Early diagnosis and treatment of acute heart failure in prehospital and emergency settings. Part 1 of the International Expert Opinion Series on acute heart failure management.

Acute heart failure (AHF) is diagnosed in about 0.5% of all patients seen by emergency medical systems (EMS) and represents about 1% of emergency department (ED) visits. Leg swelling and shortness of breath are the most frequent patient complaints. Despite significant advancements in patient care pathways, the proper diagnosis, treatment and disposition of AHF may be further improved in emergency settings. The present document is an expert consensus document outlining key points in diagnosis, treatment and decision-making of patients being diagnosed with AHF by EMS and in the ED. Pillars of co

Beyond the blood: A practical guide to thalassemia care in the emergency department.

β-thalassemia is a hereditary hemoglobinopathy characterized by ineffective erythropoiesis, chronic anemia, and iron overload. Improved survival rates due to advances in transfusion and iron chelation therapies have shifted the clinical burden toward managing complex, often acute, complications that commonly present to the emergency department (ED). This review provides an evidence-based, system-oriented framework for emergency physicians to recognize and manage β-thalassemia-related emergencies. Initial ED assessment should focus on hemodynamic stability, transfusion history, iron chelation r

Low-dose ketamine for acute pain: A narrative review.

INTRODUCTION: Acute pain management is a critical component of prehospital and emergency medical care. Opioids are effective; however, the risks and side-effects of opioids have led providers to use low-dose ketamine (LDK) for safe and effective treatment of acute pain. METHODS: We conducted a scoping narrative review to explore the efficacy of LDK for the treatment of acute pain in the prehospital setting and emergency department (ED) setting. The prehospital review includes studies evaluating the use of LDK in both civilian and military settings. We utilized PubMed to identify prospective a

Emergency department care of patients with Duchenne muscular dystrophy.

Patients with Duchenne muscular dystrophy are living longer and are increasingly seen in Emergency Departments. Though the most common cause of death remains progressive respiratory failure, increased life expectancies have unmasked the significance of progressive myocardial dysfunction, now associated with nearly 40% of mortalities in the DMD population. Cardiac complications such as arrhythmias and cardiomyopathy are becoming ever more widely recognized. Emergency physicians may encounter DMD patients with untreated, undiagnosed or worsening of known heart disease. This review will initially

Understanding the association between frequent emergency department use and jail incarceration: A cross-sectional analysis.

BACKGROUND: Frequent emergency department (ED) use and incarceration can be driven by underlying structural factors and social needs. If frequent ED users are at increased risk for incarceration, ED-based interventions could be developed to mitigate this risk. The objective of this study was to determine whether frequent ED use is associated with incarceration. METHODS: We conducted a retrospective cross-sectional study of 46,752 individuals in San Francisco Department of Public Health's interagency, integrated Coordinated Care Management System (CCMS) during fiscal year 2018-2019. The primar

[Emergency care as a high-risk workplace-Measures to deal with violence against employees].

BACKGROUND: Many employees in the field of emergency care experience verbal and physical violence caused by patients or visitors. The aim of this project is to gain insights into which measures are available to deal with violence in emergency care and how employees subjectively evaluate them. METHODS: A nationwide cross-sectional online survey was conducted in 2020. The questionnaire contained items on violence prevention, intervention, and follow-up measures. Quantitative data were analysed descriptively and free text data according to Mayring's qualitative content analysis. RESULTS: A tota

Validity and reliability of the Interagency Integrated Triage Tool in a regional emergency department in Papua New Guinea.

OBJECTIVE: The Interagency Integrated Triage Tool (IITT) is a novel, three-tier triage system recommended by the World Health Organization. The present study sought to assess the validity and reliability of a pilot version of the tool in a resource-limited ED in regional Papua New Guinea. METHODS: This pragmatic prospective observational study, conducted at Mount Hagen Provincial Hospital, commenced 1 month after IITT implementation. The facility did not have a pre-existing triage system. All ED patients presenting within a 5-month period were included. The primary outcome was sensitivity for

The FAST-FURO study: effect of very early administration of intravenous furosemide in the prehospital setting to patients with acute heart failure attending the emergency department.

AIMS: The effect of early administration of intravenous (IV) furosemide in the emergency department (ED) on short-term outcomes of acute heart failure (AHF) patients remains controversial, with one recent Japanese study reporting a decrease of in-hospital mortality and one Korean study reporting a lack of clinical benefit. Both studies excluded patients receiving prehospital IV furosemide and only included patients requiring hospitalization. To assess the impact on short-term outcomes of early IV furosemide administration by emergency medical services (EMS) before patient arrival to the ED. M

[Violence in the Emergency Medicine (Emergency Rescue Service and Emergency Departments) - Current Situation in Germany].

BACKGROUND: During the last years healthcare personnel is more often faced with different degrees of violence and aggression while doing their jobs. However, systematic data with respect to offender profiles, reasons, frequency and types of violence in emergency medicine in Germany has not been systematically analyzed. AIM: The aim of this article is to overview the data of present German studies. RESULTS: Only a few German studies could be included. Data suggest that violence in emergency medicine is an underreported but ubiquitous, persistent problem. 90% of the participants in the surveye

Shock.

Critically ill patients with undifferentiated shock are complex and challenging cases in the ED. A systematic approach to assessment and management is essential to prevent unnecessary morbidity and mortality. The simplified, systematic approach described in this article focuses on determining the presence of problems with cardiac function (the pump), intravascular volume (the tank), or systemic vascular resistance (the pipes). With this approach, the emergency physician can detect life-threatening conditions and implement time-sensitive therapy.

Emergency Department Behavioral Health Observation and Stabilization.

Emergency department visits for pediatric mental health crises have risen sharply in recent years, while inpatient psychiatric bed capacity has declined, resulting in prolonged patient boarding. This article describes the development and implementation of behavioral health observation (BHO) models to address this gap in care. BHO provides time-limited, intensive psychiatric assessment and crisis stabilization with a multidisciplinary team focused on symptom reduction, diagnostic clarification, and structured disposition planning for patients. The article outlines patient selection, interdiscip

Management of supraventricular tachycardia in the emergency department.

While supraventricular tachycardia is a common condition and seldom leads to serious consequences, it can be challenging to treat. Optimal management requires an accurate diagnosis, especially in the distinction between regular and irregular tachycardias; as well as an understanding of the mechanism responsible for the arrhythmia. In this article, the acute management of supraventricular tachycardias as they present in the emergency department is discussed, based on the underlying electrophysiological principles of arrhythmias.

The Cardiac Direct Access Clinic: Improving Urgent Access while Reducing Unnecessary Emergency Department and Inpatient Utilization.

Emergency department (ED) crowding, constrained inpatient capacity, and long waits for outpatient cardiology delay care for patients with urgent cardiac symptoms. In 2016, the Beth Israel Deaconess Medical Center opened a non-ED-based, cardiologist-staffed Cardiac Direct Access Clinic with examination rooms, an infusion room, and six overnight observation beds to provide rapid specialty evaluation and short-stay care. Using administrative data and contribution-margin analyses, the authors assessed operational, clinical, and financial outcomes and summarized implementation strategies. Of 11,121

The usage of the Boussignac continuous positive airway pressure system in acute respiratory failure.

Traditionally, continuous positive airway pressure (CPAP) and bilevel positive airway pressure (BiPAP) devices have been used to treat patients in acute respiratory failure. However they require an electric power source, are relatively large in size, and may be difficult to use in prehospital settings. The recently introduced Boussignac CPAP system is capable of delivering 10 cmH2O of CPAP, is compact, portable and requires only an oxygen source. This paper reviews the efficacy of using Boussignac CPAP as a treatment for acute respiratory failure in both prehospital and hospital settings. All

Common features of rare disease patients in the emergency department: a systematised literature review.

BACKGROUND: Patients with rare diseases (RD) may present to the emergency department (ED) with an acute manifestation of an undiagnosed RD. If not identified, some of these patients may suffer from increased morbidity, mortality and worse outcomes compared to patients with more common diseases. Unfortunately, patients with RD often present with common symptoms, e.g. abdominal pain, making it even more difficult for ED physicians to identify those patients. Thus, strategies are needed to determine when certain RD should be suspected in the ED. We aimed to identify common features of RD through

A battle for attention: how do emergency physicians respond to interruptions? A scoping review.

OBJECTIVES: Managing constant interruptions is an intrinsic aspect of emergency medicine practice; however, effective physician responses to mitigate their impact remain unclear. Although interruptions are exceedingly common and closely linked to adverse patient outcomes, the approaches for managing such disruptions have yet to be established. In this study, we aimed to identify how emergency physicians and residents respond to interruptions and the factors influencing their responses. METHODS: We conducted a scoping review to identify how emergency physicians and residents manage interruptio

Code Malignant Hyperthermia: Malignant Hyperthermia in the Emergency Department-Implications for Nursing.

Malignant hyperthermia is a rare, life-threatening hypermetabolic reaction that can strike unexpectedly in the emergency department, demanding swift recognition and intervention to save lives. This syndrome can be fatal if not treated and occurrs in 1 in 100,000 adults and 1 in 30,000 children, with a 3% to 5% mortality rate. Although often associated with perioperative environments, malignant hyperthermia should remain on the radar for emergency nurses, especially when patients exhibit sudden hyperthermia, muscle rigidity, and tachycardia after intubation or procedural sedation.
