[Research progress of femoral neck system in treatment of femoral neck fracture in young and middle-aged patients].

OBJECTIVE: To summarize the research progress of femoral neck system (FNS) in the treatment of femoral neck fracture in young and middle-aged patients. METHODS: The literature on FNS at home and abroad in recent years was extensively reviewed, and the results of mechanical and clinical studies on FNS were summarized based on clinical experience. RESULTS: FNS has good mechanical stability, which can reduce complications such as femoral neck shortening, internal fixation failure, and varus caused by mechanical instability. At present, FNS is mainly selected for comparison with cannulated compr

Effect of Postoperative Weightbearing on Intracapsular Femoral Neck Fractures Treated with The Femoral Neck System: A Comparative Cohort Study.

BACKGROUND: Postoperative weightbearing management of patients surgically treated for femoral neck fractures is controversial. This study aimed to compare the outcomes of patients treated with a fixed-angle implant, the Femoral Neck System (FNS), as a function of their postoperative weightbearing protocol. METHODS: A comparative cohort study of patients with intracapsular femoral neck fractures who underwent fixation with the FNS at a Level I trauma center from 2019 to 2023 was conducted. Patients were assigned to either the weightbearing as tolerated (WBAT) group or the toe-touch weightbeari

Femoral neck locking plate versus multiple cannulated screws for femoral neck fractures in young adults: a randomized controlled trial.

BACKGROUND: Managing femoral neck fractures (FNFs) in young adults remains a significant clinical dilemma. No single internal fixation method has demonstrated clear superiority. The aim of this study was to compare the clinical and radiographic outcomes of FNFs in young adults treated with femoral neck locking plate (FNLP) or conventional partially-threaded 6.5 mm multiple cannulated cancellous screws (MCCS). METHODS: A randomized controlled clinical trial (RCT) study was conducted on 74 patients to assess FNLP and MCCS in management of FNFs in young adults in Sohag university hospital betwee

A Modern Collared Cementless Femoral Stem for the Arthroplasty Treatment of Femoral Neck Fractures.

Background/Objectives: For femoral neck fractures (FNFs) treated with hip arthroplasty, cemented femoral fixation is frequently recommended due to its association with reduced early perioperative fracture and reoperation rates. However, newer-generation collared, cementless triple-tapered (CCTT) stems may present benefits compared with conventional press-fit designs. This study sought to assess 30-day survivorship of a CCTT stem in patients undergoing hip arthroplasty for FNF via the direct anterior approach (DAA). Methods: We conducted a retrospective review of all patients who underwent hemi

Improve biomechanical stability using intramedullary nails with femoral neck protection in femoral shaft fractures.

BACKGROUND AND OBJECTIVE: Elderly patients treated for femoral shaft fractures have a higher risk of hip fracture. We hypothesized that intramedullary nails protecting the femoral neck can improve mechanical strength and reduce the risk of subsequent hip fracture. This study aims to analyze the biomechanical stability using intramedullary nails with or without femoral neck protection through finite element analysis. METHODS: Thirty finite element models (FEMs) were established, including five different conditions of femoral shaft fracture: Fracture healing, Proximal fractures (Transverse and

Comparison between femoral block and PENG block in femoral neck fractures: A cohort study.

BACKGROUND: Regional analgesia is worth performing in the multimodal postoperative management of hip fracture (HF) because it reduces hospital morbidity and mortality. The aim of this study is to compare the efficacy and side effects of the recently described "Pericapsular Nerve Group (PENG) Block" with those of the femoral block, which is considered the standard of care for postoperative pain control after femoral neck fracture. MATERIALS AND METHODS: We conducted a comparative observational study at a university hospital (Saint Antoine Hospital, Sorbonne University, Paris, France), where th

Risk Factors for Avascular Necrosis of the Femoral Head in Pediatric Femoral Neck Fractures.

Introduction A neck of femur fracture is a rare injury in the pediatric population and is of foremost importance, as it is associated with a high rate of complications. It usually occurs due to high-velocity trauma or a fall from a height. There is a scarcity of data on risk factors and their role in the prognostication of avascular necrosis. The purpose of the study was to retrospectively analyze the association of various risk factors with avascular necrosis (AVN) of the femoral head in patients with a neck of femur fracture in the pediatric age group. Material and methods The study included

Total hip arthroplasty for acute femoral neck fractures using a cementless tapered femoral stem.

The purpose of this study was to evaluate the osseointegration potential and implant-related complications of cementless total hip arthroplasty with a titanium alloy collarless, tapered, wedge-shaped femoral stem with a proximal circumferential plasma-spray coating in patients with acute hip fractures. The cohort consists of 85 patients with a mean age of 78.1 years. The mean duration of follow-up was 3.8 years. Total hip arthroplasty conferred significant improvement in function for all patients. All femoral components were stable with evidence of bone ingrowth (84 hips) or fibrous fixation (

Ballistic femoral neck fractures: Current concepts in evaluation and management.

Ballistic femoral neck fractures are complex, high-energy injuries that require a management approach distinct from conventional blunt trauma. These current concepts review examines the evolving landscape of evaluation and treatment for these rare injuries based on contemporary literature. Initial management emphasizes the Advanced Trauma Life Support (ATLS) protocol, with a critical focus on identifying associated neurovascular and skeletal injuries, which are present in approximately one-third of cases. Diagnostic evaluation relies on plain radiography and thin-slice CT to delineate fracture

Femoral Neck Fractures: How to Avoid Failures and Manage Problems.

Femoral neck fractures are common injuries sustained by patients of all ages and are caused by both low- and high-energy mechanisms. The physician must diagnose the fracture expeditiously to initiate treatment. The surgeon must then evaluate the patient's medical condition and fracture characteristics to determine a treatment regimen that will minimize the all-too-common risks of osteonecrosis and fixation failure or fracture nonunion. In elderly, frail, and inactive patients, hip arthroplasty is usually the best treatment. In younger patients, efforts should be directed at expedient reduction

Direct anterior approach for femoral neck fractures: Why and how?

Femoral neck fractures are among the most common orthopedic injuries, often managed using closed reduction internal fixation (CRIF), hemiarthroplasty, or total hip arthroplasty (THA). While several approaches have been utilized in the surgical management of femoral neck fractures, the direct anterior approach (DAA) has emerged as a popular approach for hip arthroplasty given its smaller incision, muscle sparing technique, and robust outcomes. Several studies and meta-analyses have demonstrated that the DAA has fewer complications, dislocations, and better early functional outcomes compared to

Primary Hemiarthroplasty for the Treatment of Basicervical Femoral Neck Fractures.

Purpose Basicervical femoral neck fractures are uncommon injuries that occur at the extracapsular base of the femoral neck at its transition with the intertrochanteric line. Controversy remains in the orthopedic literature as to the optimal method of treatment for this fracture type given the inherent instability and greater rate of implant failure with traditional fixation constructs. The purpose of this study is to quantify the incidence and preferred treatment methods of basicervical hip fractures at a single, regional, Level 1 trauma center and to identify differences in postoperative comp

Neglected femoral neck fractures in cerebral palsy: a narrative review.

Hip fractures are severe conditions with a high morbidity and mortality, especially when the diagnosis is delayed, and if formulated over 30 days after the injury, is termed a 'neglected femoral neck fracture' (NFNF).Cerebral palsy (CP) is probably one of the major risk factors for NFNF in Western countries, mainly because of both cognitive and motor impairments. However, considering the high prevalence of fractures in these patients, the incidence of NFNF in this population is probably underestimated, and this condition might result in persistent hip or abdominal pain.Several techniques are a
