[Notes on some self-induced diseases in prison communities].

The self-induced and simulated diseases in prison communities are numerous and not always easy to detect. This is, at any rate, a peculiar phenomenon that also involves other closed communities external to the prison environment, such as those of the large built-up areas, of the factories, of the schools, of the barracks, etc... As a rule, recourse is had, in the latter, to self-induction and simulation to achieve economic or absenteeistic purposes. In prison communities, self-induction and simulation are instead implemented in order to escape the rigours of the law and detention itself. It is

Biopsychosocial interplay of personal injury insurance, legal frameworks, and functional neurological disorder: an emerging perspective on recovery and disability determinants.

Functional Neurological Disorder (FND) is a disabling neuropsychiatric condition marked by motor/sensory symptoms not explained by neurologi … his review journeys through historical perceptions of FND in this setting, from its conflation with malingering to its eventual distinction from feigning, before examining how features of compensation environments, such as their legal and administrative structures, are etiologically relevant to FND. Badge of Courage: Pain and Suffering After Military Service in a Nonveteran.

Feigning and fabrication by a patient can evoke anger and distress in staff and generate diagnosis and management challenges. When faced with difficulties that can lead to moral injury among health care providers, and even risk compromised patient care, clinical teams must balance empathy with appropriate boundaries. This case report examines an individual with a history of polysubstance use and acute-on-chronic pain whose persistent patterns of fabrication-spanning exaggerated military service, terminal illness, family trauma, and academic accomplishments-posed substantial risk-assessment and

Classification accuracy and resistance to coaching of the Spanish version of the Inventory of Problems-29 and the Inventory of Problems-Memory: A simulation study with mTBI patients.

Objective: The present study aims to evaluate the classification accuracy and resistance to coaching of the Inventory of Problems-29 (IOP-29) and the IOP-Memory (IOP-M) with a Spanish sample of patients diagnosed with mild traumatic brain injury (mTBI) and healthy participants instructed to feign. Method: Using a simulation design, 37 outpatients with mTBI (clinical control group) and 213 non-clinical instructed feigners under several coaching conditions completed the Spanish versions of the IOP-29, IOP-M, Structured Inventory of Malingered Symptomatology, and Rivermead Post Concussion Symptom

Evaluating the performance of the MMPI-3 over-reporting scales: Sophisticated simulators and the effects of comorbid conditions.

OBJECTIVE: We examined the utility of the Minnesota Multiphasic Personality Inventory-3 (MMPI-3) to detect feigned over-reporting using a symptom-based coaching simulation design across a control group and three diagnostic conditions: posttraumatic stress disorder (PTSD), minor traumatic brain injury (mTBI), and comorbid PTSD and mTBI. METHOD: Participants were310 college students who wererandomly assigned to one of the four conditions. For participants in the feigning conditions, they were provided with a descriptionof their respective disorder condition throughout the duration of the sessio

Specificity and item endorsement rates of personality assessment inventory over-reporting scales across ethnic, gender, and diagnostic groups in a forensic inpatient sample.

The accurate evaluation of response style, particularly with respect to overreporting, is imperative in forensic settings wherein an externa … tem endorsement and configural elevations on the Negative Impression Management (NIM) scale and the Malingering Index (MAL) were associated with genuine psychopathology, results indicated that the overall specificity estimates across groups were generally adequate. Simulated paraphilias: a preliminary study of patients who imitate or exaggerate paraphilic symptoms and behaviors.

In a consecutive series of admissions to the Johns Hopkins Sexual Disorders Unit, 4 out of 20 patients appeared to have simulated paraphilic symptoms that further assessment indicated were either exaggerated or not present. The paper presents case histories of these 4 patients. A descriptive comparison is made between these patients and control groups of patients who admitted having paraphilic symptoms and a group of patients accused of having paraphilic symptoms but who denied them. Patients who simulated paraphilias tended to be self-referred (75%) and without current legal charges (100%). N

[Social medicine aspects of persistent somatoform pain disorder].

Persistent somatoform pain disorder is a mental disease the symptom of which--namely, physical pain--cannot be satisfactorily explained by physical findings. This is a frequently occurring disease. But it is often not identified or at least not recognised at an early stage. This leads to negative effects on the patient (e.g. chronification) and on the national social welfare system (for example, costs can mount up to thousands of million DM for medical and social welfare measures). To reduce these problems as far as possible, it is imperative to recognise the disease at an early stage. In part

Objective determination of dark adaptation: accuracy of the evoked potential method.

The psychophysical threshold and the appearance of the visually evoked potential (VEP) were compared in the dark adapted state. Nineteen normal subjects and 11 persons with defective dark adaptation were studied. The VEP threshold was an average of 0.2 log units (standard deviation 0.2) above the subjective threshold in the normal group, and 0.4 log units above it in the night-blind subjects. In 7 normal subjects, the alpha-activity of the electroencephalogram interfered with the VEP recording and rendered the evaluation more difficult. The results indicate that the VEP can be used as an accur

Delayed recovery in the patient with a work compensable injury.

Under certain circumstances patients who are receiving compensation for an injury will have a disproportionate disability and delayed recovery because of reinforcers provided by the accident. Successful treatment depends on early recognition that the primary problem is emotional rather than due to concurrent physical problems. A few general rules can be helpful in the diagnosis and treatment of this type of patient. A thorough psychosocial history should be taken, and return to work as soon as possible is essential. With awareness of this process, the physician can work with the company, the t

[Functional cognitive disorders: How to diagnose and manage them? Discussion based on a clinical case].

Functional cognitive disorder (FCD) is a frequent and underrecognized cause of cognitive complaints. Long regarded as a diagnosis of exclusi … ive symptoms related to depressive disorder, neurodegenerative diseases at the prodromal stage, and malingering. Diagnosing FCD requires a comprehensive patient evaluation, integrating a qualitative analysis of the interview and the patient's conversational profile alongside the assessment of symptoms, physical examination, and neuropsychological testing.

The English-language version of the Self-Report Symptom Inventory: a pilot analogue study with feigned head injury sequelae.

Questionnaire-based symptom validity tests (SVTs) are an indispensable diagnostic tool for evaluating the credibility of patients' claimed symptomatology, both in forensic and in clinical assessment contexts. In 2019, the comprehensive professional manual of a new SVT, the Self-Report Symptom Inventory (SRSI), was published in German. Its English-language version was first tested in the UK. This experimental analogue study investigated 20 adults simulating minor head injury symptoms and 21 honestly responding participants. The effect sizes of differences between the two groups were large, with

Headache subsequent to whiplash.

Methodology varies greatly in whiplash studies; therefore, results are not directly comparable. Headache seems to be present in 50% to more than 75% of cases in the acute stage, and in 20% to 30% of cases in the early chronic stage. Headache naturally occurs frequently (> 75% of the cases ) in patients who consult headache specialists due to protracted symptoms. Malingering may explain some cases, and continuation/activation of prewhiplash headache may explain other cases. De novo headache also seems to occur. This headache may partly have cervicogenic headache characteristics: side-locked uni…

[Posttraumatic headache. Pathophysiology, clinical, diagnostic and therapeutic aspects].

Posttraumatic headache (PTHA) is, usually, one of several symptoms of the posttraumatic syndrome and therefore may be accompanied by somatic, psychological or cognitive disturbances. The aetiology of these symptoms in individuals with mild traumatic brain injury or whiplash injury has been a subject of some controversy with explanations ranging from neural damage to malingering. PTHA can resemble a tension-type headache, migrainous or cervicogenic headaches. Post-whiplash headache habitually is a pain radiating from the neck to the forehead, with moderate intensity and benign, but prolonged co…

Fitness-to-work considerations in the paradigmatic pain condition of headache disorder.

Headache disorders are common, including in the working population. Clinicians caring for patients with headache need to be aware of work-re … lications of associated medical or psychiatric conditions, and the potential of symptom feigning or malingering for secondary gain. As clinicians often struggle with fitness-to-work determinations, a structured approach to fitness-to-work assessments in headache conditions and other pain conditions would improve clarity for clinicians and increase the quality …
