The use of an endothelium-targeted cationic copolymer to enhance the barrier function of lung capillary endothelial monolayers.

Acute changes in lung capillary permeability continue to complicate procedures such as cardiopulmonary bypass, solid organ transplant, and major vascular surgery and precipitate the more severe disease state Adult Respiratory Distress Syndrome (ARDS). To date there is no treatment targeted directly to the lung microvasculature. We hypothesized that biomimetic polymers could be used to enhance passive barrier function by reducing the porosity of the endothelial glycocalyx and attenuate mechanotransduction by restricting the motion of the glycoproteins implicated in signal transduction. To this

Comminuted periprosthetic humeral fracture after reverse shoulder prosthesis.

Periprosthetic fractures represent a severe complication after joint replacement. A case of comminuted displaced humeral fracture around the stem of a SMR reverse shoulder prosthesis is reported. The patient was a 81-year-old lady who had had a total shoulder replacement 28 months previously. The surgical solution consisted of a partial revision of the modular implant with conservation not only of the glenoid component but also of the prosthetic humeral body, which was well fixed in the humeral metaphysis. The humeral stem was removed and a long uncemented revision stem was implanted providing

Advanced Mechano-Electrical Coupling Biomaterials for Orthopedics: From Fundamental Mechanisms to Clinical Translation.

Musculoskeletal tissues are continuously subjected to mechanical stimulation during physiological activity, accompanied by the generation of endogenous bioelectrical signals. Such mechano-electrical coupling plays a pivotal role in tissue development, homeostasis, and repair. However, injury or pathological conditions often disrupt or weaken the local bioelectrical microenvironment, thereby limiting regenerative outcomes. Driven by the need to reconstruct the electrophysiological microenvironment, mechano-electrical coupling strategies based on piezoelectric, triboelectric, magnetoelectric, an

Effectiveness of Low-Level and High-level Laser Therapy for Rotator Cuff-Related Shoulder Disorders: A Systematic Review and Meta-Analysis.

Introduction: While photobiomodulation and high-power lasers are common for rotator cuff-related shoulder pain, evidence interpretation is hindered by inconsistent dosimetry and clinical findings. This systematic review and meta-analysis evaluate the effects of low-level (LLLT) and high-level laser therapy (HLLT) on pain, range of motion (ROM), and shoulder function. Methods: This PROSPERO-registered review included randomized controlled trials comparing LLLT or HLLT against sham, no treatment, or conventional care. Two reviewers independently conducted screening, data extraction, and risk-of-

Connecting research to the needs of patients and clinicians.

Modern principles of gait rehabilitation after CNS lesions favour a task-specific repetitive approach, i.e. who wants to regain walking has to walk. Budget constrains and the required effort, e.g. to place the paretic limbs in SCI patients, limit the treatment intensity; accordingly gait machines have been introduced. One can distinguish between an exoskeleton and end-effector approach. Our group opted for the latter, less costs, faster donning and doffing as well as the potential risk of shear forces in case of a malignement between the external and internal joint axis were argumented. On the

Effects of temporary functional deafferentation on the brain, sensation, and behavior of stroke patients.

Following stroke, many patients suffer from chronic motor impairment and reduced somatosensation in the stroke-affected body parts. Recent experimental studies suggest that temporary functional deafferentation (TFD) of parts of the stroke-affected upper limb or of the less-affected contralateral limb might improve the sensorimotor capacity of the stroke-affected hand. The present study sought evidence of cortical reorganization and related sensory and motor improvements following pharmacologically induced TFD of the stroke-affected forearm. Examination was performed during 2 d of Constraint-In

Whole-body vibration to prevent intensive care unit-acquired weakness: safety, feasibility, and metabolic response.

BACKGROUND: Intensive care unit (ICU)-acquired weakness in critically ill patients is a common and significant complication affecting the course of critical illness. Whole-body vibration is known to be effective muscle training and may be an option in diminishing weakness and muscle wasting. Especially, patients who are immobilized and not available for active physiotherapy may benefit. Until now whole-body vibration was not investigated in mechanically ventilated ICU patients. We investigated the safety, feasibility, and metabolic response of whole-body vibration in critically ill patients.

Conservative medical intervention as a complement to CDT for BCRL therapy: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: The effect of first-line complex decongestive therapy (CDT) for breast cancer-related lymphedema (BCRL) depending on various factors forces patients to seek additional treatment. Therefore, this meta-analysis was conducted to evaluate the effect of different conservative medical interventions as a complement to CDT. This is the first meta-analysis that includes various kinds of conservative treatments as adjunctive therapy to get broader knowledge and improve practical application value, which can provide recommendations to further improve BCRL patients' health status. METHODS: RC

Pulmonary embolism prophylaxis in more than 30,000 total knee arthroplasty patients: is there a best choice?

Prophylaxis for pulmonary embolism (PE) prevention in total knee arthroplasty remains controversial. A joint registry evaluated venous thromboembolism prophylaxis and anesthesia impact on the incidence of PE, fatal PE, and death. Patients received mechanical prophylaxis alone or chemical with or without mechanical prophylaxis. The overall PE incidence was 0.45%; fatal PE, 0.01%; and death, 0.31%. The only significant difference in any outcome was the incidence of PE between Coumadin and mechanical prophylaxis alone. Variables associated with a higher incidence of PE were age, an American Socie

Stroke rehabilitation reaches a threshold.

Motor training with the upper limb affected by stroke partially reverses the loss of cortical representation after lesion and has been proposed to increase spontaneous arm use. Moreover, repeated attempts to use the affected hand in daily activities create a form of practice that can potentially lead to further improvement in motor performance. We thus hypothesized that if motor retraining after stroke increases spontaneous arm use sufficiently, then the patient will enter a virtuous circle in which spontaneous arm use and motor performance reinforce each other. In contrast, if the dose of the

A randomized single-blinded trial of early rehabilitation versus immobilization after reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) does not have a standard postoperative protocol. Although instability is a worrisome complication, prolonged immobilization may also be disabling in the elderly population. This study aimed to determine if early vs. delayed range of motion (ROM) after RTSA affected postoperative ROM, patient-reported outcomes, and the dislocation rate. METHODS: A single-blinded, randomized controlled trial was performed enrolling patients from 2013 to 2017. Patients were randomly assigned to either a delayed-rehabilitation group (no ROM for 6 weeks) or ea

Two-stage late reconstruction with a fresh large osteochondral shell allograft transplantation (FLOCSAT) for a large ostechondral defect in a non-union after a lateral tibia plateau fracture 2-year follow up.

This is the description of a 58-year-old female patient presenting 8 months after a horse riding accident with significant pain and inability to walk independently. Imaging revealed a large osseous defect of the lateral tibia plateau which was not united posteriorly. The patient refused knee replacement and we developed a patient specific two-step procedure for her. Step 1: Filling of the defect with a large cortico-cancellous autograft from the posterior iliac crest; step 2: Transplantation of a fresh large osteochondral shell allograft (FLOCSAT). The postoperative protocol included continuou
