Effect of tidal volume and anesthetic agent on airway responsiveness to histamine.

Dose-response relationships for bronchoconstriction in response to aerosal histamine were assessed before and after vagotomy in 11 dogs anesthetized with barbiturates and in 9 dogs anesthetized with alpha-chloralose-urethan. The dose-response relationships following vagotomy were assessed during spontaneous ventilation and during muscular paralysis and mechanical ventilation with tidal volume (VT) similar to each animal's VT prior to vagotomy. After vagotomy the spontaneous VT of both groups increased but the VT of the alpha-chloralose-urethan group was significantly less than that of the barb

Effects of changes in tidal volume on avian intrapulmonary chemoreceptor discharge.

It has been suggested that avian intrapulmonary CO2-sensitive receptors (IPC) may be capable of monitoring the rate and extent of CO2 washout from the lung during spontaneous breathing. The purpose of this study was to analyse IPC discharge activity (using computerised bin-averaging and counting techniques) in spontaneously breathing domestic fowl when VT was elevated from resting levels. This was accomplished by administration of almitrine (2 mg X kg-1 i.v.), a respiratory stimulant drug that has been shown to have a specific long-lasting stimulatory action on carotid body chemoreceptors. The mechanism of arterial hypoxia occurring in acute pancreatitis.

Respiratory Function has been measured in 14 patients with acute pancreatitis, none of whom has a previous history of cardiac or respiratory disease. Their mean age was 40 years, mean PaO2 on admission, 11 kPa, and all had normal chest radiographs at that time. Arterial hypoxia was a common development and the mean PaO2 at the time of most severe hypoxia was 8.6 kPa with the lowest individual recording 4.9 kPa. In addition to daily arterial blood gas monitoring, dynamic ventilatory tests and measurements of lung volume, transfer factor, closing volume, right to left shunts and studies of gas e

Hyperinflation and its management in COPD.

Chronic obstructive pulmonary disease (COPD) is characterized by poorly reversible airflow limitation. The pathological hallmarks of COPD are inflammation of the peripheral airways and destruction of lung parenchyma or emphysema. The functional consequences of these abnormalities are expiratory airflow limitation and dynamic hyperinflation, which then increase the elastic load of the respiratory system and decrease the performance of the respiratory muscles. These pathophysiologic features contribute significantly to the development of dyspnea, exercise intolerance and ventilatory failure. Sev

Pilot study of closing volume in byssinosis.

A study of the relative sensitivities of forced expiratory volume in one second (FEV1), maximal mid-expiratory flow (MMF), and closing volume (CV) in the detection of subjects with byssinosis was carried out in a North Carolina cotton mill. Altogether 35 workers participated in the study. Of these, nine showed a decline in FEV1 of 10% or more during the first work shift that followed the weekend break. Twelve subjects showed a decrease in MMF of 15% or more. In contrast only six workers exhibited a 10% increase in closing capacity, while ten showed a 10% increase in CV. Recent evidence of the

Elastic properties of the centrilobular emphysematous space.

Bronchograms were performed using finely particulate lead on emphysematous lungs obtained at necropsy. X-ray films were taken of these lungs at distending pressures of 0, 5, 10, and 20 cm H(2)O. The volumes of individual centrilobular emphysematous spaces were calculated at each distending pressure from measurements made on these bronchograms and pressure-volume curves were constructed for each space. The pressure-volume characteristics of seven normal lungs and one lung with centrilobular emphysema was also measured. The normal lungs, the lung with centrilobular emphysema, and the centrilobul

The influence of wetsuit thickness (≥ 7 mm) on lung volumes in scuba divers.

INTRODUCTION: We hypothesised that although thicker (≥ 7 mm) wetsuits delay hypothermia and allow divers to dive in cooler waters, they may hinder pulmonary function. The aim of this study was to investigate whether thicker wetsuits worn by Tasmanian divers affected lung volumes, primarily the forced vital capacity (FVC) and forced expiratory volume, one second (FEV1). METHODS: Sixty-two volunteer active divers were recruited from recreational dive clubs and Tasmania's occupational diving industry. After confirming fitness and that the divers were currently active, spirometry testing was perf

The association between regional transcriptome profiles and lung volumes in response to mechanical ventilation and lung injury.

BACKGROUND: Lung inhomogeneity plays a pivotal role in the development of ventilator-induced lung injury (VILI), particularly in the context of pre-existing lung injury. The mechanisms that underlie this interaction are poorly understood. We aimed to elucidate the regional transcriptomic response to mechanical ventilation (MV), with or without pre-existing lung injury, and link this to the regional lung volume response to MV. METHODS: Adult female BALB/c mice were randomly assigned into one of four groups: Saline, MV, lipopolysaccharide (LPS) or LPS/MV. Lung volumes (tidal volume, Vt; end-exp

Expiratory effects of brief carotid sinus nerve and carotid body stimulations.

The expiratory response, measured directly as ventilatory volume and as internal intercostal or abdominal muscle EMG activity, to brief intermittent electrical stimulation of the carotid sinus nerve or chemical stimulation of the carotid body was determined in anesthetized cats. Stimuli given during expiration had a specific expiratory effect, manifested by increased expiratory duration, increased activity of expiratory muscles and increased depth of expiration. The later in expiration the stimulus was exhibited, the greater all effects became. Vagal section had no effect on the response and t

Assessment of lung volume by 3-dimensional sonography and magnetic resonance imaging in fetuses with congenital diaphragmatic hernias.

OBJECTIVES: The purpose of this study was to evaluate the influence of different rotation angles in assessment of the contralateral lung volume by 3-dimensional (3D) sonography in comparison to magnetic resonance imaging (MRI) in fetuses with congenital diaphragmatic hernias. METHODS: A total of 126 measurements by 3D sonography and MRI were conducted in 81 patients between 18 and 39 weeks' gestation. The 3D sonographic volumes of the contralateral fetal lung were calculated by the rotational technique (virtual organ computer-aided analysis) with rotation angles of 6°, 15°, and 30°. Transvers

Quantification of MRI T2 Interstitial Lung Disease Signal-Intensity Volume in Idiopathic Pulmonary Fibrosis: A Pilot Study.

BACKGROUND: Imaging has played a pivotal role in the diagnosis of idiopathic pulmonary fibrosis (IPF). Recent reports suggest that T2 -weighted MRI could be sensitive to monitor signal-intensity modifications of the lung parenchyma, which may relate to the disease activity in IPF. However, there is a lack of automated tools to reproducibly quantify the extent of the disease, especially using MRI. PURPOSE: To assess the feasibility of T2 interstitial lung disease signal-intensity volume quantification using a semiautomated method in IPF. STUDY TYPE: Single center, retrospective. POPULATION:

Airway reactivity to methacholine in nonatopic asymptomatic adults.

We studied 50 nonsmoking volunteers, ages 18-35 yr, with no past or present history or physical examination findings of asthma, rhinitis, allergic disease, or recent respiratory infections, to evaluate the usefulness of the methacholine bronchoprovocation challenge (MBPC) as a screening test for asthma. All were skin-test-negative to 29 aeroallergens and had base-line pulmonary function values greater than 80% predicted. Fourteen (28%) subjects had a drop in forced expiratory volume in 1 s (FEV1) of 20% or greater at a provocative dose (PD20FEV1) less than or equal to 225 breath units. Moreove

THE LUNG VOLUME IN HEART DISEASE.

The lung volumes in a group of individuals suffering from chronic cardiac disease have been studied by a method which is applicable to patients suffering from dyspnea. In a number of instances the same patients were investigated during various stages of decompensation and compensation. The values found have been compared with those determined in a group of normal subjects. Lung volumes have been considered from three points of view: (1) relative lung volumes or subdivisions of total lung volume expressed as percentage of total lung volume; (2) the absolute lung volumes of patients with heart d

Improvements of the shape and strength of the diaphragm after endoscopic lung volume reduction.

RATIONALE: Endoscopic lung volume reduction improves lung function, quality of life and exercise capacity in severe emphysema patients. However, its effect on the diaphragm function is not well understood. We hypothesised that endoscopic lung volume reduction increases its strength by modifying its shape. OBJECTIVES: To investigate changes in both diaphragm shape and strength induced by the insertion of endobronchial valves. METHODS: In 19 patients, both the diaphragm shape and strength were investigated respectively by 3D Slicer software applied on CT scans acquired at functional residual c

Bedside Assessment of PEEP-Induced Volume and Mechanical Power in Mechanically Ventilated Adults Without ARDS: A Post Hoc Analysis.

BACKGROUND/OBJECTIVES: Positive end-expiratory pressure (PEEP) increases end-expiratory lung volume through the addition of PEEP-induced lung volume (PEEPVol), potentially affecting respiratory mechanics and energy load. However, its physiological impact in patients without lung injury remains poorly characterized. We hypothesized that PEEPVol would be closely associated with respiratory system loading beyond conventional respiratory mechanics variables. METHODS: We conducted a secondary analysis of a prospective physiological study including 16 deeply sedated, mechanically ventilated adults

Assessment of Hydration Status in Peritoneal Dialysis Patients: Validity, Prognostic Value, Strengths, and Limitations of Available Techniques.

BACKGROUND: The majority of patients undergoing peritoneal dialysis (PD) suffer from volume overload and this overhydration is associated with increased mortality. Thus, optimal assessment of volume status in PD is an issue of paramount importance. Patient symptoms and physical signs are often unreliable indexes of true hydration status. SUMMARY: Over the past decades, a quest for a valid, reproducible, and easily applicable technique to assess hydration status is taking place. Among existing techniques, inferior vena cava diameter measurements with echocardiography and natriuretic peptides s
