Upper-lip vermilion reconstruction with a modified cross-lip vermilion flap in hemifacial atrophy.

Lip-vermilion reconstruction is part of the comprehensive treatment of progressive hemifacial atrophy. In this study, we described a two-stage reconstruction procedure to address the defect of the upper lip in patients with hemifacial atrophy, using a modified cross-lip vermilion flap with the pedicle specifically positioned at the commissure for functional and aesthetic purposes. During the first stage of surgery, a pre-determined mucosal flap from the lower lip with a vascular pedicle containing an inferior labial artery and the surrounding orbicularis oris muscle cuff were raised and transp…

Fetal Cleft Lip and Palate.

Fetal cleft lip and cleft palate are among the most common craniofacial anomalies, affecting approximately 1 in 1,000 live births worldwide. Cleft lip/cleft palate is caused by a combination of genetic and environmental factors and requires prompt diagnosis and lifelong multidisciplinary care for adequate treatment of anatomic and psychosocial challenges that extend well beyond surgical procedures in infancy. Cleft lip/cleft palate is a complex anomaly present from the first trimester onward that has prenatal and postnatal considerations. Diagnosis of cleft lip/cleft palate is most common in t…

Bilateral Cleft Lip: Primary Repair.

Primary bilateral cleft lip repair is among the most complex procedures in cleft surgery. Presurgical infant orthopedics address the prominent premaxilla and align alveolar segments for an optimal primary repair. Principles established by Mulliken have greatly increased the quality of bilateral cleft lip repair. These enduring principles emphasize symmetry, synchronous nasolabial repair, orbicularis continuity, proper philtral flap design, and median tubercle construction with lateral lip elements.

Lip Reconstruction Following Cancer Resection.

BACKGROUND: An analysis of all possible lip defects and a comparison of outcomes following various reconstructive techniques is lacking in the literature. METHODS: An analysis of lip reconstructions following cancer resection from 1994 to 2024 was performed. RESULTS: A total of 221 patients underwent lip reconstruction. Tobacco use (p = 0.003), diabetes mellitus (p = 0.042), and concurrent mandible or maxillary reconstruction (p = 0.019) were associated with surgical complications. Recurrent disease (p = 0.011) and oral commissure resection (p = 0.020) were associated with oral incompetence.…

Functional reconstruction of lip defects.

The lip is a crucial structure in the oral and maxillofacial region, serving vital physiological functions such as speech, swallowing, chewing, and expression. Due to the complexity of lip anatomy, function, and the various types of defects, the functional restoration and reconstruction of lip defects remain complex and challenging tasks. In this article, we summarize several methods for functional restoration and reconstruction of lip defects using local flaps that carry the depressor anguli oris muscle, as well as some free flaps. We also introduce methods for repairing extensive defects in …

Rare Congenital Upper Lip Pit.

Congenital lip pits are characterized by sinuses or fistulas in the lips that can occur in isolation or as part of a genetic disorder. A 6-year-old girl with a right upper lip lesion present at birth presented with recurrent swelling and occasional erythema. Examination revealed a mildly swollen punctum at the right upper wet/dry vermillion with expressible serous drainage. There were no other phenotypic or cognitive concerns. The lesion was surgically excised using vertical wedge resection. The postoperative course showed well well-healed incision. The pathology report confirmed a lip pit. Th…

Dog Attack Causing Lip Wound.

Animal bites are relatively common occurrences reaching about 4.5 millions of people every year. The main aggressor is the domesticated dog, responsible for around 90% of the patients, with children being the most affected, with 70% of the registered patients, while with adults that number is a lot lower (15%). Bites around the head and neck require special attention. Due to the presence of noble structures and the rich local vascularization, any wounds have been immediately addressed to stop bleeding and further complications. The present study shows a woman patient, victim of a dog bite in h

Surgical treatment of cleft lip.

We performed a systematic review of the literature on the surgical treatment of cleft lip, emphasizing the prevalence, complications associated with the treatment and the points of disagreement between authors. We conducted a literature cross-sectional search that analyzed publications in books, articles and on the databases SciELO - Scientific Electronic Library Online, PubMed, of the National Center for Biotechnology Information. We conclude that: 1) the severity of the cleft will indicate the technique presenting more advantages; 2) the different approaches indicate that there is no consens…

[Lip synechiae after erythema multiforme].

BACKGROUND: Mucosal erosions in bullous diseases and leading to mucosal sequelae are widely described in toxic epidermal necrolysis (TEN). These complications cause disfigurement and functional impairment. They are more rarely reported in erythema multiforme (EM). We report a case of lip adhesion following EM induced by Mycoplasma pneumoniae. PATIENT AND METHODS: A 12-year-old boy was hospitalized in a paediatric intensive care unit. He had a prominent target skin rash on the palms and soles. Mucosal injury was associated with conjunctivitis, balanitis without dysuria and hyperalgesic stomati…

Mild incomplete cleft lip repair.

Of all the methods for repair of the unilateral cleft lip, none has gained as much popularity as the rotation advancement. Limitation of the technique leads to various modifications. It is well known that different cleft lip patients have different deformities. However, the modifications are always focused on nasal symmetry and alar base position. We found out that some microform incomplete cleft lip patients have distinctive appearance, of which the cleft located on the lower upper lip, nasal deformity is mild, and distinctive appearance presents a terrace before the peak on cleft side where …

Adenomatoid hyperplasia of lower lip.

Adenomatoid hyperplasia (AH) is an uncommon, non-neoplastic swelling on the palate caused due to hyperplasia of the mucinous acini. The lesion clinically presents as a sessile tumor-like nodule resembling pleomorphic adenoma. Histopathologic findings include lobules of enlarged mucinous acini which are filled with secretory granules. The nuclei are squeezed to the basal portions, associated with focal inflammation and ductal dilatation, and a history of trauma is often elicited. Here, we report a rare case of AH of the lower lip in a 20-year-old male patient, which mimics a mucous retention cy…
