important observation. During a routine dissection of abdomen of approximately 55-year-old male cadaver, unique vascular abnormality was observed. On the left side, a common arterial trunk originating from abdominal aorta immediately branched to give rise to superior testicular and inferior suprarenal arter

Midterm change of descending aortic false lumen after repair of acute type I dissection.

BACKGROUND: Persistent false lumen in the descending aorta after repair of acute type I dissection adversely affects long-term prognosis. In this study, we investigated changes of the descending aortic false lumen during the midterm postoperative period. METHODS: Postoperative computed tomographic (CT) images of 122 patients who underwent conventional ascending with or without arch replacement for acute type I dissection were reviewed. Patency and width of false lumen and maximal diameter of the aorta were compared between early and last follow-up images. Changes were analyzed separately in t

Aortoiliac dissection after blunt abdominal trauma: case report.

The distal abdominal aorta is rarely injured after blunt trauma but a direct blow to the abdomen from a seatbelt or handlebars may cause intimal dissection or rupture. We present the diagnosis and surgical management of aortoiliac dissection in a 16-year-old boy injured in a motorcycle accident. The technical aspects of vascular repair are emphasized.

Late Post-Dissection Dynamic Intermittent Malperfusion of the Aortic Arch in Association with a Rare Heterogenous LOX Gene Variation.

Late ischaemic consequences of type A aortic dissection are rare. We present a 6-year late complication of type A aortic dissection treated by Bentall surgery in a 41-year-old patient. The patient presented with several episodes of lipothymia associated with hypertensive attacks with anisotension, cervicalgia, hemicranial headache, abdominal pain and lower limb slipping initially on exertion and later at rest. On dynamic examination, we diagnosed an intermittent dynamic occlusion of the aortic arch and rare LOX gene variation, which is considered to be associated with aneurysm or dissection of

Multi-vessel spontaneous coronary artery dissection in a patient with aortic dissection: a case report.

BACKGROUND: Spontaneous coronary artery dissection (SCAD) is a rare cause of acute coronary syndrome that is often misdiagnosed. CASE SUMMARY: We describe a case of multi-vessel SCAD in a 73-year-old patient with no evidence of fibromuscular dysplasia that is presented with Type A aortic dissection after undergoing an ascending aorta and aortic arch replacement with stent placement in the abdominal aorta. The use of percutaneous coronary intervention with cutting balloons and drug-eluting stent implantation helped wean the patient off extracorporeal membrane oxygenation successfully. DISCUSS

Incorrect frozen elephant trunk deployment into the false lumen of a patient with complicated type B acute dissection.

Total arch replacement with frozen elephant trunk (FET) is used for type B acute aortic dissection with insufficient proximal landing zone. Herein, we report incorrect deployment of FET into the false lumen. A 45-year-old man underwent femorofemoral bypass for complicated type B acute aortic dissection. However, chest pain recurred 4 days postoperatively, and an oozing rupture of the dissecting descending aorta was diagnosed. As preoperative computed tomography showed insufficient proximal landing zone and separate chronic aortic dissection of the proximal arch, total arch replacement with FET

Acute Stanford Type A Aortic Dissection Mimicking Acalculous Cholecystitis: A Case Report.

Acute aortic dissection (AAD) is a time-critical cardiovascular emergency that classically presents with abrupt chest or back pain; however, atypical presentations may lead to misdiagnosis, delayed treatment, and worse outcomes. We report a case of AAD presenting with isolated abdominal pain and ultrasound findings mimicking acalculous cholecystitis. A 53-year-old male patient with poorly controlled hypertension and hypercholesterolemia presented with acute-onset epigastric and right upper quadrant pain associated with nausea. Lab evaluation showed direct bilirubin elevation and mild acute kid

Treatment of Retrograde Type B Aortic Dissection after Endovascular Repair of Abdominal Aortic Aneurysm.

Retrograde ascending aortic dissection during or after thoracic endovascular aortic repair is a nonfrequent complication (2.4%) especially in patients for type B aortic dissections with a high mortality rate (42%). This complication can occur not only during the index hospitalization but also after discharge up to 3 years. However, retrograde type B aortic dissection is much less common after standard endovascular repair for infrarenal abdominal aortic aneurysm. We present a 76-year-old female patient with a history of ascending aorta aneurysm and obesity who was referred to our clinic with re

Misdiagnosed mid-back pain by Stanford type B aortic dissection masquerading as musculoskeletal origin.

Aortic dissection is a life-threatening condition that often presents with acute chest pain; Misdiagnosis can lead to delayed management and potentially fatal outcomes. This case report aims to highlight a rare instance involving a 40-year-old male patient who initially sought care at a spine clinic for his severe mid-back pain (MBP). His condition had worsened over the past 24 h, ultimately leading to a diagnosis of Stanford type B aortic dissection. The initial evaluation included physical examination and imaging studies focused on the musculoskeletal origin of the pain. However, as the pati

[Modern diagnostic concepts in dissection and aortic occlusion].

For the diagnostic work-up of the aorta, non-invasive cross-sectional imaging techniques have almost replaced invasive catheter angiography. CT- and MR-angiography are supplemented by sonography which is used predominantly for the assessment of abdominal aortic aneurysm and dissections of the thoracic aorta. This review deals with the diagnostic approach to two disease entities involving the aorta: aortic dissection and aortic occlusion. Transoesophageal echocardiography (TEE), CT- and MR-angiography (MRA) are used in the assessment of aortic dissection. Published sensitivity and specificity v

Seeing the invisible: painless aortic dissection in the emergency setting.

Acute dissection of the aorta can be one of the most dramatic cardiovascular emergencies. Classically, aortic dissection presents as sudden, severe chest, back, or abdominal pain that is characterised as ripping or tearing in nature. However, a timely diagnosis can be elusive in the event of an atypical presentation. In this report, the authors present two patients with painless aortic dissection who were misdiagnosed during their initial evaluation in the emergency department.

A Case of an Abdominal Aortic Dissection in a Hemodynamically Stable Marfan Syndrome Patient Presenting without Pain.

Introduction. Marfan syndrome (MFS) is a rare connective tissue disorder attributed to a defect in the fibrillin-1 gene. Aortic aneurysms and dissection are common causes of morbidity and mortality in Marfan syndrome. Case Report. A 43-year-old female with a history of MFS and a 4.0 cm dilated ascending aorta presented to her cardiologist reporting that since a C-section two years prior, the left side of her abdomen painlessly protruded when standing. An outpatient CT scan of the abdomen/pelvis noted a 5.5 cm abdominal aortic dissection, and she was directed to the hospital. Repeat CT scan of
