OBJECTIVES: This study sought to assess the performance of cardiovascular magnetic resonance (CMR) myocardial perfusion mapping against invasive coronary physiology reference standards for detecting coronary artery disease (CAD, defined by fractional flow reserve [FFR] ≤0.80), microvascular dysfunction (MVD) (defined by index of microcirculatory resistance [IMR] ≥25) and the ability to differentiate between the two.
BACKGROUND: Differentiation of epicardial (CAD) and MVD in patients with stable angina remains challenging. Automated in-line CMR perfusion mapping enables quantification of myoca

We encountered a 77-year-old woman who had a roof-dependent macro-reentrant left atrial (LA) flutter (LAFL), masked by probable epicardial conduction via the septopulmonary bundle (SPB). The SPB, an epicardial connection in the LA, may contribute to the maintenance and complexity of LAFL. Thus, when evaluating LAFL, physicians should consider the potential involvement of the SPB. Detailed observation using a three-dimensional mapping and comprehensive electrophysiological pacing maneuvers is crucial for identifying the re-entry circuit of LAFL.

Recent developments in imaging, mapping, and ablation techniques have shown that the epicardial region of the heart is a key player in the occurrence of ventricular arrhythmic events in several cardiac diseases, such as Brugada syndrome, arrhythmogenic cardiomyopathy, or dilated cardiomyopathy. At the atrial level as well, the epicardial region has emerged as an important determinant of the substrate of atrial fibrillation, pointing to common underlying pathophysiological mechanisms. Alteration in the gradient of repolarization between myocardial layers favouring the occurrence of re-entry cir

We report a case of a malignant right free wall AP that showed very rapid conduction during atrial fibrillation. The accessory pathway was attached to from the right atrial appendage to the right ventricle. Catheter ablation from an endocardial approach was met with limited success. Epicardial mapping of the atrial insertion site was achieved via a subxiphoid pericardial puncture. Long-term success was noted after ablation was performed in the epicardial space at the site of the tightest VA interval.

BACKGROUND: His bundle pacing (HBP) is an alternative to biventricular pacing (BVP) for delivering cardiac resynchronization therapy (CRT) in patients with heart failure and left bundle branch block (LBBB). It is not known whether ventricular activation times and patterns achieved by HBP are equivalent to intact conduction systems and not all patients with LBBB are resynchronized by HBP.
OBJECTIVE: To compare activation times and patterns of His-CRT with BVP-CRT, LBBB and intact conduction systems.
METHODS: In patients with LBBB, noninvasive epicardial mapping (ECG imaging) was performed dur…

Coronary venous mapping and ablation can be an effective strategy in targeting ventricular arrhythmias that arise from intramural or epicardial sites of origin. We discuss the case of a patient with ischemic cardiomyopathy referred to our center for index ventricular tachycardia ablation after receiving multiple shocks from his implantable cardioverter-defibrillator who underwent coronary venous mapping and ablation as an adjunct to endocardial ventricular tachycardia ablation.

Pulmonary vein isolation (PVI) is used for rhythm control in atrial fibrillation (AF). Posterior wall isolation (PWI) is often an adjunct to PVI. Successful PWI is limited by esophageal location, epicardial bridging signals, tissue thickness, and mapping catheter resolution. High-density grid mapping catheters can assist with PWI. Here, we report a case of a 71-year-old woman with persistent AF who underwent PVI and PWI with high-density grid mapping catheters, thus demonstrating the use of omnipolar technology in facilitating successful PWI.

BACKGROUND AND AIM: Epicardial approach to VT ablation increases the success rate of ablation but is not without complications. We studied the safety and efficacy of epicardial VT ablations performed at our institute.
METHODS: All patients who underwent epicardial VT ablation at our institute were studied retrospectively. The outcome of VT ablation was among three groups: ischaemic cardiomyopathy (ICM), non-ischaemic cardiomyopathy (NICM) and granulomatous myocarditis (GM). Safety outcomes assessed included all complications considered to be due to pericardial access or epicardial mapping/abl…

Epicardial electrophysiological procedures rely on dependable interfacing with the myocardial tissue. For example, epicardial pacing systems must generate sustainable chronic pacing capture, while epicardial ablations must effectively deliver energy to the target hyper-excitable myocytes. The human heart has a significant adipose layer which may impede epicardial procedures. The objective of this study was to quantitatively assess the relative location of epicardial adipose on the human heart, to define locations where epicardial therapies might be performed successfully. We studied perfusion-

OBJECTIVES: This study aimed to evaluate the feasibility and accuracy of using a novel grid mapping catheter during scar-related ventricular tachycardia (VT) ablation.
BACKGROUND: Ultra-high-density (UHD) mapping improves identification of local abnormal ventricular activities (LAVAs) and characterization of scar substrates.
METHODS: Consecutive patients underwent endocardial and/or epicardial ablation guided by a HD grid mapping catheter. A linear duodecapolar catheter was used in the initial cases for systematic correlation. Isochronal late activation mapping was performed during sinus rhy

Here we report a novel electroanatomic mapping method to determine intramural activation, which we call electro-tomographic mapping. Based on our previous experiments, which demonstrated that multipolar electrograms can resolve near-field activity with little contamination in the x/y-plane but with a confocal view in the z axis, we designed a strategy for identifying activation from a focal source deep to the mapping surface through a combination of annotating the earliest -dV/dtmax and identification of a multipolar QS signal. We hypothesized that this strategy would be of use to map the true

BACKGROUND: The subxiphoid pericardial access is technically difficult and has a considerable rate of complications, thus transatrial access may be an alternative.
OBJECTIVES: This study sought to assess the feasibility and safety of this strategy regarding periprocedural period and after 1-week follow-up.
METHODS: The investigators performed epicardial mapping through transatrial puncture in 20 swine. Animals were divided into group A, in which aspiration of the sheath was performed to maintain negative pressure after the withdraw of the catheters, and group B, in which a device (Konar-MF V…

OBJECTIVES: This study sought to assess the association between electrocardiographic imaging (ECGI) parameters and voltage from simultaneous electroanatomic mapping (EAM).
BACKGROUND: ECGI offers noninvasive assessment of electrophysiologic features relevant for mapping ventricular arrhythmia and its substrate, but the accuracy of ECGI in the delineation of scar is unclear.
METHODS: Sixteen patients with structural heart disease underwent simultaneous ECGI (CardioInsight, Medtronic) and contact EAM (CARTO, Biosense-Webster) during ventricular tachycardia catheter ablation, with 7 mapped epic

Accessory pathways (APs) associated with the coronary sinus (CS), often linked to diverticula, are rare and challenging to ablate, frequently requiring CS venography for anatomical guidance. We report the case of a 56-year-old woman with recurrent palpitations, manifest pre-excitation, and two prior failed ablations for a presumed posteroseptal pathway. The electrocardiogram showed an immediately negative QRS complex in the inferior leads, suggesting an epicardial CS-related pathway. BACKGROUND: Ventricular tachycardia (VT) catheter ablation success may be limited when transcutaneous epicardial access is contraindicated. Surgical ablation (SurgAbl) is an option, but ablation guidance is limited without simultaneously acquired electrophysiological data.
OBJECTIVE: We describe our SurgAbl experience utilizing contemporary electroanatomic mapping (EAM) among patients with refractory VT storm.
METHODS: Consecutive patients with recurrent VT despite antiarrhythmic drugs (AADs) and prior ablation, for whom percutaneous epicardial access was contraindicated, underwent open Surg

BACKGROUND AND OBJECTIVES: Macroreentrant atrial tachycardias often occur following atrial fibrillation ablation, most commonly due to nontransmural lesions in prior ablation lines. Perimitral atrial flutter is one such arrhythmia which requires ablation of the mitral isthmus. Our objectives were to review the literature regarding ablation of the mitral isthmus and to provide our approach for assessment of mitral isthmus block.
METHODS: We review anatomical considerations, ablation strategies, and assessment of conduction block across the mitral isthmus, which is subject to several pitfalls.
