Proton pump inhibitors (PPIs) are widely used medicines worldwide. However, a rare etiology of syndrome of inappropriate secretion of antidiuretic hormone (SIADH) related to PPI was recently reported. Therefore, the putative role of PPIs in SIADH cannot be underestimated. A 78-year-old Japanese woman was admitted to our hospital for treatment of left Bell's palsy. On admission, the patient was oriented with normal laboratory data, including a serum Na level of 135 mEq/L. Oral glucocorticoids and a proton pump inhibitor were initiated in combination with oral valaciclovir. Six days later, the p

BACKGROUND: Primary adrenal insufficiency (PAI) in children is an uncommon but severe condition which can be either inherited or acquired. It consists in clinical manifestation of defective production or ineffective action of endogenous glucocorticoids; deficiency in mineralocorticoids and adrenal androgens may coexist. Diagnosis of PAI in children and young people can be challenging; while adrenal crisis (acute decompensation) is a life-threatening condition, with patient presenting with characteristic features of hypoglycemia, hypotension, collapse and coma, chronic adrenal insufficiency may

INTRODUCTION: In Europe body purification and natural balance restoring rituals are becoming increasingly popular, but an introduction of Amazonian shamanic rituals in urban Europe can result in unexpected adverse events.
CASE REPORT: A 44-year-old woman attended a Kambô or Sapo ritual in Slovenia where dried skin secretion from a giant leaf frog (Phyllomedusa bicolor) was applied to five freshly burned wounds at her shoulder. Afterwards, she drank 6 litres of water and gradually developed nausea and vomiting, confusion, lethargy, muscle weakness, spasms and cramps, seizure, decreased conscio

Syndrome of inappropriate secretion of antidiuretic hormone (SIADH) is recognised to occur after cardiac and traditional open abdominal surgery. However, SIADH after laparoscopic surgery is not well documented in the literature. We report a case of SIADH after laparoscopic (totally extraperitoneal) inguinal hernia repair in an elderly man.

The term cerebral salt wasting (CSW) was introduced before the syndrome of inappropriate antidiuretic hormone secretion was described in 1957. Subsequently, CSW virtually vanished, only to reappear a quarter century later in the neurosurgical literature. A valid diagnosis of CSW requires evidence of inappropriate urinary salt losses and reduced "effective arterial blood volume." With no gold standard, the reported measures of volume depletion do not stand scrutiny. We cannot tell the difference between CSW and the syndrome of inappropriate antidiuretic hormone secretion. Furthermore, the disti

A 22-year-old woman was admitted to intensive care with severe hyponatraemia. She suffered from lower abdominal pain, vomiting and irritability since one week. Physical findings showed euvolemia and an altered mental status with severe agitation and slurred speech. Abdominal examination was painful but there were no signs of peritonitis. Laboratory data were compatible with the diagnosis of syndrome of inappropriate secretion of antidiuretic hormone. Since patient was in a premenstrual phase, recently started to take an oral contraceptive and since no abnormalities were seen on an abdominal CT

Pituitary tumors are common and are often associated with endocrine abnormalities. Furthermore, pituitary surgery itself may result in additional hormonal changes, including impairment of anterior pituitary hormone secretion and, more commonly, abnormalities of ADH regulation. Endocrine management of patients with pituitary or other sellar lesions involves acute hospital-based and longer term office-based evaluation and treatment. In the immediate postoperative period, careful attention must be directed toward sodium and water balance as well as toward recognition of changes in endocrine funct

OBJECTIVE: To report a case of possible fluoxetine-induced syndrome of inappropriate antidiuretic hormone secretion (SIADH).
CASE SUMMARY: A 92-year-old patient was prescribed oral fluoxetine 20 mg daily for depression. After 13 days of therapy, she developed severe weakness and was found to have hyponatremia. Fluoxetine was discontinued. Treatment included fluid restriction and sodium chloride and potassium supplementation. Eight days after admission, the hyponatremia resolved and the patient was discharged without any pharmacologic treatment for depression.
DISCUSSION: Case reports on fluo

The Syndrome of Inappropriate Secretion of Antidiuretic Hormone (ADH) or SIADH has been reported in a great variety of diseases and disorders of the central nervous system. However, to our knowledge only three cases [Kaplan et al., Decaux et al. 1981, Agus et al. 1983] of SIADH associated with systemic lupus erythematosus (SLE) have been described in the literature. We report here a new case of SIADH in association with SLE.

This is a report of a patient who developed symptomatic hyponatraemia during chlorpropamide therapy for diabetes mellitus. The patient's clinical and biochemical abnormalities were corrected by withdrawal of chlorpropamide. This represents a drug induced, reversible form of the syndrome of inappropriate secretion of antidiuretic hormone (SIADH).

Renal function was assessed in 270 patients with brucellosis. In 58 consecutive patients of this total, plasma and urinary osmolality and urinary sodium excretion as well as adrenal and thyroid functions were measured prospectively. Results of these measurements satisfy criteria for the syndrome of inappropriate secretion of antidiuretic hormone (SIADH). Such inappropriate secretion explains the asymptomatic hyponatraemia observed in our patients. The hyponatraemia together with the usual haematological findings and cellular derangement of the liver may be useful in the differential diagnosis

This article has presented the complex system by which the hypothalamus regulates body fluid balance. In summary, ADH is synthesized and released via the hypothalamohypophyseal system. The supraoptic nucleus in the hypothalamus produces the ADH and the neurohypophysis stores and releases it. Osmoreceptors in the hypothalamus sense minute changes in the extracellular osmolality and stimulate or inhibit ADH synthesis and secretion. At the same time the thirst center of the hypothalamus is stimulated by the extracellular osmolality and brings conscious awareness of thirst into play. Once ADH is s
