Midface advancement by Le Fort III osteotomy is a common procedure in craniofacial surgery. However, little data exist concerning the effect of midface advancement on mandibular growth. This is a retrospective study of 38 patients from two craniofacial centers who had Le Fort III osteotomy. The aims of this investigation were to document the size and shape of the mandible in Crouzon and Apert syndromes and to determine the effect on these parameters of downward and forward movement of the midface. The syndrome patients had increased gonial angle, increased MP-SN, increased ramus height, and in

INTRODUCTION: Bilateral sagittal split osteotomy of the mandible (BSSO) is the most common orthognathic surgery procedure. The use of bone grafting seems widely used whereas its interest has not been shown to date. The aim of this study was to describe the current state of practice of bone graft in BSSO in France.
MATERIALS AND METHODS: An online survey was used to collect data about the use of bone grafting in BSSO procedures among French maxillofacial surgeons.
RESULTS: One hundred and forty-eight surgeons responded to the survey, mostly maxillo-facial surgeons (96 %). Only 31.8 % of parti

This study evaluated in vitro three internal fixation methods for short bilateral sagittal split osteotomy (BSSO) of the mandibular ramus using polyurethane replicas. After simulating an 8-mm mandible advancement with a counterclockwise rotation of 6° to the occlusal plane, the short BSSO was fixed bilaterally by using: (i) three linear bicortical screws; (ii) one miniplate with four monocortical screws and one bicortical screw (hybrid system); and (iii) two miniplates with eight monocortical screws. The load was measured on the central incisors or first molars bilaterally at the displacements

PURPOSE: In mandibular reconstructive surgery with free fibula flap, 3D-printed patient-specific cutting guides are the current state of the art. Although these guides enable accurate transfer of the virtual surgical plan to the operating room, disadvantages include long waiting times until surgery and the inability to change the virtual plan intraoperatively in case of tumor growth. Alternatively, (electromagnetic) surgical navigation combined with a non-patient-specific cutting guide could be used, requiring accurate image-to-patient registration. In this phantom study, we evaluated the accu

OBJECTIVES: The accelerated tooth movement phenomenon after orthognathic surgery has been observed. However, the underlying mechanism remains unclear. There is no experimental study showing the effect of orthognathic surgery on orthodontic tooth movement of the opposing jaw. Therefore, the present study aimed at investigating if mandibular osteotomy enhances maxillary tooth movement and bone remodelling.
MATERIALS AND METHODS: Fifty-four male Sprague-Dawley rats were randomly divided into two groups: maxillary tooth movement (TM) and maxillary tooth movement + mandibular surgery (TM + MS). Th

This report describes osteotomy and iliac bone graft for malunion caused by failed mandibular fracture reduction. A 27-year-old man was referred 3 months after a motor vehicle accident. At another hospital, two operations had been performed for symphyseal fracture using two resorbable plates. Malocclusion was noted, and panoramic radiography and computed tomography revealed a misaligned dental arch, with a 9.37-mm gap between the central and the lateral incisor of the left mandible. A wafer was made from the patient's dental model, and a maxillary arch bar was applied. Through a lower gingivol

UNLABELLED: The bilateral sagittal split osteotomy is considered the standard surgery to correct facial asymmetries. More recently, unilateral sagittal split osteotomy (USSO) was used to treat such malocclusions.
AIM: To assess facial symmetry following USSO in the treatment of class III laterognathia.
METHODS: Frontal facial photographs of four groups of patients were assessed: (1) pre-surgical group (n = 30) with skeletal asymmetry, (2) postsurgical group assessing patients of the first group 2 years after USSO, (3) control group (n = 30) of patients judged to have harmonious facial norms,

INTRODUCTION: Several surgical procedures have been proposed for the treatment of hyperdivergent dentoskeletal deformities. We propose a new osteotomy technique allowing for lengthening and advancement of the mandibular ramus by intra-oral approach.
SURGICAL PROCEDURE: This technique differs from the conventional sagittal split osteotomyin that which the anterior osteotomy line is not continued until the basilar edge but stopped 5-6mm above it. Cutting of the pterygomasseteric sling is systematically done allowing for the lowering of the mandibular angle. Osteosynthesis is performed by transj

Severe dentofacial deformities require both orthodontics and surgical management to repair. Modern mandibular orthognathic surgery commonly uses sagittal split ramus osteotomy (SSRO) and intraoral vertical ramus osteotomy (IVRO) methods to treat patients. However, complications like neurosensory disturbances and temporomandibular joint disorders are common following both procedures. In 1992, Choung introduced the intraoral vertico-sagittal ramus osteotomy (IVSRO) which led to a decrease in postoperative complications. The 'straight' IVSRO or Choung's type II osteotomy has a 'condylotomy' effec

PURPOSE: This study compared the number of unfavorable fractures after sagittal split osteotomies (SSOs) of the mandible when third molars were present or absent.
PATIENTS AND METHODS: The treatment records of 262 patients (500 SSOs) were retrospectively evaluated. Group 1 consisted of 250 SSOs and concomitant removal of impacted mandibular third molars and group 2 consisted of 250 SSOs with absence of third molars. A modified SSO technique with inferior border cuts was used on all patients, and the third molars, when present, were removed after separation of the proximal and distal segments.

This study evaluated bone healing after mandibular swing osteotomies fixed with biodegradable poly(L-lactide) (PLLA) bone plates in four patients. A step osteotomy treated with two PLLA bone plates (n=3), and a straight osteotomy treated with one PLLA bone plate (n=1) were performed. Bone healing was uneventful in all patients; only in the patient with the straight osteotomy was callus observed. After 5.5 years, radiologic changes at the site of implantation were observed in the remaining patient. Histologic examination revealed a nonspecific foreign-body reaction on highly crystalline PLLA re

The composite resection operation with sacrifice of a portion of the hemimandible is considered to be the fundamental operation for cancers in the posterior oral cavity. The mandible is resected for cancer control (to ease reconstruction) and perhaps for traditional reasons. Mandibular sacrifice is not always essential for oncologic resection. Access through the mandible is usually required for effective resection of cancers in this region. The lateral mandibular osteotomy approach provides this access in selected patients. The adjacent, remaining portion of the tongue provides 75 to 100 sq cm

Sagittal split ramus osteotomy is a fundamental procedure in orthognathic surgery. Precise identification of the mandibular foramen is critical for achieving optimal splitting patterns and minimizing neurosensory deficits. Preoperative cone-beam computed tomography assessment is a current method for foramen identification; however, radiographic images do not represent the intraoperative anatomical conditions. Visual access interference and anatomical constraints from the internal oblique ridge make foramen identification challenging. To address this unmet need, we developed a specialized instr

There has yet to be a consensus on the appropriate technique for mandibular distraction in the infant Pierre Robin population nor is their sufficient data on sensory nerve outcomes. The purpose of this study is to validate the safety of the horizontal osteotomy by: (1) determining mandibular foramen location in infant Pierre Robin patients relative to the dentoalveolar plane and (2) evaluate long-term function of the inferior alveolar nerve in previously distracted Pierre Robin patients. Preoperative 3D Computed tomography (CT) scans of Pierre Robin patients < 1 year old were reviewed. A line

PURPOSE: Bone defects of the inferior mandibular border (osseous inferior border defects) can cause unesthetic postoperative outcomes after bilateral sagittal split osteotomy (BSSO). The aim of this study was to estimate the frequency of osseous inferior border defects after BSSO and to identify risk factors for this complication.
MATERIALS AND METHODS: This retrospective study included consecutive patients who underwent BSSO for mandibular retrognathia. The primary outcome was the presence/absence of osseous inferior border defects. Predictors included the mandibular movement, rotation of th

PURPOSE: The effect of the type of nerve injury on subjective sensory disturbances and recovery has not been addressed in orthognathic surgery. Using neurophysiologic monitoring during 19 bilateral sagittal split osteotomy operations, we were able to classify intraoperative inferior alveolar nerve injuries as either axonal or demyelinating. This study aimed to analyze the quality and extent of the subjective sensations experienced by the patients after these 2 injury types at different time points up to 12 months.
MATERIALS AND METHODS: Of the 36 injured nerves, 21 showed signs of demyelinati
