AIM: Stercoral perforation is a rarely suspected life-threatening condition. Early diagnosis is difficult but essential. A comprehensive systematic review was performed to evaluate its presentation, diagnosis and treatment.
METHOD: A systematic review was carried out of Embase, MEDLINE, PubMed and Cochrane databases for all articles published between 1998 and 2011. Only studies describing stercoral perforation were included.

Despite improvements in diagnostic techniques including the use of radionuclide imaging, locating the site of severe rectal bleeding is not always possible. We present two patients in whom a survey film of the abdomen showed a dense accumulation of stool in the transverse colon. Subsequent evaluation confirmed that the source of rectal bleeding was distal to this area. We present successful treatment of a patient with tubular colonic duplication complicated by fecal impaction, perforation and fecal peritonitis. This anomaly is usually detected in children younger 2 years old. In adulthood, this diagnosis is of a precedent-setting nature. If the diagnosis was not confirmed in early childhood, the absence of typical clinical picture, long-term course of disease and difficult interpretation of clinical data complicate subsequent verification of congenital anomaly. Only infectious complications and emergency surgery in adults can make a correct diagnosis.

A 25-year-old male with cerebral palsy presented with sigmoid volvulus complicated by ischemia and fecal impaction-an underrecognized but serious risk in this population. Emergency Paul-Mikulicz colostomy achieved safe resection. Clinicians should suspect volvulus early in CP patients, and proactive bowel management is essential.

Sigmoid volvulus was diagnosed in two patients with progressive system sclerosis. In one case, this was initially confused with a high fecal impaction. Both cases responded to early surgical intervention. Sigmoid volvulus should be considered in the differential diagnosis of intestinal obstruction associated with progressive systemic sclerosis.

Faecal incontinence in the elderly is not compatible with normal social function and leads to interventions and/or institutionalization. In homes for the elderly about 10% and in nursing homes 42.8% of the patients are faecally incontinent. There are predisposing, precipitating and uncleanliness factors. Four ways of faecal loss are described. The purposes of diagnosis and therapy are: 1. prevent and cure diseases, which cause unintentional faecal loss, 2. maintain lavatory visiting as long as possible and 3. prevent soiling the old patient himself and his surroundings.

Stercoral colitis is a rare inflammatory colitis caused by the accumulation of impacted fecal material. Obstruction of the colon caused by a fecalith is not a rare condition, but endoscopic attempts at removal of the fecalith are often unsuccessful because of the size of the fecalith and its extremely hard stone-like consistency. Isolated acute typhlitis caused by a caecal faecolith is extremely rare. Hereby we report such a case. A 30-year-old man presented clinically as acute appendicitis. During open surgery, an inflamed caecal mass was found. A limited colectomy including the mass and grossly health margins was performed. Histopathological examination confirmed that acute typhlitis was caused by an impacted caecal faecolith. The appendix was normal. Faecolith is an extremely rare cause of acute inflammation of the caecum presenting as a mass which is usually treated by limited right hemicolectomy.

Small bowel obstruction is one of the common conditions presenting in surgical wards, however fecalith is one of the rare causes of bowel obstruction. We present here a case of 65 years old lady, who presented with sub-acute intestinal obstruction. In spite of the initial diagnosis being sigmoid volvulus, exploratory laparotomy revealed a focolith in ileum which was retrieved through an enterotomy and primary closure was done. The patient recovered uneventfully. Thus emphasizing the need of through history and workup which steer us to the correct diagnosis.

This report describes the fecalith-induced intestinal obstruction of a free-ranging red diamond rattlesnake (Crotalus ruber) and the snake's subsequent history following surgical removal of the fecalith. The captured snake exhibited an abnormally distended abdomen and an extremely hard mass, detected via palpation, near its vent. Coeliotomy yielded a 2.5-cm, 5-g fecalith from the large intestine.
